A mandated reporter is anyone who has a full, intermittent, or occasional responsibility for tha care or custody of an incapacitated, or dependent adult. A mandated reporter states that all licensed healthcare professionals, including but not limited to: Physicians, Nurses, Mental Health Professionals, EMTs, paramedics, medical examiners – all employees in a long‐term health facility, social worker, marriage & family consular, childcare custodians, elder or dependent adult, employee of a protective service or law enforcement agency. I am in the State of Maine and Maine law requires hat certain people report immediately to the Adult Protective Services when the person knows or has reasonable cause to suspectthat an incapacitated or dependent adult has been or is likely to be abused. Many states, including Maine, require that a mandatory reporter provides his or her name and contact information during the initial report.
I once took care of a 76 year old patient who was admitted from his family home where he lived with his grandchildren. Patient was admitted for severe malnutrition, altered mental status with confusion. Patient had autism, was non-verbal and was impulsive to healthcare staff, as we tried to give him care, however he later on calmed down and became more relaxed around health care workers. He showed signs of elderly abuse and I had to let the Nurse Manager know before i could proceed to Adult Protective Services because Maine State requires immediate reporting, of any patient who is admitted and requires the medications and I am also a Mandated Reporter. However it is a criminal offence to withhold reporting child/adult abuse and it also predisposes further child or adullt abuse, if action is not taken.

Mandatory reporters are the ones who are required to report legally by law, failure to do so consider a crime. Their job is to report any suspected abuse, neglect, financial abuse not to investigate even when they are just suspect (Marschall, 2021). Every state in United State has a specific law that is mandatory and reports and ways of reporting. All healthcare workers (physicians, nurses, caregivers), teachers, social workers, therapists are Washington State’s mandatory reporters should report any suspected abuse, neglect, abandonment of any vulnerable adult or child. In Washington state suspect abuse in any care services facility report can be called by calling toll-free number 1-800-562-6078 or online report. For reporting person name, address, contact information, reason of concern, and report name and number for investigate purposes. The reporter’s information should be kept confidential unless court action or law enforcement has been called in. In cases when vulnerable children or adults are suspected of abuse or neglect in their home reports can be made to adult protective or child protective services (Washington State Department of Social and Health Services, n.d.).
In case of scenario, one female older adult age 65 years admitted to hospital with fall from a private residence, upon assessment her
condition with weight loss, bilateral feet gangrene, and bad odor, extremely poor hygienic condition suspected case of abuse or neglect, in this case, adult protective services called (Washington State Health Care Authority, 2021).
