I had a Chinese male patient in his 70s who was alert and oriented. Upon hand-off he was really grumpy and mad at the outgoing nurse and aide because they were not letting him get out of bed. He was yelling at everyone and even threatened to leave against medical advice. His wife was there but she remained quiet, and eventually left. I went in and talked to the patient calmly. He told me that he is the head of his family and that he is used to doing things independently. It is annoying enough for him to be connected to IVs and monitors, and not being allowed to get out of bed to go to the bathroom was just too much for him. I know that many Asian cultures are patriarchal and the father is usually the one who makes decisions and does many things for the family. I realized that my patient wanted that independence and sense of control. I explained the protocols at my work and the safety risks. Then, seeing that he is not a fall risk, I gave him some freedom by letting him go to the restroom and walk around the unit. He calmed down a lot and became compliant and cooperative to all the staff. He slept through the night and did not refuse things that we needed to do in the wee hours of the night (labs, vital signs, medications). His wife came the following morning and she observed that her husband was smiling and in better spirits. She also looked relieved to see her husband in a better mood.

Ultimately, understanding the patient’s background and cultural characteristics is key in providing culturally competent care. There are a lot of times when we have to make adjustments to our tasks or schedule to accommodate our patient’s needs, but doing so can have more positive outcomes.

Medical Records: I work in an observation unit where we admit many patients every day. Since most of our patient comes from ER, they all tend to have some kind of medical record.
