I agree with you that opioid misuse is a large problem here in the U.S. It has gotten into such a problem that many providers became afraid to prescribe narcotic medications to those that need it. Then people started to get it off the streets such as fentanyl but the fentanyl on the streets contains other drugs that have killed many people. This has led to some patients being afraid for a nurse to provide narcotic medication for them when they are in pain. I have seen this many times when taking care of patients post-surgery. They would require education that they are being monitored and the narcotic medication would be very little with just enough to stop their pain so they can rest.

We also see this in hospice patients. Patients and their families are scared to take pain medications and we have to educate them on their disease process and how likely pain may occur at that time. We also have to educate them on their addiction concerns and end of life. Patients will have end stage illnesses and be in severe pain, but be so scared of taking pain medication because they are afraid of becoming addicted. I have seen patients with stage four cancer with metastasis to many other areas, suffer with pain all the way to the end because of these fears. Education is our greatest tool for these patients and we try everyday to make sure they know how pain and these medications affect their prognosis and their quality of life.

A group of nurses who all agree on the same issue can have a great impact on change. “Nurses have always been the patients’ advocate by providing proper care in a safe environment” (Helbig, 2018). The safe staffing for nurse and patient safety act is a bill that requires hospitals to establish a committee, composed of at least 55 percent direct care nurses, to create nurse staffing plans that are specific to each unit. Patients risk longer hospital stays, increased infections, and avoidable injuries when units are understaffed. Understaffing also leads to a decrease in nurse retention, higher rates of injury, and burnout (ANA, 2018). I think that each hospital needs to be on the same staffing grid and it should be universal, depending on the area you work in. Studies have shown that safe staffing saves lives. Lower nurse-to-patient ratios are linked with lower mortality rates. This legislation directs hospitals to develop and implement nurse staffing plans that ensure quality care and patient safety.

As a float nurse working in one of the bigger hospitals in my area, I appreciate the staffing grids although they are not always followed. It seems that each hospital has a different grid to follow and certain nurses are only allowed 4 patients max at one hospital during day shift but at another hospital the max is 6 or 7. Having 6 patients as a nurse is too many. Patients are becoming more and more sick when they are coming into hospitals which require a lot more care and time. With 6 patients you have minimal time to spend with your patients.

From my own experience as a PRN RN working in different clinical settings, I have found that assessments from nurses are not accurate in settings where 5 or more patients are assigned. Usually this is the case for physical rehab facilities or SNFs sadly, where I found myself, mostly in SNFs, taking care of 15 patients by myself. In this particular facility, CNAs do not do glucose checks in the morning, and do not keep track of patients VS throughout the day. 13/15 of my patients required G-feedings and crushing meds. It is humanly impossible to perform an accurate assessment when medications are due at 8am, 12pm, 2pm, 4pm and 6 pm for everybody. For this reason, I find imperative introducing a law that forces these facilities to change their ratios, allowing nurses to ACTUALLY interact with patients, note changes in condition, and follow the nursing process accurately, prevent burnout, and sentinel events (White et al., 2019).
