I agree with you that nursing practice is any form of nursing intervention that is meant to influence the care outcomes. The nurse practice has been evolving overtime especially with the transformation of the healthcare systems and healthcare policies. In the current healthcare industry where the focus is on the patient, what is commonly referred to as patient centered approach has ensured that the patient is given autonomy to choose on the best care practice that is appropriate and improves the patient’s health condition (American Association of Nurse Practitioners, 2019). This has called upon the nurse training institutions to shift and adapt to the new and emerging trends in healthcare. The advanced nursing role has been the significant part that the Affordable Care Act aims to enhance health promotion measures among the aging US population (Woo et al., 2017). The research findings reveal that the advanced nursing role would be one of the professions at a highest demand by the year 2022.

Do you agree or disagree with how the Institute of Medicine (IOM) describes the advanced practice registered nurse role evolving? Why or why not?

RESPONSE

The Institute of Medicine (IOM) recommends that, in order for the healthcare industry to meet the rising demand for quality and affordable care, there is need to redefine and redesign the nurse training framework to march with the emerging trends in healthcare. Among the recommendations for nursing education to promote effectiveness include expanding the nursing faculty and improve the capacity of the available nursing schools. Additionally, there is need to reframe the nurse training program to expand the scope of training and ensure that the nurses graduating with the BSN are well trained on incorporation of technology, research methods, basic statistical data analysis and interpretation and above all they are well equipped to provide education to the patients and emphasize the current role of nursing profession. According to the ACA, the nurses are not only supposed to offer intervention measures in terms of treatment and taking care of the sick but they should also put the preventive measures on the forefront of their care services. It is imperative that the nurse training institutions help the student nurses sharped their communication skills and instill some pedagogical skills to help them deliver the message of health promotion measures as a way of improving healthcare and promoting quality life (Cleveland et al., 2019). I do agree with the IOM on about the evolving role of the advanced nursing practice. The recommendations remove the restrictions on the scope of practice of the APRN and allows them to practice to the full extend of their education and training (Salmond & Echevarria, 2017).

According to Brenner 5 level of nursing experience nurse, I would rate myself as a proficient nurse. I have been a nurse since 2014, taking care of patients across all ages has allowed me to gain more knowledge and clinical experience. During my work as nurse, I was able to take care of patients with GT, Tracheostomy, patient on ventilation, PICC line, Foley catheter insertion to mention but few, I was able to have direct care for both Adult and Pediatric patients.

Presently, where I work. I was able to care for patients with kidney transplant, to monitor for signs and symptoms of rejection and appropriate interventions. Not only that I have also taken care of patients with stab wound, patients with amputation of the limbs, patients with GI surgery. I am been optimistic

that after this program, I will be a better nurse.

Over the past several decades, possibly more accurately over the last several centuries healthcare in our society has transformed in many ways. One of fastest growing and most beneficial areas we’ve evolved is with nursing as a whole. Nurses have been on the forefront for centuries, dating back to the 1800’s we learned of the nurses of Wald’s Henry Street Settlement House in New York City. Nursing was, and still is today, primarily noted for providing care for people who are termed underserved, poverty-stricken, and elderly. Nursing has transformed over time into roles within the scope of Advanced practice nursing. “Advanced practice nursing has been defined as follows: any form of nursing intervention that influences health care outcomes for individuals or populations, including direct care of individual patients, management of care for individuals and populations, administration of nursing and health care organizations and the development and implementation of health policy.” (AACN 2004)

Over the years the role of advanced registered nurse expanded into many other specialties to meet the needs of our communities; under the advanced practice registered nurse there are four specialties; nurse practitioner, certified nurse midwife, certified registered nurse anesthetist, and clinical nurse specialist. Each of these expanding roles have exploded in our healthcare system to the point that they are meeting the needs so effectively that we won’t be drastically negatively affected by the gradual decrease in primary care physicians. Several of the roles require license post basic registered nurse and they are proving to be not only well educated but more personable with the community at large. Many states have given the clinical nurse specialist authority to practice without a physician’s supervision, six states have given CNSs independent authority to prescribe drugs. This makes a huge impact on the cost of healthcare. “Nurse practitioners are the largest group of advanced practice nurses, with some level of prescriptive authority in all 50 states and the District of Columbia” (American Association of Nurse Practitioners, 2017)

The history of Advanced Registered Nursing is very interesting and I’m looking forward to being a part of it’s evolution in the near future.
