What comes to mind for me and this goes way back, at least 15 years ago, is when we went to an electronic documentation system. The “sacred cow” experience was not just present with the nursing staff, but more pointedly towards the providers. We had a great deal of push back from the older physicians who were not taking too well to the computer. They simply wanted to dictate orders for nurses to write down or write the orders down in a chart. I remember taking a great deal of time to sit next to these providers and aide them in entering their orders. They eventually caught on.

As future nurse leaders, addressing sacred cows and embracing evidence based practices will most likely require a great deal of patience and persistence. Knowledge and education is always key in the profession of nurses. As acute care nurses, we undergo a great deal of in-services and updates. This would most likely be the approach I would take here. I believe the key element in promoting actual acceptance and practice of new elements into the nursing field is making the topic relevant and appealing to the staff. Promotion of EVP tied into the new adaption of practice is detrimental in creating the importance of “the why” its a necessary change.

This is an interesting topic, I graduated as a nurse in the middle of COVID where everything drastically changed for the medical field, and we continue to suffer from the changes. What I have noticed is that yes hospitals have so far been the best in giving new grads the opportunity to grow, but there has also been a lot of burn out that happens to these nurses within their first year. I speak from experience that I was looking for other job opportunities because I felt I was not being heard by management but decided to drop to part time after a year to help with my mental health instead and to keep me from treating it as “just a job” rather than a career or profession. Most of these issues arise from management not putting the needs of their nurses first, it’s understandable that it has been a difficult time and we are all still learning to cope but understanding that listening and trying to help the core staff of the hospital reach their goals and knowing that management is trying to help them would retain more staff.

As a future nurse leader, addressing “sacred cows” and embracing evidence-based practice in the face of resistance to change requires a multifaceted approach. It is crucial to foster an environment that encourages open dialogue and critical thinking. By creating platforms for discussion and inviting diverse perspectives, nurses can challenge long-standing beliefs without fear of retribution.

Similarly, emphasizing the importance of evidence-based practice is essential. Sharing research findings, case studies, and success stories that demonstrate the positive impact of change can help overcome resistance. Educating staff about the benefits of evidence-based practice, such as improved patient outcomes and increased efficiency, can motivate them to embrace change. On top of that, involving staff members in the change process can enhance acceptance. By engaging nurses in decision-making, seeking their input, and addressing their concerns, leaders can empower them to become change agents. Collaboration and shared ownership can facilitate a smoother transition from traditional practices to evidence-based approaches. Adopting the above strategies can help nurse leaders to overcome resistance and promote a culture of continuous improvement and excellence in patient care.

Today I actually attending a training that addressed changing our normal routine and practices with patients and following more of a physiological patient led experience. My training was about the use of peanut balls in labor and in pushing with a patient. My unit all agreed that we were nervous to implement these strategies because of fear of push back from the provider. A few of the ideas we came up with was to slowly implement the change. This will allow the provider to see the improved outcomes from patients who use the strategies to those who don’t. We also decided to present the research to the providers and show how it is backed up by not only AWHONN but ACOG. Providers love being shown the research and facts so they can then do their own research and see that the use of peanut balls cuts the c-section rate and lessens the 1st stage of labor by up to 90 minutes. My favorite suggestions was to just implement the change because there is no policy against it and we have proper education on it. Setting up the room, educating and asking permission from the patient and then having the provider come in the room when its time for baby with the providers stool and tools set up how it needs to be for a peanut ball delivery already lessens the anxiety and “what if’s” the provider may have. After my own research and practice, educating other staff on what to do, research, demonstration and positive outcome stories may also encourage and lessen the anxiety and promote change in our practices as well.

That article made some really good points. I remember a colleague that was in nursing school talking about how she wanted to work in L&D after graduation but everyone was telling her to start in med surg and get the experience there first. Myself and some other nurses encouraged her to start wherever she feels drawn to start. The amazing thing about nursing is you can start in one specialty and end up in a completely different specialty later in life. Discovering what interests you and what stresses you out too much in nursing is going to be done throughout all of our careers. Nurses are needed in so many different settings, especially outpatient. As we work towards more health promotion and preventative health education, we need more resources outpatient.

When it comes to addressing sacred cows, I believe policies are a very important resource. If hospital policies are being created using the most up to date evidence-based practice, this is a great way of discussing and educating. I think sacred cows require a why when they are being told about a new practice. Not that anyone should be responding with a “because I said so,” when asked why, but I especially think the nurses set in their old ways would benefit more from explaining why it is best to do it this new way and ask them to give it a try.

Diversity among individuals, as well as cultures, provides a challenge for nurses when it comes to delivering meaningful health promotion and illness prevention-based education.
