Three Major Policy Issues That Affect the Current State of Health in the United States 

Policy reforms in the United States health service delivery address major challenges that affect equitable access to care. Despite the formulation of policy initiatives, access to care is still a major challenge for many Americans especially those in the low-income bracket. One of the major policy issues of the Trump administration is on the changes to Medicaid and Medicare (Mulligan & Castañeda, 2017). Reforming the Obamacare will affect access to quality care especially among the poor citizens who rely on the services of third-party payers during treatment. Instituting higher premiums to the Medicaid beneficiaries alters equitable access to care which limits the purchasing of health coverage for poor populations.  

The second policy issue relates to the Opioid Crisis majorly prescribed for chronic pain management. A disparity is identified among the care providers in which African Americans are denied the medication due to prescriber bias (Weinstein et al., 2017). However, over-prescription of the opioid medications results in compulsive use especially among populations vulnerable to drug abuse. The U.S. Food and Drug Administration (FDA) relies on the services of the doctors to address the challenge of over-prescription. The third health policy issue relates to drug prices which continue to rise affecting the American spending as they seek care services (Mulligan & Castañeda, 2017). This is majorly evident in the introduction of new drugs and thus alter quality access to health especially for populations in the low-income category.  

The population health policies and initiatives alter the pattern of care delivery as advanced nurse practitioners have to comply with the regulations of the state. For instance, nurses are obliged to deny or administer an opioid prescription to selected groups of patients to avoid addiction. Moreover, nurses are in a better position to advice patients on a range of medications that are affordable and guarantee quality care.

The health service delivery in the United States of America is readily influenced by a myriad of policy reforms. The latest legislation aims to reduce the Premium Tax Credit contributed by the state government to the health policy. This means that Americans are required to pay more premiums to the third party payers in order to access quality care (Melo & Fleuriet, 2016). Changes to this policy affect the health seeking pattern of low-income earners in the United States of America. The policy to address the prescription opioid (PO) crisis is a major aspect that affects health care delivery. According to the National Institute on Drug Abuse (NIDA), 91 Americans die daily due to an overdose of opioid. Policymakers have proposed increased funding to fight the PO menace (Wilensky (2019). However, the policy intervention is bias on certain racial groups in America. Particularly, the African Americans diagnosed with chronic pain disproportionately receive PO therapies due to a presumption that the population will use the drug for non-medical purposes.  

The advanced nurse practitioners are integral in the implementation of the population health reforms since they spend the longest time with patients. The nurses are required to ensure equity in service delivery (Wilensky (2019). Besides, advanced nurse practitioners need to adhere to stipulated guidelines on PO administration to minimize the rates of opioid addiction (Melo & Fleuriet, 2016).
