HPI: A 75-year-old patient of Hispanic descent arrives in an immigrant healthcare clinic with fever, chills, a sore throat, and a nonproductive cough. She speaks some English. The patient appears very ill, with dry mucous membranes, dark circles under the eyes, and pale skin. The patient appears anxious and is reluctant to make eye contact or speak. The health history reveals that the patient lives with extended family in a home setting, including infants and elderly. All of the family members are undocumented residents of the United States.

Influenza: Based on the patient’s signs and symptoms, including fever, chills, a sore throat and nonproductive cough, the fact that the patient’s family were recently sick, and  that she just got the flu shot, can support our primary diagnosis for this patient. Another factor is that older adults are at high risk of getting flu. For this patient, a rapid influenza test would be recommended (National Foundation for Infectious Diseases 2023).

Pneumonia: Signs and symptoms of pneumonia include dry cough with thick yellow, green or brown mucus, difficulty breathing, sharp or stabbing pain, fever or loss of appetite, and weight loss. Based on our patient, she doesn’t experience all the symptoms of pneumonia, but she can be potentially at-risk factor since she is an elderly woman. Proper auscultation of patients’ lung, and an x-ray, would help us to rule in or rule out this diagnosis (Case, A., 2022).

COVID: Even though our patient had covid vaccines and a booster, she would be a potential candidate for covid due to her age. However, her symptoms are slightly different than those of Covid. Signs and symptoms that would confirm diagnosis include loss of smell, fever, sore throat, malaise, headache, congestion, and runny nose. In the elderly, symptoms of Covid may be slight alterations in mental status or in blood glucose control. This patient’s Rapid PCR test would help to rule in or rule out Covid (McIntosh, K., 2022).

Upper respiratory infection: Signs and symptoms of upper respiratory infection include scratchy or sore throat, sneezing, stuffy nose, and coughing. Considering the fact that our patient is living with her daughter and granddaughter can be a potential risk factor of getting this patient any viral infection. Ruling out our previous diagnosis could help us to confirm URI. (Lieberman, D., & Lieberman, D., 2023).

Diagnosis: Influenza

 

Pharmacology

Ibuprofen 400 mg tablet PO every 6-8 hours as needed for pain (Dunphy et al., 2019)

Acetaminophen 325 to 650 mg PO q4h PRN for fever and pain

75 milligrams (mg) or 12.5 milliliters (mL) two times a day (one in the morning and one in the evening) for 5 days (Pelton, S., & Tähtinen., 2022).

 

Non-pharmacology

Take a rest

Increase fluid intake

Humidifier, cool mist (Dunphy et al., 2019)

 

Labs/Diagnostics

COVID PCR

Chest XRAY

Influenza PCR

Education

Hand hygiene

Avoid smoking

Take as much rest as needed

Receive annual vaccinations

Use clean humidifier

Utilize the recommended pharmacological and nonpharmacological recommendations
if you have a fever that is higher than 102° F, call your doctor right away

 

Health maintenance

Follow all vaccinations recommendation

 

Follow up

Follow-up visit or telehealth call within 48-72 hours (Dunphy et al., 2019).

The patient can face a language barrier at the appointment, which is a social factor of health. For these patients, it’s crucial to make sure they have access to a medical interpreter. There are various methods of interpretation that can be utilized, including live, telephonic, and video.

I strongly encourage that you do not wait until the very last minute to complete your assignments. Your assignments in weeks 4 and 5 require early planning as you would need to present a teaching plan and interview a community health provider. I advise you look at the requirements for these assignments at the beginning of the course and plan accordingly. I have posted the YouTube link that explains all the class assignments in detail. It is required that you watch this 32-minute video as the assignments from week 3 through 5 require that you follow the instructions to the letter to succeed. Failure to complete these assignments according to instructions might lead to a zero. Your initial discussion post should be a minimum of 200 words and response posts should be a minimum of 150 words. Be advised that I grade based on quality and not necessarily the number of words you post. A minimum of TWO references should be used for your initial post. For your response post, you do not need references as personal experiences would count as response posts. If you however cite anything from the literature for your response post, it is required that you cite your reference. You should include a minimum of THREE references for papers in this course. Please note that references should be no more than 5 years old except recommended as a resource for the class. Furthermore, for each discussion board question, you need ONE initial substantive response and TWO substantive responses to either your classmates or your instructor for a total of THREE responses. There are TWO discussion questions each week, hence, you need a total minimum of SIX discussion posts for each week. I usually post a discussion question each week. You could also respond to these as it would count towards your required SIX discussion posts for the week.

I understand this is a lot of information to cover in 5 weeks, however, the Bible says in Philippians 4:13 that we can do all things through Christ that strengthens us. Even in times like this, we are encouraged by God’s word that we have that ability in us to succeed with His strength.
