Hello, Dr. Arredondo and Class
I would like to start by saying this course has been very demanding yet, an enhancing course. The course needed prudent time management so as to complete assignments, readings, quizzes and exams. The advanced pathophysiology course has prepared me to function in an advanced practice role by offering a detailed understanding of the pathophysiological processes, allowing me to predict clinical symptoms, choose evaluative studies, begin appropriate treatments, and plan for possible complications (CCN, 2018). Insights into the underlying disease process will enable the nurse practitioner for incorporation of current and groundbreaking interventions. Relevant screening and diagnostic testing approaches will also be incorporated.
Program outcome
The MSN course program outcomes included delivering high standard, safe patient centered- care by forging a caring atmosphere and taking part in long lasting personal and professional development and advocating for positive health outcomes in virtue of evidenced-based synergetic advanced nursing practice (CCN, 2018). Course outcomes require exploring pathophysiology processes, investigating the approach in which homeostatic, adaptive and compensative physiologic processes may be sustained or altered through specific interventions, risk factors, and approaches of diagnosis and therapy of health problems in chosen populations and analogous research conclusions to management of patients with multifaceted pathophysiologic dysfunction (CCN, 2018). The course outcomes allow the advance practice nurse to utilize pathophysiology to support clinical decision making concerning diagnosis and therapy of acute and chronic presentations frequently managed by nurse practitioners (CCN, 2018).
MSN Essential
Essential IV: Interpreting and incorporating scholarship into practice (AACN, 2011). The master’s prepared nurse should apply research findings within the practice environment, solve issues, work as a change advocate and promulgates results (AACN, 2011). The case study assignments we did in this class allowed me to employ evidence, clinical discernment and interpreting methods to enhance related outcomes for patient groups (AACN, 2011). The advanced practice nurse must be able to articulate to a diversity of audiences, the evidence-base for practice determinations, in addition to the reliability of sources of evidence and the pertinence to the practice problem encountered (AACN, 2011).
Nurse Practitioner Competencies
Nurse practitioners have an extensive range of knowledge and expertise, which they bring into play in their appraisals and decision making to begin application of interventions to patients (Stanford, 2016). Their work is all evidence-based (Stanford, 2016). Competencies may be seen as a mixture of practical and academic knowledge that enhances performance or the capability to carry out a specific role (Stanford, 2016). The word advance practice is suggestive and oftentimes conferred in the context of a degree of practice with autonomy supported through skillful knowledge basis and clinical expertise through expanded and extended roles (Stanford, 2016). As advanced practice nurses we should utilize information in this course and use this information to evaluate patients, make diagnosis, order and translate diagnostic exams and conduct treatment plans in addition to prescribing medications as learnt from this course.

Hello Professor and Class!
Program Outcome #1: Provide high quality, safe, patient-centered care grounded in holistic health principles. (holistic health & patient-centered care)
- This class has helped me to achieve program outcome #1 by focusing on the different ways in which the patient should be part of their own care. By this I mean that in each case study that we worked with, we were asked the ways that we would educate the patient about their illness. Patient education is extremely important so that the patient feels that they are being treated like an actual person instead of just some body that we are trying to fix. By speaking with them and talking things through with them, the patient becomes the most important thing, not the illness.
MSN Essential 1: Background for Practice from Science and Humanities:
- Through all of the research that we did in this class for the various case studies and the project that we did, we were encouraged to gather resources from a number of different backgrounds. For example, in my project on Rheumatoid Arthritis, I used sources for nurses, sources that focused on the public health aspect, sources that focused on the psychological aspect of the illness, etc. I did not just look at and use sources directed at nurses. This research showed me that there are many different resources that can help me help the patient in a number of different ways.
Nurse Practitioner Core Competencies #1 Scientific Foundation Competencies: 1. Critically analyzes data and evidence for improving advanced nursing practice
- I decided to focus on the first core competency for this discussion. Reading through the case studies that we had each week taught me to look for the most important evidence within a patient history. Sometimes they are going to tell us things that we don’t necessarily need to know and it is up to us to decide what is important for a diagnosis and what is not. With that being said, we need to critically analyze this evidence and apply it to a number of different resources to make sure that we are correctly diagnosing our patients.
Thank you guys for an awesome class!

Hello Professor and Class,
I can truly say this class has challenged me over the past eight weeks to become a more effective nurse and a better student.
