The purpose of this paper is to provide an in-depth analysis of incivility in nursing care, its impact on healthcare, and how to cultivate a healthy nursing environment.

Incivility is defined differently by various authors, however, the definitions reflect a common idea. According to the American Nurses Association (2020), incivility is a rude and disrespectful behavior committed by individuals that are against organizational norms and has an ambiguous intent to harm.  On the other hand, Layne, Anderson, and Henderson (2019) define incivility as a deviant behavior in the workplace. Examples of incivility include taking in an unprofessional language, spreading unnecessary rumors, gossiping, intentionally ignoring someone, or interrupting a colleague during a meeting. Incivility is an issue because it might create an environment where staff members do not support one another making the healthcare delivery process very complicated (American Nurses Association, 2020). Based on the fact that nurses work in a stressful environment, and that the situation is between death and life, incivility becomes a significant issue in the system.

Normally, the elements of this issue are often observed in the hospital environment, and it is very difficult, almost impossible, to eliminate incivility at the workplace. Due to many risks and stressful situations associated with nursing, both the providers and patients may feel frustrated, which may lead to them acting differently (Phillips, Stalter, Winegardner, Wiggs, & Jauch, 2018). With this kind of understanding, it is possible to reduce this issue by implementing both primary and secondary interventions. Additionally, incivility is an issue because it might create disagreements between healthcare providers and patients, or between healthcare providers themselves leading to conflict. Addressing this issue is one of the ways of ensuring quality care (Layne et al., 2019). Incivility is a common issue in nursing and has been increasing over the years. It can impact negatively the working environment, people, and quality of care. This makes it an important issue in the nursing arena. When the situation sets in, the providers and the patients may first bear the outcome.  However, when it gets worse, the organization finds it difficult to obtain the quality of healthcare resulting in undesirable patient outcomes (Layne et al., 2019). The importance of incivility related to nursing, therefore, manifests itself when it bothers nursing operations and patient outcomes in the healthcare setting. Incivility can have adverse impacts on individuals, for instance, victims who experience rude or disrespectful behaviors often lose interest in their jobs.

Additionally, they may appear mentally disturbed, show up to work late, and even quit their job when the situation persists. Phillips et al. (2018) assert that such individuals may also experience a feeling of stress and humiliation. The authors stated further that employees in all industries have indicated that public humiliation often makes them feel small and impacted negatively by their self-esteem.  Furthermore, incivility may make some nurses fall sick due to stress and a feeling of low self-esteem. It also impacts negatively on their performance making nurses receive bad ratings in the performance appraisal. Such ratings along with those negative experiences reduce their level of commitment further. Other emotional impacts on individuals due to incivility include isolation, anger, and fear.

Reflecting on the impacts of incivility on individuals, it is clear that the situation also has a significant influence on microsystem work environments. For instance, if the staff is not happy, they might not work in collaboration, which reduces the

quality of healthcare. Phillips et al. (2018) define clinical microsystems as a small group of professionals who work together regularly, or as needed, to provide care to discrete populations of patients. Incivility breaks the bond between these professionals making the microsystem operations impossible. It compromises communication and trust in a microsystem workplace environment. Lack of closed-loop communication and trust can lead to strain in the workplace. The strain in the workplace is known to influence teamwork behaviors such as coordination and monitoring. Such strain is likely to diminish microsystem workplace environment performance and efficiency (American Nurses Association, 2020). Therefore, incivility impacts negatively on the nursing profession, by inducing bad experience on nurses, healthcare performance, and microsystem environments. Proper strategies can be designed and implemented to prevent incivility in nursing care.

Storytelling

I first encountered the incidence of incivility at my workplace when a team of nurses and clinicians working together has disagreements. One of the nurses was unnecessarily spreading a rumor that a clinician was giving fake prescriptions to patients, which was not the case. The physician was very furious upon hearing this developing story. The normal care coordination could not go on well as the workplace environment turned in a conflict arena. There was no effective communication between the teams and performance of work on that day was negatively impacted as the two openly exchanged words and could even involve in a fight. Both the clinician and the nurse were angry at each other. The clinician looked stressed and low self-esteemed. The managers from both teams had to come in to solve the conflict before it got worse. However, because they could not get along and it was impossible for them working in collaboration. The nurse had to switch her shift with another nurse for sometimes until they shall have agreed with each other.

Creating a Healthful Environment

The creation of a healthy environment is the key to successful healthcare delivery. The first strategy for a healthy environment is the establishment of effective communication. Healthcare is about teamwork, which involves constant communication to share, discuss, and pass information. Furthermore, effective communication may help solve disputes or disagreements among the collaborative teams (Almost et al., 2016). Communication enables the establishment of a therapeutic relationship between the provider and the patient which makes a workplace better for care delivery. The second strategy includes employee engagement.  Inclusivity in the decision-making process and leadership activities is the best way to keep everyone motivated.

