Nursing profession is a critical part of all the healthcare systems. The nurses are tasked with a noble responsibility of helping the patients through the health-illness continuum. Therefore, in order to promote safety of patients and improve the quality of healthcare services, it is imperative for all the stakeholders in the healthcare systems to accord the nurses the support they need to deliver their services. The support starts from the nurse training institutions, where nurses are well trained with relevant skills that match with the emerging healthcare systems. Additionally, the training of nurses should be a lifelong endeavor to achieve quality care. The nurse empowerment initiatives are also crucial to help the nurses be engaged actively in the process of healthcare policy formulation and implementation.
The adoption of the report by Institute of Medicine (IOM) on the future of nursing is a great milestone in promoting the nurse empowerment agenda. An empowered nurse is able to have a direct positive impact on the quality of healthcare services and enhance patient’s safety. This paper aims to discuss the report by IOM on the future of nursing in consideration to the work of the Robert Wood foundation committee that lead to the development of this report. Highlighting the key messages in the report, role of the state-based action coalitions in the implementation of the report, the barriers experienced and how to overcome them.
Description of the Work of the Robert Wood Foundation Committee Initiative that Led to the IOM Report
The work of the Robert Wood Foundation Committee on the future of nursing was prompted by the glaring challenges that were being faced by the nurses worldwide in their line of duty. The nursing profession as delicate as it is facing a number of complex challenges that have been compromising on the quality-of-care services provided. This put the health of the population at a greater risk if the challenges are not well addressed and the nursing needs met with uttermost concern. It is these challenges and the quest to have a more vibrant, well equipped and empowered nursing force across the US that shaped the Robert Wood Johnson Foundation (RWJF) committee to look into the nursing practice in a number of healthcare systems settings across the US. In the assessment report revealed some glaring disparities in the nurse competencies and the academic credentials across all the states. The reported emphasized on the need for some states to allow nurses to practice to the full extend of their training and competencies in order to promote accessibility to quality care across the country. Additionally, the report emphasized on the need for the nurse education and training to be made more relevant, advanced and strengthened to match the emerging healthcare trends and needs worldwide (National Academies of Sciences, Engineering, and Medicine, 2016). The committee’s fact-finding missions and the recommendations on the future of nursing were crucial to the shaping of the healthcare systems across the US.
It is through the initiative of the RWJF that lead to the development of the IOM report titled,” The Future of Nursing: Leading Change, Advancing Health.” The report was in itself a well-researched evidence-based report that had detailed plans on how to promote better nursing experiences and practice for a better future in the healthcare industry. The report main areas of target included the nurse training reforms that would see more nurses advance in their level of academic training to have more nurse with at least a baccalaureate degree and above. There was also need to have more flexible nurse training programs that will be crucial in helping the nurses achieve a lifelong education that will equip them with skills and attitudes to adapt to the emerging trends in nurse practice such as the advancement of technology in nursing.
Key Messages in the IOM Report Recommendations
The key message contained in the IOM report on the future of nursing, leading change and advancing health was built on four main recommendations. First, there was need to allow all the nurse to practice to the full extend of their qualification and academic credentials. The state and the federal governments are supposed to work in tandem to ensure that this regulation is well implemented and the states that have restrictions on the nurse’s scope of practice were to reconsider this regulation and provide a framework on which nurses with relevant training and qualification can deliver the care services required. The second recommendation emphasized on the need for nurses to seek further education and training. This was also meant to allow a smooth transition of nurses to a higher level of academic training and advance on their level of practice with limited obstacles. This was also meant to have a high competent nursing force that will be able to deliver quality care that is relevant to the emerging trends in the healthcare industry. It will also guarantee patients safety and satisfaction (Arzouman, 2016). The third recommendation was on the nurse work partnership with other healthcare professionals. The report emphasized on having the nurses recognized as full partners with other healthcare professionals. It is meant to have the nurses take full responsibility and be accountable to the actions that help promote better nursing practices, improved efficiency, reduced costs and the achievement of the collective organizational goals. Additionally, nurses have to take up active leadership roles and develop core competencies in leading healthcare organizations. Nurse were required to take an active role in crucial healthcare policy formulation and implementations. Nurses were called upon to take up positions in the healthcare boards, committees and commissions. This will help them in advancing the nurse advocacy agenda. The fourth recommendation was on the use of sophisticated data management systems in the healthcare industry. The effective management of data will be helpful in effective planning and the development of appropriate policies (Arzouman, 2016). The report recommended on having a collective database that will have all the information on the nurses from the total population of the nurses, their roles, employment data in all the areas of nursing. This will provide critical information on planning and future of the nursing decisions based on verifiable information.
Role of State-Based Action Coalitions
The US federal government system allows the states some autonomy to manage their healthcare systems. The healthcare programs, promotions, laws and regulation are controlled at the state levels. The development of the state-based action coalitions was meant to help improve the participation of the nursing professions, public, private and the government-based healthcare organizations in the implementation of the IOM report on the future of nursing, leading change and advancing health. These coalitions are guided by the RWJF and the Centre to Champion Nursing in America (CCNA) guidance to ensure the delivery of universal standardized healthcare services across all the states These organizations coordinate the sharing of the information among the inter-state healthcare organizations. The IOM report on the future of nursing, leading change and advancing health is a crucial step forward in reconciling the disparities that exist in the nursing standards across all states and create a collective and more vibrant nursing profession for the future practice. The recommendations of the report in their entirety focus on the quality of healthcare services delivered countrywide and promote safety of the entire population. The key recommendation of restructuring the nurse scope of practice, nurse education and training, use of modern data collection, storage and analysis in nursing, points to the endeavor of the IOM and the RWJF committee to revolutionize the nurse practice and the call on nurses to take advanced roles in both care and advocacy roles. Nurses are called upon to take active leadership roles that would shape the future of nursing practice and the entire profession.
