The MSN specialty track that I am currently enrolled in is the Family Nurse Practitioner (FNP). The problem that was focused on was type two diabetes in the thirty to fifty (30-50) year old age group. Type 2 diabetes is a concern to nursing and healthcare because it is a global pandemic. The scope of the concern is the maintenance of a healthy hemoglobin A1C value and/or have a reduced A1C in diabetic patients (Monteiro-Lago et al., 2019). The evidence that indicates that there is a need for change is that according to the World Health Organization (WHO), the number of patients with diabetes have drastically increased by over three hundred million ( 300,000,000) from 1980 to 2014; also, in 2016 there was an estimated 1.6 million diabetic related deaths (Shiau, So & Dent, 2018). Also, according to WHO diabetes is the major cause of kidney failure, stroke, amputation, heart attack and blindness; with proper control of patients’ hemoglobin A1C values, these types of deaths can be reduced and even avoided (Spencer et al., 2018). In most cases, there is higher HbA1c levels on the diabetic obese patients compared to non-obese patients (Zayr et al., 2016).

In type 2 diabetics between the ages of thirty and fifty (30-50) years old, how does the effect of a reduced daily caloric intake of one thousand five hundred to two thousand (1500-2000) and one thousand two hundred and one thousand eight hundred (1200- 1800) n for males and females respectively and a thirty (30) minute increase in physical activity affect hemoglobin A1C compared to those without diet or activity modification within an eight (8) week period.

I:        Reduced daily caloric intake of 100 to 2, 000 for males and 1,200 to 1,800 for   females and an increased physical activity of at least 30 minutes daily.

Key Stakeholders

Key stakeholder in the evidenced-based practice change project are medical professionals including nurses, physicians, caregivers, as well as the community members who are suffering from type 2 diabetes. Also, healthcare insurance companies and the government institutions particularly the departments of health will be some of the stakeholders. Theoretical framework refers to the structure that can support or hold theories for the evidenced-based practices. It often introduces and describes theories that elaborates on why the research problem under the study exists. In this section, there will be explanations of different theories surrounding the evidenced-based practices when it comes to the management of type 2 diabetes. There will be

analysis of theories involved in the management of type 2 diabetes as well as the control measures that may be employed to deal with the causative factors such as obesity (Bailey & Locke, 2015). Literature Review

There will be the discussion of different literature sources about type 2 diabetes. The sources of literatures will incorporate theories as well as the past qualitative and quantitative research articles, books, journals and even the websites. Different Research from World Health Organization will mostly be employed in elaborating some of the causes, management, treatment, and prevention of type 2 diabetes. Also, this sources of information will be used to identify the population that is most vulnerable to type 2 diabetes. Literature review will be written in line with the research questions and objectives. In other words, the literature review will involve theoretical explanation and elaboration on the research objectives. Here there will also be answering of the PICOT question in line with the previous studies and literatures published in different times. There will be focus on self-care areas namely healthy eating, exercises, blood glucose monitoring, medication intake, healthy coping, problem-solving and reducing risks. There will also be the examination of the work by Rallis (2019) which analyzes the optimization of glycemic control in type 2 diabetic patients through the use of a low-carbohydrate, high-fat, and ketogenic diet. Controlling diet often achieve a significant reduction in body weight. In other words, a very-low-carb, high unsaturated fat, low-saturated fat diet with a high-carb, low-fat diet have positive impacts on glycemic control (Tay et al., 2015). Another piece of literature that will be of great interest is “Fiber In Diet Is Associated With Improvement Of Glycated Hemoglobin And Lipid Profile In Mexican Patients With Type 2 Diabetes” written by Velázquez-López et al., (2016) which analyzes the relationship between the consumption of proteins with enhanced control of blood sugar levels. The journal by Hatamoto (2017) also proved to be an important resource as it define the relationship between physical exercise and type 2 diabetes.

The data will be collected from the laboratory tests through the use of questionnaires. The participants of the study will mostly involve diabetic patients who will be randomly selected for the study. The figures as well as the information collected will be recorded in the Excel spreadsheet. Some of the data that will be collected will include weight, age, level of type 2 diabetes, gender, A1c levels in the body, self-care efficacy and intervention methods used among the patients with diabetes. Data cleaning process will be done to remove outliers or extraneous variables.

Analysis

The participant characteristics in the usual care group and intervention group will be compared using the Pearson x2 test for categorical variables and student t-test for continuous variables. The means HbA1c levels in the two groups will be calculated and then compared to establish there are significant differences between the two groups. The researcher expects that the intervention group will offer better results compared to the usual care group. The data will be calculated using the SPSS software. Other aspects compared will be self-efficacy, BMI and self-care. Intervention effects will be evaluated using linear mixed models that will adjust repeated measures using terms for time (3, 6 and 9 months), time-by treatment arm interactions and treatment arm. The participants with available follow-up measures will be analyzed within their assigned group using the intent to treat principles.

Expected Outcomes

The study is expected to reveal that self-efficacy care is one of the best approaches in managing type 2 diabetes. Diabetes self-management education has significant impacts on the Body Mass Index which is a major factor in the development of type 2 diabetes (Chen et al., 2016). There is a close relationship between type 2 diabetes and obesity. Possessing a higher amount of body fat usually increases the chances of developing diabetic conditions. There was a strong link between break down of the HLE in DPP4i monotherapy with dietary serum ferritin intake in type 2 diabetics (Kuwata et al., 2018).
