The American Association of Colleges of Nursing (AACN) defines  Advanced Practice Registered Nurse (APRN) as master’s-prepared nurses who provide direct clinical care. This includes the nurse practitioner (NP), certified nurse–midwife (CNM), certified registered nurse anesthetist (CRNA), and clinical nurse specialist (CNS).  According to DeNisco, the advanced registered nurse is a nurse with a master’s or doctoral degree recognized by the AACN.

Nurse Anesthetists have practiced for more than 150 years as the main providers of anesthesia care for the military on the front lines during wartime. In 1952, the American Association of Nurse Anesthesiology established the certified registered nurse anesthetist credential to monitor the quality and consistency of nurse anesthetist education. Today, nurse anesthetists autonomously and in collaboration with physicians provide 45 million anesthetics annually.

The first nurse-midwife school was established in 1925 to provide family health, childbearing, and delivery care to women in the Appalachian Mountains due to high maternal and child death rates. Currently, all nurse-midwifery programs are in colleges and universities. Nurse midwives now provide autonomous primary care focusing on pregnancy,  childbirth, the postpartum phase, newborn care, family planning, and gynecological care.
The nurse midwife has prescriptive authority in all 50 states and territories and mandatory Medicaid reimbursement status.

The nurse practitioner (NP) role was created in the 1960s to address the shortage of pediatric primary care providers in rural areas. Initially, the training program ranged from 3-12 months. As the NP’s scope and responsibilities changed, the master’s degree became entry level of education required. Today, the  AACN recommends a doctoral of nursing practice for NPs. Currently, NPs are the largest group of advanced practice nurses.

The role of the clinical nurse specialist was created in the 1960s to provide advanced care to psychiatric patients. Today, the clinical nurse specialist is able to practice without physician supervision, prescribe medications and durable medical equipment, and order diagnostic tests in some states.

The level of education and training for entry as an advanced practice nurse continues to increase as the scope of practice and responsibilities also increases. The population is aging with more co-morbidities. The predicted physician shortage signals an impending lack of care for patients with complex care needs. Advanced practice nurses must be educated, supported, and empowered to continue to step in and fill the gap to meet the demands of the patients.

Today most midwives graduate at the master’s level degree with some obtaining their DNP degree. Since 1925, when the first midwifery school was established, the nurse midwifery program has grown such that today, the programs must be accredited by the American College of Nurse Midwives (ACNM). The graduates can then be eligible to take the national certification exam by the AMCB. The role of the Nurse Anesthetist has also evolved tremendously for about 150 years now in the United States. The role emerging during the Civil War, onto the Vietnam War. Nurse Anesthetist have legal authority to practice independently without anesthesiologist supervision in all 50 states.  The role of Nurse Administrators has yet to be defined specifically as the knowledge and skills needed for this role are not taught in the nursing baccalaureate programs. Some of the valid reasons that nurse administrators are not prepared well for the role are that they are not assessed for their leadership abilities, the lack of need for an advanced degree and ineffective orientation to the position.

The role of the APRN is every evolving with new roles emerging in the 21 St century within the health continuum requiring an inclusion of understanding of the healthcare delivery system in the nursing curriculum.

According to DeNisco and Baker (2021), nursing education has transitioned from an apprentice style of education to one that involves an education built in liberal arts requiring the development of critical thinking abilities. Even the term advanced nursing practice has been shrouded in considerable uncertainty until a watershed seems to have been reached for a definition that indicates master’s-prepared nurses who provide direct clinical care and to include the roles of nurse practitioner (NP), certified nurse–midwife (CNM), certified registered nurse anesthetist (CRNA), and clinical nurse specialist (CNS) (DeNisco & Baker, 2021). The nurse practitioner practice is thought to have emerged in the 1960s taking cue from Lilian Wald’s nurses of the 1800s and was developed to provide primary care underserved population (Stewart, 2021). In 1965, Loretta Ford and Henry Silver formally developed the role of nurse practitioner to advance the nursing profession and to respond to the need for providers in rural, underserved areas (Stewart, 2021). DeNisco and Baker (2021) affirmed that the role evolved out of the unavailability of primary care physicians to take care of pediatric populations with educational preparation changing from the initial 3 to 12 months to the present-day master’s degree as the entry-level education for nurse practitioner specialties.

Formal midwifery training was also necessitated to solve an urgent need to curb high maternal and infant mortality rates which led to the establishment of the first nurse–midwifery school in 1925 by Mary Breckenridge. Today, all nurse–midwifery trainings are college and university level programs which culminate in the DNP degree. The practice of midwifery requires licensed and independent certified nurse–midwives and certified midwives (CMs) who are autonomous primary care providers with prescriptive authority to manage women’s health, that concentrate on pregnancy, childbirth, the postpartum period, care of the newborn, family planning, and gynecologic needs of women (DeNisco & Baker, 2021).

Having evolved more than 150 years ago, nurse anesthetists are another licensed independent practitioners who autonomously deliver care in collaboration with surgeons, dentists, podiatrists, anesthesiologists and other healthcare professionals (DeNisco & Baker, 2021).

DeNisco and Baker (2021) averred that there is a national initiative to improve access to high-quality but cos-effective health care. This directive is bound to necessitate the advent of many new and yet to be conceived roles for nurses. Newly identified nursing roles such as nurse navigators and healthcare coaches, have been instituted and may require advanced degrees and certification. There are thriving care coordination opportunities for nurses throughout the continuum of care (DeNisco & Baker, 2021).
