As a registered nurse, I get the privilege to sit front and center, as well as participate in the advancement of the nursing profession and healthcare in general. The National Academies of Sciences, Engineering, and Medicine, The Future of Nursing 2020-2030: Charting a Path to Achieve Health Equity details a comprehensive review for what is necessary in an ever-changing environment of safety, culture, education, leadership, and practice within nursing. Although I see the benefits of all the recommendations within this report, the concept that nurses improve health equity stands out as one of the most effective for creating change within the industry. Health equity is “the state in which everyone has the opportunity to attain full health potential and no one is disadvantaged from achieving this potential because of social position or any other socially defined circumstance” (NASEM, 2017, as cited in National Academies of Sciences, Engineering, and Medicine, 2021). Nurses play a particular role in screening for social issues that can interfere with an individual’s access to food, a stable home, transportation, basic needs like heat and water and domestic violence (National Academies of Sciences, Engineering, and Medicine, 2021).

This is of particular interest to me as a former emergency department nurse that often conducted these screenings, provided food and temporary shelter to those that did not have the means to acquire it and gain the trust of some at risk of self-harm or domestic violence. Although these short-term provisions did not directly solve any health inequities, it highlighted areas of needs within the community and allowed me to collaborate with nurses in social services and care management to begin bridging these gaps.

Title: NUR 513 Refer to the consensus report of the National Academies of Sciences, Engineering, and Medicine

The consensus report of the National Academies of Sciences, Engineering, and Medicine, The Future of Nursing 2020-2030: Charting a Path to Achieve Health Equity, made recommendations to improve the equity of healthcare for the population. These recommendations are a step in the right direction toward preparing nurses to promote health equity, reduce health disparities, and improve the health and well-being of everyone (The National Academies Press, 2021).

The consensus report identified while most nursing education programs included content on social determinants of health (SDOH) within the public or community health courses, the material was not integrated throughout the curriculum. The recommendation for the accrediting body to include health equity and SDOH would require nursing education to prioritize the inclusion of these standards in their curriculum to maintain accreditation.

The report revealed that nurses and nurse practitioners who work in schools, public health, community health, and emergency and urgent care settings do not believe they received adequate training in recognizing SDOH. I believe that incorporating SDOH training for nurses in all practice settings would be beneficial for patients as providers could refer patients to resources, as needed.

As a nurse who has worked away from the bedside for many years, I have witnessed healthcare providers who do not consider poverty, unequal access to healthcare, racism, or education when developing a plan of care for medically complex patients. It is important to recognize the most well-written plan of care will not be followed if the patient does not have the knowledge or resources to maintain compliance with the plan.

I agree that nurses will remain on the front line of change within healthcare. Advanced practice nurses must be educated on SDOH to continue to evolve and provide equitable care for patients in their charge.

Title: NUR 513 Refer to the consensus report of the National Academies of Sciences, Engineering, and Medicine

The National Academies of Science, Engineering, and Medicine, The Future of Nursing 2020-2030: Charting a Path to Achieve Health Equity report was a collaboration between the Robert Johnson Foundation and the Institute of Medicine(IOM) to explore the contributions of nursing in addressing the social determinants of health (National Academies of Sciences, Engineering and medicine, 2021). The report tabled recommendations regarding nursing education that are aimed at improving healthcare.

The recommendations regarding the nursing educations that I believe will be most effective in creating change in the industry focused on training, accreditation and advancement in the nursing profession. On training, the report recommended that nurses should receive education to help them function as leaders, advance science and meet diverse patients needs (National Academies of Sciences, Engineering and medicine, 2021). For nursing profession fully advocate for itself, there needs to be nurses on the policy table coupled by sound research to push for reforms such as allowing advance practice nurses to practice with multi-state licenses.  On advancement, the report emphasized the need for the entry level nurses to understand the importance of furthering their education while providing opportunities to do so early in their career (National Academies of Sciences, Engineering and medicine, 2021). When nurses graduate with an understanding of the many pathways of growth, it helps them continue into the many divisions of advance practice nursing. A community college in Rapid city has produced thousands of licensed practical nurses(LPN) who are immediately transitioning to Associate Diploma in Nursing (ADN) through their  LPN -ADN program and many are later joining online Registered Nurse (RN) to baccalaureate (BSN) programs while already employed.  Regarding accreditation, the report further recommended that the accrediting and licensing of nurses should be based on demonstrated mastery of core skills and competencies besides the board examination. I believe this requirement will enable the production of competency sound nurses with hands on and critical thinking skills. I have met new graduate nurses who are very good in theoretical knowledge but lack simple competencies required for the job. Trying to learn a host of new skills and competencies,  results in possible frustration leading to loss of passion in the profession which should not be the case.

