Policies Affecting Practice and State of Health Care Delivery 

The three major policy issues that influence the current state of population health equity and healthcare delivery are the entitlement reform affecting the Medicaid program, increasing prices of prescription drugs, and increasing opioids use. There have been debates about increasing funding of the Medicaid program, which costs the taxpayer more than $380,000 billion (Young, 2019). Some legislators want to have this funding reduced and the burden transferred from the federal government to the states. Such attempts imply that many Americans may not access care through the Medicaid program. Second, the price of prescription drugs has been rising, which has prevented many from accessing health care. The 2017 Reauthorization Act of the FDA was almost passed to authorize competition from generic drugs (PEW, 2019). This can help bring the price of drugs down, even though it may imply an increased health risk to users. Third, there has been increasing use of opioids in the United States.  

According to a report by NIDA, more than 91 people in the United States die every day owing to opioid use. Thus, there have been calls for increased funding to address this menace. Funding for the regulation of opioid use have increased due to the enactment of the 21st Century Cures Act (NIH, 2019). The increasing use of opioids implies that there are disparities in access to health care services, particularly in pain management. Population health equity cannot be achieved when only some people have access to quality drugs. These policy issues have significant impact on advanced nursing practice. First, the number of patients with serious health care needs associated with opioid use has been rising, which implies increased workload among nurses. Second, the advanced nursing cannot manage to provide care to every American owing to problems associated with funding and high cost of prescription drugs.
