The definition of an Advanced Practice Registered Nurse (APRN) according to the American Association of Colleges of Nursing (AACN) indicate master’s-prepared nurses who provide direct clinical care (DeNisco, 2024). This term encompasses the roles of nurse practitioner (NP), certified nurse–midwife (CNM), certified registered nurse anesthetist (CRNA), and clinical nurse specialist (CNS) (DeNisco, 2024).
The advanced practice role has existed in nursing for over 40 years (Lewis, 2022). This role initiated in the 1960s in the United States and Canada as an effort to provide and expand high quality healthcare. During the 1970s, the Advanced Practice Nurse title and jobs started to expand to other countries. As a result, the advanced practice nurse (APN) title became more apparent in the nursing literature.
Currently, the largest group of advanced practice nurses are the nurse practitioners. This group includes over 355,000 licensed and with some level of prescriptive authority in all 50 states and the District of Columbia (DeNisco, 2024). Nurse practitioners have been providing care to vulnerable populations in rural and urban areas since the 1960s (DeNisco, 2024). The role was born out of the shortage of primary care physicians able to serve pediatric populations (DeNisco, 2024). Initially, there was a 3 to 12 months of educational requirement to prepare for the nurse practitioner role. The role of nurse practitioners increased and so did the educational requirements. By the 1990s, the master’s degree was endorsed as entry-level education for NP specialties (DeNisco, 2024). Currently, nurse practitioners are not only educated and working, they are also board certified in different specialty areas such as pediatrics, family and adult-gerontology.
In what ways will the advanced registered nurse role and responsibilities continue to evolve and emerge as the American health care system changes?
In my 20 years working as a nurse, I have seen nursing roles evolve along with health care system changes. One of the health care system changes is to improve access to high quality healthcare while reducing costs of which is part of a certified nurse specialist’s (CNS) role. These health care system changes are developing new roles including nurse navigators and health care coaches which in the future may require advanced degrees and certifications (DeNisco, 2024).

Throughout history, advanced registered nurses in healthcare have been essential for ensuring patient safety and care. However, their roles have changed over time due to various technological, professional, and government changes. Undoubtedly, the responsibilities of advanced registered nurses have evolved as the dynamic healthcare industry necessitates swift adaptation and decision-making based on sound reasoning. The caring approaches of these nurses will continue to evolve, driven by the changing dynamics of the American healthcare system.
A national plan aims to enhance healthcare access and reduce costs, leading to the creation of new, unforeseen nursing jobs. Recently introduced roles like care coordinators (such as nurse navigators and healthcare coaches) may require additional degrees and certifications in the future. With an aging population, there will be increased demand for nurses specializing in chronic illness and wellness coaching. Specialized roles like population health, gender-specific healthcare, and global health specialists are expected to become more common. Efforts are underway to expand and clarify the definition and standards for advanced practice nursing (DeNisco, 2024).
The Affordable Care Act of 2010 (ACA) will bring both challenges and chances for health professionals to build a more patient-centered system.
