Nurses play an integral role in the implementation of healthcare programs. A good healthcare program should address the specific needs of the population and result in good health outcomes for the population. In this case, the focus is on reducing falls among the elderly population. The risk of falls among the elderly population continues to increase at a high rate and this presents a significant health concern.  

Interviewer: Tell us about a healthcare program, within your practice. What are the costs and projected outcomes of this program? 

Me: The elderly patient fall reduction program aims at reducing the incidences of falls among the elderly population. The key activities within the program include conducting the home safety assessment to determine factors that could expose the elderly populations to fall. Assessing the homes for the elderly population will include determining whether their floors are slippery (Wilkinson et al., 2018). Also, poor dietary intake may expose the patients to osteoporosis. Therefore, equipping the elders with the proper dietary intake information will ensure that they take appropriate diet and so reduce the risk of falls and the associated complications (Popejoy et al., 2017). Furthermore, the elderly population will be educated on the need to engage in light regular physical exercise. The program is estimated to cost about $96 million which will cater for providing the teaching resources and paying the human resource for about 6 months. The program targets a large population to achieve a desirable outcome within 6 months.  

Interviewer: Who is your target population? 

 Me: The program targets elderly populations who may be at risk of falls due to their comorbidities and polypharmacy. The elderly patients also have reduced mobility and this could make them lose balance and easily slip or fall and get injured.  

Interviewer: What is the role of the nurse in providing input for the design of this healthcare program? Can you provide examples? 

Me: The nurses will be responsible for designing the assessment forms for home safety for the population. Also, the nurses will guide the processes to be undertaken in determining the health needs of the population and factors that could expose them to falls. Besides, they will also play an important role in educating the patients on the medication and dietary intake. For example, the nurses will work with other healthcare providers to design patient education and how to implement them to the patients (Tomaschewski-Barlem et al., 2018) 

Interviewer: What is your role as an advocate for your target population for this healthcare program? Do you have input into design decisions? How else do you impact design? 

The advocate has a role in informing the target population to collaborate and be part of the program. The willingness of the elders to participate in the education and implement what they are taught has an important role in influencing the outcome of the program. The advocate has a direct impact on the design decision since they will collect data from the community and identify possible challenges and the best way to overcome them in the program implementation. Moreover, the advocate can impact the design by researching tailored approaches to addressing the issue.  

Interviewer: What is the role of the nurse in healthcare program implementation? How does this role vary between the design and implementation of healthcare programs? Can you provide examples? 

Me: The nurses work with other healthcare providers in implementing the program. Furthermore, they will liaise with the community health volunteers in carrying the home assessment (Dadzie, Aziato & Aikins, 2017). Their role may also vary between the design and implementation. At the design level, nursing informatics skills are required. On the other hand, communication and health assessment skills are required at the implementation phase.  

Interviewer: Who are the members of a healthcare team that you believe is most needed to implement a program? Can you explain why? 

Me: The healthcare team to be involved in the program include the nurses, community health volunteers, clinicians, pharmacists, and nutritionists. Nurses are the most required since they have to coordinate with other healthcare providers and interact with the patients to identify their needs and educate them accordingly (Gerber, 2018).  

Finally, the health promotion program will result in better health outcomes for elderly people by reducing the incidences of falls and the associated mortality and morbidity.
