The presentation will comprise of 12 slides to provide the context of the project. The first slide is an introduction to provide a brief about what will be discussed (Hoffmann et al., 2014). The implementation process will be discussed in slide nine in which roles of stakeholders will be outlined (Hoffmann et al., 2014). Slide ten will address challenges encountered in the project and strategies used in the management of these barriers to facilitate health change. Evaluation of the project will be undertaken in slide eleven to analyze the effectiveness of the outcomes for implementation.
