It could lie within how the medication was transcribed by the physician, or how the medication is being administered by the nurse, or the preparation and delivery of  the medication to the nursing unit(Irajpour et al.,2019). I can recall when medications were coming to the nursing h if and needed to be reconstituted at the time of delivery. Some nurses were not reconstituting the medication because they missed the inservice and was failing to deliver the dose. With the next dose due the nurse would discover the bag of medicine not reconstituted. Discovery of a medication that could cause great harm to a patient, especially if the following nurse did not follow up on the missing dose. The third problem lie in that the reconstituted medication was not dissolved in the amount of fluid the order was written. If all three professions were collaborating together the medication issue could have been avoided or discovered before the patient condition deteriorated to sepsis.
