The primary stakeholders impacted by this change are,

As a nurse, it indicates the need to effectively manage the conditions that lead to increased readmission.

The change process seeks to address the high readmission rate among the patients. The healthcare organization has to deal with the financial penalties imposed due to the high readmission rate. In addition, the patients have to deal with a high cost of care. The proposed change seeks to ensure that there is better adherence to the treatment plan. Many patients fail to adhere to the treatment plan when they transition from the hospital to home. Failure to take medication means that the recovery process will be delayed. The change will involve all nurses who will be tasked with providing the patients’ education as they transition from the care facility to home.

The need to make improvements in the current discharge process is an indication that the type of change being implemented is developmental change. The change seeks to make adjustments to the process of discharging patients from the hospital facility. There is a need to have more involvement of nurses in the discharge process and the post-discharge stage.

The management team will consist of three nurse leaders, finance manager, project manager, operation manager, and human resource department members. The team will be assigned different functions intended to make the intervention successful. happy to hear your experience ended well and you had support from your nursing supervisor. I agree speaking

up should be the most effective way to stop incivility. However, I have not found that route effective in my

work setting. Leadership positions such as managers, directors, and charge nurses “have been reported as

primary sources of bullying and incivility (Johnson & Rea, 2009, as cited in Broome & Marshall., 2021, p. 259).

Sometimes there is no capable and supportive leader to voice your concerns to. When I voiced my concerns

about a nurse I was precepting, my concerns were not acknowledged, and it was turned back on me to handle

it by myself. The nurse continued to work on our unit and was a challenging individual to work

with.

One strategy put into place by our manager was to write an “opportunity for improvement” note to co-

workers when their behavior was identified as unprofessional or uncivil. This was meant to be a way for a

person to speak up to individuals who were hard to approach. The sender was anonymous. This route of

communication also proved not to be effective. I believe this was due to the anonymous nature of the note.

People felt attacked and became defensive instead of open to receiving feedback. Verbal and written

communication is essential to an “aspiring leader,” and effective communication depends on self-awareness

and “how others respond to you” (Broome & Marshall, 2021, p. 167). Instead of ignoring incivility or placing

the sole responsibility onto a preceptor to address incivility, effective strategies include a combination of

education about workplace incivility and experiential learning exercises for staff (Armstrong, 2018).

Precepting can be challenging and rewarding. One of the main reasons I became a preceptor was to directly

affect my unit’s civility. Leading by example in this role influences the overall culture.
