The community health nurse plays a vital role in ensuring community health, partners with community stakeholders and has numerous roles which include integrating community involvement and knowledge about the population with personal, clinical understanding of health and illness and experiences of individuals and families within the population. Evaluating health trends and risk factors of population groups and helping to determine priorities for targeted interventions for example malnutrition, sexually transmitted infections and diabetes mellitus. The nurse provides health education, care management and primary care to individuals and families who are members of a vulnerable population and high risk groups. The nurse assesses and evaluates healthcare services to ensure that people are informed of available programs and services to ensure that people are informed of available programs and services and assisted in the utilization of these services for example screening, therapy and counseling. It is important to appraise Faith Based Organizations because they important venues for health promotion especially in underserved communities. The role of a community health nurse is to apply the principles of nursing, social sciences, and public health, to protect and enhance the health of a community based on its unique needs. The philosophy of community health nursing is in contrast to that of common medical practice. When a patient becomes sick or injured, medical practice intervenes to bring them back to their former state of health. In contrast, community health nursing takes a more proactive approach to healthcare by improving the health of communities to decrease the occurence of hospitalizations. It is important that community health nurses and community stakeholders collaborate to provide effective care to a population. Community stakeholders are individuals who are directly impacted by the health of a community. An example scenario that illustrates this relationship would be community health nurses organizing COVID-19 vaccination clinics at a church. The pastor is the stakeholder who understands the risk of a COVID-19 outbreak and therefore urges parishoners to become vaccinated. Without the pastor’s support, vaccine hesitancy can undermine the credibility of the community health nurse, placing the community at continued risk for COVID-19 outbreak. It is important to appraise community resources during a community assessment because it guides planning for community nursing interventions. Community health nurses play a vital role in public health promotion as they communicate with the community stakeholders such as household heads, parents, business owners, etc. in order to carry out an assessment about the attitudes and knowledge that is needed to plan and design the healthcare awareness programs (de Mello et al., 2018).  This assessment is to determine facts and attitudes about required and accessible health services for the community. As opposite to Community-based care which focuses on health promotion, disease prevention, and restorative care.
