The Doctoral Degree and Professional Nursing Practice 

Doctor of Nursing (DNP) and Doctor of Philosophy (PhD) degrees are the highest level of education for the professional nurse.  Even though both degrees are considered lateral by means of education, the focus of each program is unique to the degreeThe DNP prepared nurse focuses on practice while the PHD education is based on scientific research (AACN, 2006) 

Doctor of Nursing Practice 

Nursing professionals, who pursue a DNP degree, Focuses on an integrative practice and immense practice experience (AACN, 2006).  Due to the continuous, evolution of the nursing practice, DNP professionals are high in demandThe DNP is recognized to improve the outcomes of the health care delivery system by improving patient outcomes and the knowledge of nurses (Michael & Clochesy, 2016)According to the AACN task force, the DNP role was developed to improve the practice of the expert nurse and to increase and accelerate the evidence of research into practice (Michael & Clochesy, 2016). 

DNP and PHD are terminal degrees for expert nurses.  Collaboration of these to degrees will improve the outcomes in everyday practice.  The PHD professional focuses on the scientific aspect of healthcare and the DNP professional improves outcomes through evidence-based practice.  The PHD professional focuses on the research and design of a specific topic or topics while the DNP professional focuses on priority setting and time management (Conn, 2014). 

DNP is appealing because it also the nurse to improve patient outcomes and integrate new knowledge into practice.  The goal for me as a DNP professional is to focus more on the clinical aspect than research.  Collaboration of both DNP and PHD is needed to provide optimal care, leadership and enhance the education of future practitioners.

RE: Discussion – Week 1

According to the American Association of Colleges of Nursing (AACN), the research-focused program (Ph.D., DNS, DSN, or DNSc) focuses on scientific content and research methodology for nurse scholars and scientists.  All the programs mentioned require an original research project and the completion and defense of a dissertation or linked research papers (American Association of Colleges of Nursing [ANCC], 2006).  However, the doctoral program (DNP) is a practice-focused that emphasizes a pioneering and evidence-based concept that reflects the application of credible research findings (ANCC, 2006). As a nurse that is pursing the highest nursing degree, expectations are different than those with bachelor or associate nursing degrees.  According to the AACN, a DNP program will prepare nurses to:

  • Design, implement, and evaluate therapeutic interventions based on nursing science and other sciences.
  • Guide, mentor, and support other nurses to achieve excellence in nursing practice
  • Use conceptual and analytical skills in evaluating the links among practice, organizational, population, fiscal, and policy issues.

The bullet points are only a few of the improved nursing practices we will learn in the DNP program (ANCC, 2006, p. 17).  Other nursing degrees may cover the information above, however, it is not to the extent that DNP nurses will receive.

During my years as a bedside nurse, I enjoyed learning new practices, following up to date policies, and going above and beyond for the patients.  During my master’s degree, I learned exactly what evidence=based practice was, how hospitals were reimbursed based on patient satisfaction scores, and the importance of treating the patient as a whole not just their diagnosis.   The DNP will help me during my career to improve patient outcomes along with moving the culture of nursing past the “we have always done it this way” to one of up-to-date evidence-based practice.

Professional Nursing Practice and a Doctoral Degree

In response to the constant changes in health care today, nurses are responsible for staying up to date on many rapidly changing aspects of patient care. They are also expected to help improve the quality of health care. Participating in doctoral education is an admirable way to position oneself for this goal, and it can lead to new professional opportunities.

Why did you decide to pursue a graduate degree? What factors influenced your decision to pursue your degree? What impact will this degree have on your career?

 

This week, you examine the characteristics of various programs, such as the DNP and the PhD. The DNP degree focuses on clinical practice, whereas the PhD degree focuses on research. DNP- and PhD-prepared nurses frequently collaborate with other professional colleagues to analyze critical issues and find appropriate solutions.

 

This first Discussion allows you to investigate what it means to earn a doctorate and how your chosen degree program relates to your professional goals.

I want to first say that I appreciate your in-depth review of both the Ph.D and DNP roles. I honestly feel as though my learning was solidified with your explanations. There are two things that stuck with me as I read your discussion post—goal alignment and how COVID-19 has impacted nursing.

To begin, I think it is critical for nurses to reflect on their position and consider how they want to impact nursing in the future. Often times, I hear nurses state that they want to pursue an advanced degree, but they waiver back and forth with either being a nurse practitioner or Ph.D. When I ask what exactly they want to do, they say that they just want an advanced degree to be more marketable and “keep up with the times”.  I warn that this is dangerous because both roles are critical to the nursing practice and require a different skillset—advanced degrees are not to be entered into lightly. I believe the importance of these degrees and their contribution to nursing practice should be communicated at the BSN and MSN levels to help appropriately inform future nurse leaders about their impact on the profession. From my personal experience, I think that most nurses just want to attain the “glamour” of the degree to add the credentials, but they do not have a passion for research or practice improvement. Therefore, I agree that nurses need to identify and align their goals.

COVID-19 has significantly impacted the entire healthcare delivery system. While it has wreaked havoc on life as we know it and nursing care, it has also ignited my interest in evidence-based practice. I fully enjoy looking at data, creating a plan, implementing the plan, and waiting for the results. With that being said, as a labor and delivery nurse at the start of the pandemic, it was evident that care for COVID + labor patients would look different from the norm. Over the course of the year, we realized that pregnant COVID patients would present with pre-eclampsia symptoms, which posed a major problem for our unit—we were faced with asking this question, “how do we know who needs magnesium sulfate and who does not”? Did COVID pre-eclampsia symptoms take the same toll on the body as true pre-eclampsia? This was major shift in critical thinking and nursing practice. I absolutely fell in love with this, and I joined a task-force committee at work to help address the new concerns as we arise. It was exciting to engage in the research process and help create practice change on our unit. So, it is safe to say, I am ready for the DNP ride (insert major smile). The AACN (2006) identified the eight essentials of the DNP that are foundational competencies that are core to all advanced nursing practice roles. I am sure that I will develop the foundational essentials, but I most closely relate to Clinical Scholarship and Analytical Methods for Evidence Based Practice.