Some sections of the report suggested that baccalaureate prepared nurse should be the entry level while phasing out associate diploma in nursing training within a stipulated time frame. While BSN prepared nurses are associated with positive patient outcome, lower mortality rates and reduced hospital stay (Blegen et al., 2013), this move will deny the low income earning students from pursuing nursing due to the cost or the distance to the institutions. Moreover, the ADN to Masters of Science in Nursing (MSN) program has been noted to have contributed majorly to advanced practice nurses and the nursing faculty (National Academies of Sciences, Engineering and medicine, 2021). Therefore, I disagree with the recommendation to phase out the ADN programs instead, reinforce the need for career progression in the nursing curriculum.

Refer to the consensus report of the National Academies of Sciences, Engineering, and Medicine, The Future of Nursing 2020-2030: Charting a Path to Achieve Health Equity, provided in the topic Resources Identify recommendations for nursing education you believe will be most effective or radical in creating change within the industry. Provide rationale based on your experience in practice. Do you agree or disagree with how the report describes the evolution of the advanced practice registered nurse role? Why or why not?

By 2030, the nursing profession will look vastly different and will be caring for a changing America (National Academies of Sciences, Engineering, and Medicine. 2021). I believe this is true as a nurse with 20 years of experience, I have seen many changes within the healthcare industry and education whether it is nursing school education or continued education provided by employer.

One of the recommendations for nursing education that I believe will be most effective is to prepare nurses to address social determinants of health (SDOH). This is an especially important recommendation because everyone has some type of challenge such as their income status, social status and/or cultural factors.

When I initiated my nursing career at a hospital in southern Texas, 95% were Mexican American who spoke Spanish and English or only Spanish. I am Mexican American and was well versed with some of the social determinants of health within this population. However, that changed throughout the years of my nursing career and when I moved to Minnesota. Currently, I work at a well-known hospital as a certified wound and ostomy nurse (CWON) where people come from all over the United States and around the world. We see a diverse types of ethnicity, religious, cultures, etc….   Although, we receive continued education twice a year on these topics within our ostomy and wound care departments, I feel it would be essential to prepare us more in depth to address these challenges.  This is also important for the student nurses to receive more education and prepare them for when they are facing these difficult tasks in their future nursing careers.

Due to aging, population growth, and a greater insured population following the Affordable Care Act (ACA), physician availability to patients has been recognized as one of the top barriers to meet the healthcare needs of patients in the United States of America (Zhang et al, 2020) With that said, I agree with the evolution of advanced practice registered nurse’s role as mentioned on the report. This is essential to the future of health care access to the population. With physician shortages, advanced practical nurses (APRN) should have the autonomy to practice to the full extent of their education and training, even as they work collaboratively with other health professionals (National Academies of Sciences, Engineering, and Medicine. 2021).

Title: NUR 513 Refer to the consensus report of the National Academies of Sciences, Engineering, and Medicine

