One of the major area of nursing that need a thorough examination is the increased nurse burnout situation especially after the COVID-19 pandemic. There are many factors contributing to the burnout situation such as the workload, job satisfaction , reward and benefits, control , opportunity for professional growth, fairness and values. Many hospitals are now facing huge staffing shortages and the nurse burnout post pandemic is making the situation worse. In the hospital that I work many departments have more than 50 % nurse vacancy and the hospital is struggling to retain staff. Travel nurses are being on boarded to assist with this crisis but have caused increased stress on every department. Increased workload along with other stressors at work results in emotional exhaustion and depersonalization among nurses. There are many studies that has pointed out the correlation between these feeling (emotional exhaustion and depersonalization ) to poor quality of care.( Dall’Ora.,et,al. 2020). When nurses experience high stress and exhaustion at work they tend to make more errors leading to many near misses and patient safety events. Unfortunately maintaining a safe staff ratio is the focus right now for hospitals and health care organization but this issue of nursing burnout must be investigated by health care agencies urgently to improve patient outcome. While the hospitals are focused on recruitment , they need to also work on retaining the staff by assessing their work situation, their mental health situation etc. When quality of care involved, the burnout in nursing becomes a real issue. Organization and health care agencies have to look at these issues and attempt ways to assist nurses deal with these issues

A nursing dilemma I noticed and have been a part of is nurse burnout from staffing challenges to the nurse-patient ratio. The types of patients we are caring for, the demand on nursing is very high, we must take care of the patient/families, etc. Nurses are being burnout from a career that suppose to be a passion, now it just feels like a job that I must go, and you feel like a robot completing tasks sometimes. I have worked in a few different places as a nurse or in the medical field period, I was a nurse assistant, LPN and now RN, so I have worked in many areas and it all boils down to the same thing overworked underpaid. Right now, everybody is having the same problem with staff shortages, which means higher patient-to-nurse ratios, which is a risk to patients and the nursing staff. Nurse patient ratio is a concern for leaders globally it is essential for patients’ safety and quality of care, some places have passed laws to control the nurse-to-patient ratio it is EBP that is beneficial for patients and the healthcare system. (Sharma June 2020) For example, the other day at work all the nurse assistants called off and a nurse, (I really feel like they didn’t try to get anybody into work) and the nurses had to do total patient care (TPC) (which means everything for the patient two completed jobs). We had a high number of patients each of which are heavy critical ill patients on vents etc. it was of course expected for us to complete do both jobs, and if your any type of good person/nurse with a heart you are going to follow your oath and take care of your patients. We were at our whit’s end tired, hunger, stressed, etc. which brings me to my final point nurses wear many hats we are expected to be the nurse, assistant, respiratory, dietary, social work, housekeeping, etc. we are expected to do everything, and, in the end, nobody can do our jobs. There is a study showing that crucial care nurses (other area nurses probably feel the same) are stressed, have depression, anxiety, is burnout which is leading to medical errors which are sad and fatigue, PTSD, and in some cases suicide, lets add Covid pandemic nurses will for sure burnout. (Brusie 2021). It is very important for nurses to have support and time to themselves and in some cases seek therapy.

Victoria great post with great points made! Burnout right now is definitely close to an all time high, if not at the all time high. I remember learning about burnout in school and how it happens after a long period of time and that if one feels burnt out it would be time to change specialties in nursing. I now see multiple nurses, myself included, being burnt out just after a year or under due to the stress of Covid, staffing, and everything else you stated in your post. In 2017 a Nurses’ Burnout article stated “shortage of health care providers is a major concern worldwide” and that was 5 years ago pre-pandemic! Now with the pandemic we have seen more nurses retire than that are hired, and with all of us in the field we see first hand the nurse shortage and the scary ratios we are dealing with. NOW congress wants to cap nursing pay?! “A a wage cap could potentially put both patients and healthcare facilities at risk amidst another COVID-19 surge or even another health emergency” (Brusie, 2022). Even with the pay travel nurses are receiving there is clearly a shortage of nurses as all of us are from different places around the country and we all are talking about the same difficulties. I am scared to see what happens to our staffing situations and to see nurses choose to go down a different path if there becomes a cap on nurse wages.

Yes, Victoria! Nursing has to be one of the most stressful jobs and it is very demanding on the body as well. Our hospital is currently paying a $500.00 bonus for every shift we work with an 8:1 ratio. 7:1 has become the norm and 6:1 is rare. Is the bonus worth it and does it present burnout. Our hospital recently brought on 15+ travel nurses. These nurses make 2-3 times more than we do. It’s insane and discouraging. Melnyk (2020), states that some of the causes of burnout are increased clinical demands, inadequate staffing, moral distress, and reimbursement issues. Burnout not only affects nurses it also affects the quality of care for patients. Burnout causes medication errors and decreased patient satisfaction (Melnyk, 2020). We became nurses to care for people and that is very hard to do when we are exhausted and burned out.

Access to the evidence is important and imperative. It is equally if not even more important to engage staff and have EBP be driven from the bedside. How do we accomplish this?

Victoria, my fellow classmate- you hit the nail on the head with this one! I started out as a CNA, phlebotomist, LVN, and now RN. We are overworked and underpaid. Especially now with COVID, the amount of work we are expected to do is overwhelming. I love taking care of people, but I am on the verge of being burnt out. I have several nursing friends and cousins and sadly we all feel the same way. One of us left the hospital and is now a school nurse. She loves it! I have thought about it but being a bedside nurse is my calling. Our ratios used to be 4:1 and now they are 6:1. So many nurses have left my job because of this! There must be a solution. My GOD! There are some days I call out due to exhaustion. We are the aide, the phlebotomist, the secretary, the housekeeper, and even the dietician. I am a strong woman, but I must admit I too am tired.

Nurses are a critical part of healthcare and make up the largest section of the health profession. Nursing shortages affect the patient and can cause harm and error to patient care. Nursing shortages lead to errors, higher morbidity, and mortality rates. In hospitals with high patient-to-nurse ratios, nurses experience burnout, dissatisfaction, and the patients experienced higher mortality and failure-to-rescue rates than facilities with lower patient-to-nurse ratios. Nursing profession continues to face shortages due to a lack of potential educators, high turnover, and inequitable workforce distribution. Nursing faculty are experiencing a shortage, which leads to enrollment limitations, limiting the number of nurses that a nursing school can generate. Decreased and limited faculty can cause fewer students, and the overall quality of the program and classes can decline. Nurses graduate and start working and determine the profession is not what they thought it would be (Haddad et al.,2021).