People have different ideas and opinions and would like them to be heard. Engaging employees in a decision-making process and other organizational events make them poses the sense of the owner. It makes them feel wanted and important, which, in turn, boosts their self-esteem (Phillips et al., 2018). The third strategy is to promote coordination among teams because that is what healthcare is about. Healthcare is a process between various departments and units, hence, effective coordination will result in quality patient outcomes. It makes operations run smoothly and in the end, the organization can achieve its goals.

The selected specialty track is the executive MSN. A nurse executive is a nursing administration leader working in the management and administrative aspect of patient care. Areas of focus or in this specialty track include collaborating with other professionals to develop partnerships and networks (Tanaka, Taketomi, Yonemitsu, & Kawamoto, 2016). Nurse executive work in hospitals, community health agencies, and healthcare organizations. To be a nurse executive, one must obtain a master’s degree in nursing to ensure that the candidate is prepared in a fast-paced administrator role. Some of the roles of an executive nurse include designing and managing patient care, shaping health care policies at the facility or organization. Creating and managing budgets, and Supporting the team to provide optimal patient care among other things (Tanaka et al., 2016). Skills needed in this specialty track include leadership, communication and relationship management, and business skills.

To ensure a healthy working environment, it will be necessary to comply with the organizational rules and regulations which include respect for all. It is also important to work within the ethical responsibilities of a nurse (Layne et al., 2019). Nursing ethics promotes behaviors that encourage a healthy working environment to prevent incivility and other undesirable traits. These along with effective communication and coordination will help achieve a healthy workplace environment.

Incivility and Healthful Environment

Incivility is one of the problems which affect the healthcare environment negatively. While some nurses get involved directly in incivility, others experience it as it happens between other staff members. Even though it is a common phenomenon, it sometimes gets overlooked. Incivility has been shown to lead to toxic working environments and poor working conditions. Incivility may take different forms, such as bullying, disruptive conduct, unfair conduct, subtle harassment, and rudeness (Green, 2019). Therefore, the purpose of this assignment is to reflect on an experience of incivility at the workplace.

Some time ago, I experienced a first-hand act of incivility. We were always encouraged in the facility to inquire from colleagues and the senior staff members if there was anything unclear or when in doubt. Therefore, as a way of avoiding medication errors, I approached one of the senior nursing staff to help confirm the details of a medication of I was supposed to administer to a patient in terms of dosage and route. To my surprise, the nurse reacted angrily and rudely, shouting that it was a show of incompetence. The nurse even criticized me in front of others, shouting that my skills were short and that I could not handle the simplest of tasks.

The situation made me feel bad, disrespected, and demoralized. The comments of incompetence and half-baked skills cut into me and bruised my self-esteem. I thought for a while about my competence and ability concerning the skills. I don’t think I did anything wrong to warrant such a dress-down. I was only trying to be extra careful so that I don’t commit any medication errors which can lead to adverse outcomes.

Even though the situation made me feel bad, I contained my anger and walked away from the scene. I did not want to create any commotion as patients were close by. I spoke with another staff member who helped countercheck the medication details. The nurse also indicated that the reaction by the senior nurse was inappropriate and uncalled for and that I did well not to respond violently. I made efforts to meet the nurse leader to hold further discussions regarding the matter and how best to handle such situations in the future.

The situation had various consequences. One of the consequences was a strained relationship between myself and the senior nurse. Since I didn’t like how the nurse addressed me, the situation made our working relationship to get sour. Again, the nurse leader was also willing to come in and mediate to ensure the working relationship was restored. Indeed, mediation is known to be one of the most strategies for resolving conflicts (Kohlhoffer-Mizser, 2019).  The situation also had negative impacts on the work environment and the outcomes. For example, other junior nurses were more withdrawn and never willing to inquire and consult the senior nurses. There is also a potential impact on the outcomes since there are higher chances of making mistakes, such as medication-related errors, when there is no consultation.

The situation could have been prevented by constantly reminding the staff to be gentle and avoid rudeness as much as possible. Talking gently to others helps avert conflicts that could have been caused by rudeness. Various strategies can be used to support a healthy work environment. One of the strategies is encouraging open communication. Open communication helps individuals pass across the intended information and message, preventing conflicts and promoting a healthy environment (Kim et al.,2018). The other strategy is offering support for each other in the work environment. Support ensures that everyone feels comfortable in the work environment, creating a healthy environment.

For your response post, you do not need references as personal experiences would count as response posts. If you however cite anything from the literature for your response post, it is required that you cite your reference. You should include a minimum of THREE references for papers in this course. Please note that references should be no more than 5 years old except recommended as a resource for the class. Furthermore, for each discussion board question, you need ONE initial substantive response and TWO substantive responses to either your classmates or your instructor for a total of THREE responses.
