You have been selected to serve on a community outreach committee within your state’s nursing organization. The committee includes registered nurses of different specialties. At your first meeting, it becomes evident that not everyone is in agreement with a recent position statement about the role of spiritual care, with some members arguing they will no longer support the committee if the position statement is not revised or reversed. As a nurse leader and change agent, how would you approach the committee? How could you draw from change theory to address these concerns and encourage collaboration on the committee?

Nurse Leading Change

More than ever, nurses are present in every healthcare setting and possess a unique role in formulating policy. The Institute of Medicine’s 2010 report “The Future of Nursing: Leading Change, Advancing Health” recognized the importance of this role and called for nurses to take leadership in improving the quality of healthcare.
The American Association of Colleges of Nursing (2018) emphasizes the role of nursing in policy and identifies, in its essentials documents, the expected policy involvement that should be addressed in educational programs at the baccalaureate, master’s, and doctoral levels of professional nursing, including advanced practice. The Robert Wood Johnson Foundation and other nursing organizations also expect nurses to address policy as part of their professional role (RWJF, 2010).
Have you read any of these policy statements? Are you involved in any nursing organizations as an active member of committee member to enact change? Would you consider it in your advances registered nurse practice?

According to an article written by the Robert Wood Johnson Foundation (2014), nurses are the healthcare professionals who spend the most time with the patient and their families however, they are not involved in the health policy formation. This could be due to the fact that nurses are not fully prepared and educated about the role they play in policy making. A conference was organized by the said organization to identify strategies on incorporating health policy education to the curriculum of nurses at the doctoral level. They suggested that nurses possess the insight on meaningful strategies to improve the healthcare landscape. Through research, nurses can navigate the evidence and garner support for implementation of changes to produce better health policies.

Change is taking place all around us. From an organizational standpoint, some of the changes being implemented will be successful; others will fail. In fact, organizational change is only successful 25 to 35 percent of the time. Changes in healthcare organizations are inevitable and occurring at unprecedented rates. Such changes greatly impact nurses and their work, yet these experiences are rarely explored.
To be influential, nurses must see themselves as professionals with the capacity and responsibility to influence current and future healthcare delivery systems. The nursing profession is based on the science of human health and the science of caring. It operates from a framework that values all people in a holistic way and seeks to foster and advance people’s health throughout their lifespans and across all levels of society.
Through policy work, nurses can and should influence practice standards and processes to assure quality of care. Nurses who influence policy help shape the care that will be provided today and tomorrow.
