You are required to conduct an assessment of a family including health history, health lifestyle, and health risk for a child, an adult, and an older adult. The health risk assessment must include one pandemic-related health risk for the family. Because you are not permitted to interact with any individuals younger than 18 years of age, you will need to retrieve all information about the child through the parent(s). The focus of the family assessment is health and wellness across the lifespan.

For this Discussion Board, please develop 3 lists of questions based on the family you will assess. Each list should have 15 health assessment questions (45 questions total) to assess the health and wellness of the family (one for the child that is based on the age of the child, one for the adult, and one for the older adult). You will use these lists as the basis for the family assessment.

Some common health assessment questions ask about the following:

  • Tobacco use
  • Stress
  • Healthy eating
  • Physical activity

  • Sedentary behaviors, such as sitting and watching TV or playing computer games
