I was not impressed with this article. It seemed very dated, and I felt it was hard to read. The article possibly could be reflecting on one area of nursing, I’m not sure.I have been in and out of the nursing world while raising my children for 15 years and recognize that my knowledge and experience may be dated as well.
In my experience, the family/caregivers have always been part of the care plan, interventions, and health care team. I spent time in home health and the caregiver was always included in our interventions and observations. Most often, it was a necessity to have a caregiver or family present for certain visits. Our role as the home health nurse was to teach and educate patients and family on how to manage at home. There were a few instances in which the patient could be independent. In both cases, we used local resources to assist. Caregiver strain was a regular topic discussed and documented on. It was always an intervention of mine to allow time for caregiver to express feelings and follow up on “time off” activities that were accomplished, especially with our total care/high acuity patients. This also correlated with the patient to assess that their time way from the regular caregivers was healthy.
The change I note, is more revolved on what a family unit looks like now and how that relates to the primary caregivers. The evolution from the traditional female spouse or daughter caregiver could now be grandparents and grandchildren family unit, gay partner/spouse caregiver, transgender adult child as caregiver, an in law family member , roommate, or the total absence of a caregiver related to no family unit.
Having proper community resources, like caregiver support groups and adult day care centers in local area were always handy to have for families.
In this article I found their insight very refreshing. I really resonated with ” a family unit will always be greater than the sum of each of the members that compose it, and that any alteration of one of its members affects the whole system”. This holds so much truth to it. As a nurse in a hospital setting you often see the affects a hospitalization has on the family unit. The kids needing someone to pick them up from school since ‘mom” is inpatient, or financial concerns because the breadwinner is hospitalized. Illness and changes in health always have a trickle affect in a family unit. A very clear picture of your patients social determinants of health usually emerges. More often than not a social worker is closely involved with any patients we identify as experiencing any type of hardship. This is usually relayed by the family versus the patient themselves.

Thinking on the fact of the aging population, the low birth rates, increased life expectancy, and long term care patients require, it is easy to understand how important understanding family dynamics can be. It is instrumental in treating the patient as a whole and most importantly creating a successful plan of care at the facility and at home. I found it interesting how the article mentioned several times about changing policy within the government. I could not agree more. I have read many studies on the effectiveness of health promotion in a patients home environment versus at a post care facility. The numbers are staggering. If only medicare and medicaid covered at home support. When I say this I mean, caregivers, nurse aides, care that can be delivered to support the patient not just medically but in every day life, ADL’s. Plans do cover temporary support, however, it is limited. As nurse liaison, I see patients going to long term facilities due to inadequate support at home. These families unable to make bills if they do not continue working. More support via policy change within the medicare/medicaid system would be a huge step in supporting the patient and the family dynamic.
I like that there are two methods to include how family can be a part of the care plan. As I had shared in my discussion about my brother being diagnosed with type 1 DM, I can understand why there are two methods, one, due to the patient being an adult who can care for themselves and have family also put in effort to keep the patient accountable and know how to care for them in case there is an emergent situation that occurs and the second method, I like to view that as the situation we had to go through with my brother, it consists more of family to ensure the proper care is given and how to integrate that into the entire household. For example, we had to make sure we didn’t have too much sugary items in our house, learn signs and symptoms of hypoglycemia and hyperglycemia and know how to treat it. For my brother being only eight, there was a huge barrier in only teaching him about his diagnosis but also including those who he is around or who took care of him daily. I believe that in any case with newly diagnosed illnesses integrating family involvement leads to positive health outcomes as the individual is not alone or having to deal with the illness alone but instead feeling supported by those around them.
This was insightful and I agree with some of the writer’s disposition and points discussed in the article. The writer posed some solid ideas making reference to the importance of addressing not only the patient but also the care giver. In my experience especially working in child and adolescent psych, nursing worked closely with social work and other inter disciplinary teams to address and to treat the whole family system. In all cases, familial and/or the child’s support system was always incorporated and fully engaged in the care plan of the patient. Education was always provided along with emotional support and guidance for family members. More so, the interdisciplinary team worked cohesively together to find solutions to aid in financial issues or find emotional support resources available to family members in their community that were actively involved with the client’s care. I think its also important to consider in some cultures great reverence and priority is taken when caring for their family members and may not view this as a “burden” but more as their purpose. Providing stronger resource to meet the needs of the patient and familial care takers should be the central goal to providing the best care needed for this type of family dynamic.
According to the assigned article, “Health Disparity and Structural Violence: How Fear Undermines Health Among Immigrants at Risk for Diabetes,” narratives tell the story of the interconnectedness between fear and health. Thematically, the issue of fear is a dominant feature that affects how an individual approaches day-to-day living and health. I strongly encourage that you do not wait until the very last minute to complete your assignments. Your assignments in weeks 4 and 5 require early planning as you would need to present a teaching plan and interview a community health provider.