Program Outcome #1: Provide high quality, safe, patient-centered care grounded in holistic health principles. (holistic health & patient-centered care)
I have learned numerous ways to provide quality, patient centered care based in holistic health principles through the Grand Rounds sessions. These presentations consist of discussing medical problems and treatment methods that benefit the patient on a holistic level. In particular, the Grand Rounds session, “Hypothyroidism,” was extremely helpful in providing information for my week 6 PowerPoint presentation on hypothyroidism. The American Thyroid Association 2017 Pregnancy Guidelines compared to the 2011 Guidelines were noted in the presentation. The presentation made specific mention to the management of hypothyroidism during pregnancy and included recommendations regarding the interpretation of hypothyroidism tests during pregnancy, iodine nutrition, thyroid autoantibodies, complications during delivery, lactation concerns, and fetal and neonatal considerations to include thyrotoxicosis in pregnancy.
Through the weekly readings, I have augmented my understanding of evidence-based research and how it shapes and determines cost-effective and sustainable evidence-based outcomes that promote constructive behavioral changes in high-risk patient populations. The Week 4 reading lesson “Alterations in Renal Function” included information about disease causation. While I knew that urinary calculi (stones) are a common cause of functional urinary obstruction, I learned obstructions must be addressed quickly to prevent backflow of urine into the kidney before the condition results in permanent renal dysfunction. I can discuss with patients who have stones or are at risk for stones the best ways change their diet to balance their urine pH levels since the pH of the urine increases the likelihood of stone formation. I learned a good prevention or screening method is to teach patients the ideal pH levels for urine and what tests they can take to test the pH of their urine and saliva. Low pH increases one’s risk of developing uric acid stones and a high pH increases one’s risk of developing calcium phosphate stones. One of the goals of holistic care is to help patients understand the underlying cause (s) for their diseases and how this plays a role in how diseases are diagnosed and treated. In terms of providing evidence-based holistic care, I have learned that before I apply an intervention, I must decide if it should be directed at the person, agent, or environment. This leads to better patient outcomes and more effective nursing practices.
MSN Essential I: Background for Practice from Sciences and Humanities:
Throughout my graduate course studies, I have learned the importance of integrating scientific findings from various fields of nursing in my writings and in my practice. Healthcare research constantly generates large amounts of results regarding revised treatment methods for nurses and care management for patients. The lessons reiterate that if implemented in practice, scientific findings can help save lives and improve patients’ recovery times and quality of life. For example, the consideration of how culture affects one’s access to and acceptance of healthcare initiatives teaches me that nurses must carefully and thoughtfully analyze sociodemographic trends if they want to make an effective impact on culturally diverse patient populations. Reversing negative health outcomes for vulnerable populations begins with studying scientific findings appropriate to culturally diverse groups and taking the responsibility to acquire cultural competency skills. The weekly discussion posts helped to hone my skills in integrating and applying scientific medical findings. For example, when we learned about cystic fibrosis (CF) in Week 2, I learned the CDC estimates more than 10 million Americans are carriers of a faulty CF gene, but have no idea they are carriers (NCBI & NHLBI, 2018). Cystic fibrosis affects males and females from all racial and ethnic groups; however, the disease is most common among Caucasians, Latinos, and Pueblo and Zuni American Indians (NCBI & NHLBI, 2018). With this information, I can encourage my patients in their childbearing years who fall under the at-risk population category to get screened for the CF gene if they plan on having children.
Nurse Practitioner Core Competencies # 1 Scientific Foundation Competencies
I have demonstrated my competency in analyzing, integrating, translating, and developing my understanding of scientific foundation competencies through the papers, weekly discussions, and presentations required in this class. One type of assignment in particular stands-out to me. The weekly faculty question required us to present evidence-based methods to the class and interpret the research question, methodology, results, and conclusions from what we had read and link it to another adverse health condition. This taught me the importance of critically analyzing the information in the article and translating the research. I understand now that the faculty question was meant to teach me the etiology of increased risk to develop another disease or condition when a patient is already being treated for a specific condition or disease is multifactorial and connected to traditional and patient specific symptoms. Furthermore, when gathering the data and information for my Week 6 speech and PowerPoint presentation, I was able to integrate what I had learned about culture and disease progression to determine the best way to approach women who suffer from post-partum about hypothyroidism.
In conclusion, I believe I still need to work on developing new approaches to nursing practice regarding specific diseases and serving various patient populations. To do this not only takes confidence but knowledge and understanding. I will feel more confident about doing this after I graduate and have practiced as an FNP for a few years.

