Nurse Leading Change

More than ever, nurses are present in every healthcare setting and possess a unique role in formulating policy. The Institute of Medicine’s 2010 report “The Future of Nursing: Leading Change, Advancing Health” recognized the importance of this role and called for nurses to take leadership in improving the quality of healthcare.

The American Association of Colleges of Nursing (2018) emphasizes the role of nursing in policy and identifies, in its essentials documents, the expected policy involvement that should be addressed in educational programs at the baccalaureate, master’s, and doctoral levels of professional nursing, including advanced practice. The Robert Wood Johnson Foundation and other nursing organizations also expect nurses to address policy as part of their professional role (RWJF, 2010).

Have you read any of these policy statements? Are you involved in any nursing organizations as an active member of committee member to enact change? Would you consider it in your advances registered nurse practice?

Take care,

Dr. B

According to an article written by the Robert Wood Johnson Foundation (2014), nurses are the healthcare professionals who spend the most time with the patient and their families however, they are not involved in the health policy formation. This could be due to the fact that nurses are not fully prepared and educated about the role they play in policy making. A conference was organized by the said organization to identify strategies on incorporating health policy education to the curriculum of nurses at the doctoral level. They suggested that nurses possess the insight on meaningful strategies to improve the healthcare landscape. Through research, nurses can navigate the evidence and garner support for implementation of changes to produce better health policies.

Change is taking place all around us. From an organizational standpoint, some of the changes being implemented will be successful; others will fail. In fact, organizational change is only successful 25 to 35 percent of the time. Changes in healthcare organizations are inevitable and occurring at unprecedented rates. Such changes greatly impact nurses and their work, yet these experiences are rarely explored.

To be influential, nurses must see themselves as professionals with the capacity and responsibility to influence current and future healthcare delivery systems. The nursing profession is based on the science of human health and the science of caring. It operates from a framework that values all people in a holistic way and seeks to foster and advance people’s health throughout their lifespans and across all levels of society.

Through policy work, nurses can and should influence practice standards and processes to assure quality of care. Nurses who influence policy help shape the care that will be provided today and tomorrow.

What is your stance in making change through policy?

Take care,

Dr. B

Addressing committee member disagreements can be a daunting task as a nurse leader, and disagreements over spiritual care can be a difficult subject to make a team come to agreements about. As a nurse leader, it is vital to utilize change theory when approaching any issues so to have a protocol that may help structure the change so that the stakeholders of the change may be heard, educated, and encouraged in the process of the change. Using change theory can help mitigate issues and help bring a diverse group together.

Change is a constant in life regardless of how difficult it may be which makes it a common problem that leaders must face when the stakeholders have restraints and different views when the changes are initially proposed. “Iit is important for the leader to exemplify for the team the ability to own the changes that lead to the realization of these opportunities in ways that will enhance both the work experience and the patient experience”(Weberg, 2022). When nurse leaders initially propose a change process, they need to emphasize that the change is needed and good, but they must also initiate an open dialogue to fully understand the dissenting members’ perspectives. Communication that includes empathetic and active listening helps the nurse leaders to get a new perspective on the proposed change and get some ideas of the possible challenges that will be faced so they may be better prepared on how to address them. Also, listening to their team and taking their thoughts into consideration makes the team feel more valued and helps the team have more buy-in in the project and will encourage them to put their efforts into the project.

Lewin’s change theory would be a helpful tool to utilize in this situation where people do not agree on the proposed change for spiritual care. Using Lewin’s“unfreezing, change, and refreezing” theory to address the need for spiritual care, create alternatives and plans for the change, and encourage the establishment of new habits(Petiprin, 2020). To handle the spiritual care disagreements I would listen to the team about their different ideas and once they voiced their differences, I would work with them on possible compromises and research findings that could enlighten perspectives and promote mutual understanding. I would also form subgroups for both sides of the arguments so that each side could delve deeper into the issue that could reflect collective viewpoints and foster collaboration.Using these strategies and Lewin;s change theory, a nurse leader can better handle the complications that naturally come with changes.
