Clinical nurse specialists (CNS) possess advanced training within their specialty and are capable of working in a range of healthcare fields such as geriatric care, adult health, and community health as well as pediatric care in settings such as critical care (Cooper et al., 2019). Clinical Nurse Specialists are responsible for the conduct of physical examinations, medical history gathering, and the coordination of patient care.

Both CNL and CNS strive to improve the quality of care and satisfaction of patients. Although there are several differences between the two, the primary difference is in their approach. A CNL focuses on one specific issue in a system while a CNS takes a broader view of the situation. Accordingly, both roles must adhere to the Code of Ethics for Nurses of the American Nurses Association (ANA). Several codes of ethics exist in the nursing profession that specifies the expectations of members and define what constitutes the best performance and conduct in the profession. Nurses should follow the nursing code of ethics in order to clarify their role and relationship with their patients as well as to address issues that contribute to the advancement of their health (Haddad & Geiger, 2018). Additionally, it is important that healthcare providers provide accurate medical diagnosis, treatment, and prognostic information to patients and work with them to make informed decisions. When a healthcare provider denies a patient’s wishes regarding treatment, they are not exercising their authority over that patient’s values.

As a nurse practitioner, if your action provides no benefit or good effect but does inflict intentional harm, then you are violating the rule of non-maleficence. Beneficence, on the other hand, promotes action that will benefit others. These two ethical guidelines remind us that we, as healthcare professionals, must act in a manner that nurtures or help others and, simultaneously, protects that person from harm.

Whether in advanced practice or as a regular bedside nurse, work ethics among nurses don’t change. We must protect and serve the interests of our patients at all costs. Let us take non-maleficence and beneficence, for example. CRNA, for instance, are given a choice to use general anesthesia vs. moderate sedation. Using their knowledge, skills, and expertise and considering the patient’s preferences and best interest, a CRNA may choose the milder kind of sedation as a standard choice to anesthetize a patient for a left heart catheterization. The situation may escalate and require general anesthesia. Still, considering the patient’s age, physiologic functions, and pathologic conditions, their decision has to be in the patient’s best interest and outcome.

An FNP may be faced with a situation wherein she has to prescribe antihypertensive medications. A patient may not fully understand hypertension and may be hesitant to take any BP meds. Their age and education, and financial background may also sway them into noncompliance in taking their medications. An FNP using their knowledge, expertise, and interpersonal skills, may choose a drug that is easy to take to promote compliance, cost-effectiveness, and simplicity of regimen for the benefit and overall health of the patient.

The key to doing our fair share in uplifting the lives of others is to uphold our oath as nurses to protect and serve our patients at all costs.