This class has provided an in depth review of the pathophysiology of the major organ systems of the body. Over the course of the class discussions, quizzes, and assignments have provided a platform to meet program outcome #1, MSN Essential I, and Nurse Practitioner Core Competencies #1 Scientific Foundation Competencies.
MSN Program Outcome #1:
MSN program outcome #1 focuses on providing safe holistic patient care. This outcome was achieved through the recorded presentation in week 6 and the discussion in weeks 2, 3, and 5. The presentation that was recorded during week six used evidence based practice and understanding of pathophysiology to provide information and interventions for patients suffering from that disease. In weeks 2, 3, and 5 the discussions provided an opportunity to learn about specific diseases and create a plan of care for the patients in the scenarios. Providing high quality, safe, patient centered care requires an understanding of the pathophysiology of the disease in which the patient suffers from, and using evidence based practice to provide interventions aimed at promoting overall wellness.
MSN Essential I: Background for Practice from Sciences and Humanities
The MSN Essential I: Background for Practice from Sciences and Humanities requires mastered prepared nurses to have an understanding of the implications that scholarly articles and evidence has on practice. Healthcare continues to evolve and improve through the continual pursuit of new information and interventions that serve as evidence to improve patient wellness. Scientific finding were used throughout this course in both discussion and the week 6 recorded presentation. The discussions provided an opportunity to research diseases included DKA and CHF, and provide information on interventions that are essential to improving the patients symptoms. The week 6 recorded presentation was a platform for discovery of evidence that can be used to improve nursing practice. Nurse practitioners provide primary care to a variety of patients with a variety of disease. Week 6 presentation covered IBS and through research it was determined that a thorough history and physical is essential for determining if a patient has IBS or something more serious like colon cancer.
Nurse Practitioner Core Competencies #1 Scientific Foundation Competencies
The Nurse Practitioner Core Competencies #1 Scientific Foundation Competencies focuses on mastered prepared nurses having the ability to critically analyze data, integrate knowledge and data, translate data into evidence based practice, and develop new practice based on research. This competencies was achieved through discussions week 2, 3, and 5, week 6 presentation, and weeks 1-6 quizzes. Discussions required students to research diseases and interventions that improve patients with specific diseases. For instance, in week 5 discussion one a patient with DKA was focused upon. Through research it was discovered that to prevent cerebral edema appropriate fluid resuscitation is essential, and the use of bicarbonate is discourage unless a patients HCO3 is critically low (Kamel & Halperin, 2015). This information is important for creating protocols that can be used in patients who present with DKA. The quizzes provided an opportunity to express the knowledge that was gained throughout the week, and this information can be used to integrate knowledge into practice. The week 6 presentation challenged the student to use evidence from research to provide interventions for patients suffering from a specific disease. Data for patients with IBS showed that those who consumed more foods classified as FODMAP foods suffered from IBS symptoms at a higher rate than those who didn’t (Kortlever, Hebblethwaite, Leeper, O’Brian, Mulder,&Gearry, 2016). This information provided the opportunity to improve practice through evidence.

Dr. Arredondo and class,
I have learned many things throughout this class and the classes before that I hope will continue to prepare me for the goal I am working to accomplish of the family nurse practitioner. Below I have discussed how this class, as well as classes before, has prepared me for this role and meeting the outcomes, essentials, and competencies in this program.
Program Outcome #1: Provide high quality, safe, patient-centered care grounded in holistic health principles. (holistic health & patient-centered care).
When in the role of the family nurse practitioner it is important to give the safest high-quality care that is patient centered. It is easy to forget that each patient is different and that while the diseases processes might carry certain components, the patient might present in a different manner due to differences in the genes and DNA of the individual person. Over this past 7 weeks I have learned many new things that has allowed me to meet certain components such as program outcome #1. The first example of criteria that I learned to help e achieve this outcome is the study of pathophysiology as it pertains to each body system. Each week we learned about a different body system and how each component of that system works in order to maintain normal function day to day. This will allow for me to understand how the systems should work for all patients that I see. Understanding this will allow for me to recognizes abnormalities and provide quick, safe, and high-quality care when a abnormality arises. The second example of criteria that I learned to assist me in meeting this outcome is gaining knowledge of alterations that arise for the adult and child in each body system. Many disease processes exist, and it is essential to understand the disease processes to provide the best quality if care. Each week we gained knowledge of the disease as well as how the cells perform and malfunction when these disease processes occur. Understanding the alterations will allow for high quality and safe care for all adult and pediatric patients that I might come into contact with as the family nurse practitioner.
