Collaboration, improved access to online libraries, research tool transparency, a common data language, organizational and informational support, and continued research are a short list of needed items to advance translational research. Technology advancement in the area of incorporating evidence into clinical tools must continue, along with removing barriers to knowledge-seeking behavior (McGonigle & Mastrian, 2018). Describe ways in which you can remove these barriers and provide access to evidential resources that will promote knowledge and, in the end, improve patient outcomes in your practice setting.
Thank you for sharing!
Take care,
Dr. B

Hello class!
Quality measurement provides tangible feedback to clinicians and other health care team members about care processes and outcomes. Teams can use this feedback to identify opportunities to improve care and outcomes for their patients. How have you seen this or participated in this process of enhancing care through measuring results of current processes and practice patterns in your current or past facilities?
Thank you for sharing!
Take care,
Dr. B

When I was working on a medical-surgical unit in the hospital we found that our hospital wide rates of central line infections were increasing. This is tracked as part of the reportable data that CMS requires of all hospitals, and now impacts reimbursement to the hospital. The desire to decrease these rates was important both from a reimbursement perspective and also from a patient outcome perspective. After reviewing the most recent research, the hospital opted to create a central line team that became solely responsible for all central line dressing changes. The staff nurses were also taught to start using alcohol swab caps on all central line ports and ends of IV infusion lines. The combination of these two interventions were successful in decreasing the central line infection rate for the hospital. The staff nurses appreciated that the interventions implemented were not solely giving the nurses more tasks, and instead looked at the hospital as a whole and developed a plan that incorporated multiple different staff in working towards a common goal.

In our facility quality measurement is what helps us make sure that our patients are maintaining their health. We have weekly weights as a measurement that indicates a person’s nutritional status and ensures adequate food and water intake. If a patient is noted to have lost weight, we look in their EHR so see what their diagnosis is. Sometimes a person who has heart failure will fluctuate up or down in their weight related to fluid overload. If a person’s weekly weight measurement is decreasing we look for documentation about how much of their daily meals they consume, also found in the EHR under patient’s daily charting by the nurses and CNAs. We have had good patient outcomes because our facility monitors this very closely.

Hello class!
Effective use of health IT helps make the right information available to the right people at the right time, which can fuel an upward spiral of continuous improvement, moving from current care toward optimal care.
Can you think of how your facility or a facility you have worked at in the past has identified and implemented an improvement process through utilization of health IT? What IT system was utilized and how did the improvement move care toward optimal patient care?
Thank you for sharing!
Take care,
Dr. B

At the current hospice home care agency I work at we implemented the use of a HIPAA compliant messaging system called Qliq about a year ago. This system can be accessed on any laptop or mobile phone and allows all staff in our office, both clinical and non-clinical, to communicate about patients instantly. Prior to the use of this technology, the only way we could communicate about patient information was either in person, over the phone, or through a specialized type of email within the electronic health record. With most of our patients in their own homes and our nurses constantly in the field unable to always be logged into our electronic health record, nurses were often waiting for long periods of time on the phone trying to speak with other members of the team working out of our office. Once they were able to access Qliq from their phones, they could easily message back and forth with managers and physicians in our office about simple patient questions or issues. This technology also allows our nurses to send a quick message for non urgent issues in a matter or seconds, instead of having to wait until they are able to access the electronic health record again. This technology also allows our nurses to spend more time caring for patients and families since they spend less time being on the phone with our office. More time spent with patients and families ultimately leads to better patient outcomes and higher satisfaction from our patients and families.

Dr. B,
Health IT is truly an amazing aspect of healthcare. I have had the opportunity to see different health IT being implemented and utilized at my organization. The most beneficial and notable implementation was our CareAware Connect system. Prior to its implementation we had phones that were outdated and about half of our phones the caller ID didn’t work, leading to the inability to

return important phone calls. Last year our organization implemented the CareAware phones to our hospital. Not only could the phones receive phone calls it could access our patient’s chart and even barcode medication. Prior to the utilization of the phone a patient’s wounds would need to be documented and uploaded by a camera which half the time was broken. With the new CareAware Connect wound pictures could be taken and then uploaded using the phones. The CareAware phones also has messaging leading to a decrease in phone calls for staff. The utilization of this technology positively affected patient care and workflow. As a nurse I can quickly access a patient’s chart to see current labs and medications. I can provide my patients with more efficient care without the need to constantly go to a different location to access information. It was a major adjustment in my workflow, but I am so grateful for the opportunity to utilize such a beneficial tool. I feel as though I can provide my patients with more effective care because of this new technology.