The report that the Institute of Medicine (IOM) published in 2001 was information that was attempting to depict how the health system should have planned for its improvement. One of the six subjects that was inadvertently pushed aside was equity. There is inequity in health care. “Social determinants of health (SDOH) as defined by the US Centers for Disease Control and Prevention (Centers for Disease Control) are the conditions in which people live, learn, work, and play that are determined by the distribution of money, power, and resources and that affect a wide range of health and quality of-life risks and outcomes” (Macias-Konstantopoulos et al., 2023, p. 907). The health care field is not a level playing field. Insurance has a major influence unfortunately. There is a great push from the administrative staff to make sure that all the paperwork is correctly filled out to get the hospital its money. So yes, the hospital is a business, but it becomes overshadowed by the politics of reassuring that some form of payment is also filed in the patient’s profile as well. When the healthcare professional has direct patient contact, then they should not have to think about any sort of money exchange. Their focus is improving the patient’s current health care status. Providing health equity should be a given. Having worked in a county hospital, I was able to see how patients were obviously in the hospital because they were unfortunately the ones without insurance and also more so the ones who did not receive preventative care. Therefor they are now in worse shape than they would have been if they had received preventative care to begin with. The evolution of the advanced practice registered nurse will greatly benefit those who require preventative care but are just out of reach of the physician’s care. Having the option to seek care prior to desperately needing care would be extremely beneficial to the patient and extremely cost effective to the health care system.

This article focuses on specializing nursing professionals, exploring opportunities, advancements, and pristine outlooks toward the future. Nurses are defined yet underrepresented as they serve important roles in serving, caring and helping people.

As new ideas and technologies are introduced, nurses should be trained, and exposed to more inventories that make them even more skilled to withstand the pressures of new diseases, new illnesses, and treatment plans. Moreover, the policies and regulations fall on various departments below that reinforce “policymakers…leaders and educators” (National Academies of Sciences, Engineering, and Medicine, 2021). In the coming ten years, the nursing workforce will need to be stronger and more diverse to deliver care, promote health and well-being among nurses, persons, and communities, and address the systemic injustices that have driven widespread and persistent health inequalities. Besides a solid notion comes from within being healthy and how people appreciate living in a positive way, this is generated from one’s individual’s assessment of oneself. Experiencing a pandemic like COVID-19 enabled people to witness the significance of nurses and how their presence, work, and service are precious. Adding on, factors like diversity contribute towards the overall thoughts of acceptance, and open-mindedness, and also promote a safer, rich and learning environment for prospective and current nurses. Having a balanced number of men and women also denotes the uphill trend of nursing as “NCSBN survey noted men … account for 9.4% of RN workforce” indicating a “0.3% rise from 2017” (Robert, 2023). Lastly, I agree with how nurses should be recognized more, they should be appreciated more, taught, and given more exposure to every latest possibility.

Title: NUR 513 Refer to the consensus report of the National Academies of Sciences, Engineering, and Medicine

Some of the recommendations for nursing education that I believe will be most effective in creating change within the industry. After referring to the report of the National Academies of Science, Engineering, and Medication, The Future of Nursing 2020-2030: Charting a Path to Achieve Health Equity.

Some of the most effective in creating change within the industry. Nurses represent the largest population of healthcare professionals, nurses making up such a big population of healthcare professionals they can manage healthcare teams and join clinical care, public health, and social services. Nurses have a very important role in achieving health equity. For nurses to achieve that goal, they need more education and a healthcare system that will support a work environment and autonomy. Nurses need to conduct research for nursing professionals to help build a culture of health, reduce disparities in people, and help them achieve health equality (National Academies of Science, Engineering, and Medicine, 2021).

Nursing can have a big impact on social determinants of Health (SDOH). Nurses can bring about change in the clinical setting by learning about their community finding social determinants and making changes to help bring about better health equality. Nurses can help bring about change in their organization by learning about the organization’s policies for SDOH and advocating for changes to help social determinants (Scowart, 2023). Nurses can advocate and support organization strategies to implement SDOH. Nurses can also bring together other healthcare organizations to make decisions with patients to see if they are willing to talk about their SDOH. If nurses could get patients to talk about their SDOH they could bring about great changes to help patients overcome or bring about some level of change for their social determinants of health.

I do agree with the reports and how they describe the evolution of the advanced practice registered nurse’s role. The reason that I agree, is because APRNs, can bring about greater specialty areas such as midwives, and nurse practitioners it can help bring healthcare teams to rural areas that do not have physicians. APRNs have worked with patients and can research and find the communities’ SDOH and take their healthcare concerns to the stakeholders to bring about the greatest change.
