Advanced Practice Nurse (APN) is a term used to describe a nurse with additional education at the master’s or doctoral level. The recognized APN roles are the Clinical Nurse Leader, Clinical Nurse Specialist, Nurse Educator, Nurse Administrator, Nurse Practitioner, Nurse Midwife, and Nurse Anesthetist (DeNisco, 2024). No matter the role or specialty, all APNs have the same Code of Ethics. The Code of Ethics is a guide that was developed by the American Nurses Association (ANA) to provide guidance for nursing care that meets quality standards and adheres to the ethical standards of the profession (American Association of Colleges of Nursing). Upon review of the roles of the Clinical Nurse Leader and the Nurse Midwife, there are differences in the scope and duties of their roles; however, both are guided by the ANAs principles of autonomy, beneficence, justice, and non-maleficence (Weberg et al., 2023).

The Clinical Nurse Leader is a supervisory role that develops models of care delivery and is ultimately responsible for patient care outcomes in the health care setting. The American Association of Colleges of Nursing developed Standards of Conduct, in addition to the Code of Ethics, that include the principles of altruism, accountability, human dignity, integrity, and social justice (American Association of Colleges of Nursing).

The Nurse Midwife has 3 mandates that are based on the Code of Ethics. These mandates are used to guide midwives in their practice, provide a framework for peer review, and advise others about the ethical standards of midwifery practice. The first mandate is directed toward the individual women and their families for whom the midwives provide care, the second mandate is to the public for the benefit of all women and their families, and the third mandate is to the profession of midwifery to ensure its integrity and in turn its ability to fulfill the mission of midwifery (American College of Nurse-Midwives). Although the Code of Ethics states the Nurse Midwife allows the patient autonomy, an ethical dilemma is present when the Nurse Midwife is faced with providing reproductive or obstetrical care for minor patients without parental knowledge or consent. Legislation may exist that dictates the Nurse Midwife notify the minor’s parent or guardian about certain issues despite the patient’s request for autonomy.

When I think of two distinct advanced practice registered nurse (APRN) roles, I think about nurse leader and nurse practitioner (NP). These two different roles are bound by  the same golden rule which is do no harm. However, each of these roles may have different ways of thinking about the same golden rule. For example, a patient who has had a history of being aggressive, throwing objects, hitting staff but is the father of one of the biggest donors of the hospital is on a general floor at a hospital. This patient has finally gotten to the point where the staff would like to blacklist this patient from the hospital for his extremely aggressive behavior each time he is a patient. The nurse leader may be governed by fidelity or the duty to keep one’s promise to the donor’s family to keep this patient on the floor at the hospital and not allow the staff to dictate who they get to treat (DeNisco, 2024). However, on the other hand an NP would be drawn to set a professional boundary between the patient and staff and to advocate against the harassment the staff is enduring each time this patient is on the floor.

In the role of a nurse leader and nurse practitioner, both are led by fidelity and both need to set professional boundaries, however in this one instance they both may choose different ethical guidelines to either support the business or the personal strength of staff due to their very different roles as APRN’s.

According to the National Association of Clinical Nurse Specialist (2023), The Clinical Nurse Specialist (CNS) is an Advanced Practice Registered Nurse (APRN) prepared at the master’s or doctoral level. In most of the states, CNSs are required to obtain a certification of one of the three population specialty areas, Adult/Gerontology, Pediatrics or Neonatal. CNSs have various roles within their specialty such as managing patient care, providing clinical guidance, improving and achieving best possible patient outcomes by making sure the best practices are being followed while utilizing evidence-based practice.

Nurse Practitioners complete a master’s or doctoral degree program have advanced clinical training beyond their initial professional registered nurse (RN) preparation and undergo rigorous national certification (American Association of Nurse Practitioners [AANP} ,2023). Nurse practitioners practice in various settings such as hospitals, clinics and specialty areas providing direct patient care.

The scope of practice for Advanced Practical Registered Nurses (APRN) varies between states because the rules and regulations are governed by the board of nursing at the state level (Boehning & Punsalan, 2023). For example, in Texas NPs must work under the supervision of a Phyician for all their scope of practice. In Minnesota, they are able to work the full scope of their practice independently.

According to the American Nurse Association, (2017), the nursing code of ethics is a guide for carrying out nursing responsibilities in a manner consistent with quality in nursing care and the ethical obligations of the profession. The code of ethics was developed by ANA which encompasses four main principles Autonomy, Beneficence, justice and non-maleficence (Gaines, 2023).

Regarding if there are any differences in the ethical guidelines that govern their roles, for example, the National Association of Clinical Nurse Specialists has guidelines for the Certified Nurse Specialists. Their guidelines correspond to the ANA’s four main principles of code of ethics, these include fostering autonomy, truth-telling, advocacy for patients, and mentoring nurses and other professionals to deliver equitable and safe care (National Association of Clinical Nurse Specialists, 2024). Overall, the code of ethics is a guide for all nurses, which includes advanced registered nurses.

As nurses, we all have experienced an ethical dilemma throughout our careers. Between the two advanced practice nurses discussed, there are situations in which one of the roles needs to respond differently to a situation than the other regarding an ethical dilemma. The NP’s role is to provide direct patient care, and the CNS mostly manages patient care. For example, NP working in an urgent care setting and is seeing a patient with a fracture, has a long history of addiction and is expressing extreme pain. In this situation, the NP must respond to the ethical dilemma directly with the patient. On the other hand, the CNS will have situations in which a nursing staff may be demonstrating incompetence in patient care which will cause patient harm if not addressed.

The American Nurses Association defines ethics as “the branch of philosophy or theology in which one reflects on morality; the formal study of morality from a wide range of perspectives including semantic, logical, analytic, epistemological, normative, and applied” (ANA, 2015). The Code of Ethics is a set of guidelines specific to behavior regarding moral decisions in professional settings. The code of ethics for nurses is updated every 10 years. The next revision will be released in 2025. The Code is used as a guideline for all U.S. nurses, “the Code steadfastly supports nurses across all settings and in all roles” (ANA, 2015).

Although governed by the same Code of Ethics, nurses of distinct roles might face ethical dilemmas specific to their job duties, or scope of practice. If we compare the advanced registered nurse roles of Nurse Educator and Nurse Practitioner (NP), they have vastly different scopes of practice. An NP’s focus will be on prioritizing quality care for their patients. Ethical dilemmas faced in practice might include caring for a patient involved in a crime. The text gives an example of a nurse caring for a patient responsible for the Boston Marathon bombings in 2013 (DeNisco, 2024). As a part of the ethical duties as a nurse, it is a responsibility to provide quality care for every patient regardless of that patient’s history or background. That includes respecting the patient’s right to privacy, treating them with dignity, and having compassion.

A Nurse Educator’s focus will be developing curriculum, teaching clinical skills, and preparing students for the profession. An ethical dilemma faced in academia might be an unethical work environment as described by Olson (2021), educators have identified relationships with colleagues where they have been spoken to in a condescending or disrespectful way, leading to conflict and a negative workplace environment. It is imperative for educators to foster a healthy learning environment as they build the framework for prospective nurses. The Code of Ethics should be the gold standard when ensuring an ethical environment in academic settings.