MSN Essential I: Background for Practice from Sciences and Humanities:
Recognizes that the master’s prepared nurse integrates scientific findings from nursing, biopsychosocial fields, genetics, public health, quality improvement, and organizational sciences for the continual improvement of nursing care across diverse settings.
During this class we focused on the on the pathophysiology of the normal processes of each body system as well as the alterations that can arise in each of these body systems. While understanding the pathophysiology is important, it is also important to understand that healthcare is diverse and information from variety of places is essential to provide proper care. This information is how MSN essential 1 has been met in this course. One example of how this essential has been met is each week when case studies were presented. I had to research the information in the case in a variety of places that also included a variety of views from individuals. In healthcare it is essential to be flexible and search for the most up to date information in order to provide the best care possible. Another example of how this essential has been met is the information gained from the selected disease process by Dr. Arredondo. My assigned disease process was chronic renal disease and in order to find all of the needed information I done research and sorted through a variety of views to put together the presentation. I also focused a small portion on the team approach of managing this disease after being asked about this information by one of my class mates. I learned that team work is essential in treating the patient as a whole. New knowledge occurs daily and studies pertaining to better care is constantly being produced. As the family nurse practitioner, it is essential to stay up to date on the current standard of care in order to provide the safest, best care for the patients.
Nurse Practitioner Core Competencies # 1 Scientific Foundation Competencies
- Critically analyzes data and evidence for improving advanced nursing practice.
This competency was met every week while preparing the discussion case study answers as well as studying for the weekly quizzes. Having the ability to gain an understanding of the pathophysiology and disease processed from the most of to date data and evidence-based information has prepared me for the role of the family nurse practitioner and providing the most up to date safest care to patients.
- Integrates knowledge from the humanities and sciences within the context of nursing science.
This competency was also met every week while performing the assigned readings, lesson, and research for the questions being asked. This class allowed me to gain an in-depth knowledge of each of body system and also the process of the diseases that we commonly see in the healthcare field. Understanding that while treating the disease process, we are also treating an individual person is important in providing the best care as well as establishing trust with the patients. Previous classes have taught of us the importance of patient centered care and that information was often used in this class when discussing the disease processes as it pertained to a specific individual such as in the discussion case studies.
- Translates research and other forms of knowledge to improve practice processes and outcomes.
This competency was most met when researching all of information for the presentation of the assigned disease process. While the research was helpful in applying the factual information, using precious knowledge was also important in understanding the role of the nurse practitioner as well as the education that must be implemented in order for the patient to have the highest quality of life independently. It is essential for the nurse practitioner to stay up to date to provide the best care and education for the patients.
- Develops new practice approaches based on the integration of research, theory, and practice knowledge.
This class as well as the classes before has prepared me for experiences that I will possibly encounter in the future. Using the knowledge in regard to pathophysiology as well as information such as nursing theory, statistics, and other up to date research, I feel that I have been prepared to continue my knowledge in areas such as pharmacology and anatomy in order to prepare for the upcoming role as the family nurse practitioner. I hope that with all this knowledge I can provide the best care possible to all patients I will encounter.

Week 7 Reflection
Dr. Arredondo and Class,
Program Outcome #1 Provide high quality, safe, patient-centered care grounded in holistic health principles. (holistic health & patient-centered care).
During the weeks of required case-studies, I learned how to search for accurate, scholarly, and peer-reviewed literature to support my responses. Not only did I learn an extensive amount of information on Addison disease, diabetic ketoacidosis, cystic fibrosis, and CHF but I also have a clear understanding of signs and symptoms, lab tests, and recommended teaching for patients. As a nurse practitioner, a holistic approach to patient care is the most fundamental aspect of nursing care and will be a strong foundation in each patient-provider relationship in my career. I appreciate the challenge of the case studies and how it encouragedme to think critically about diagnosing and treating each patient.
The weekly lessons, workbook exercises, and textbook readings provided a solid understanding of each body system and disease process of those systems while inspiring me to continue to seek information about diseases from credible resources such as journals and government agencies. Disease in children such as cancer, pyloric stenosis, congenital heart disorders, and respiratory illnesses may be due to genetics, environmental, or socioeconomic factors, and requires a full patient history including family background related to disease and illness. Patient education, compliance, and follow-up are keys to positive outcomes. A clear understanding of the disease process, and what is required to improve health must be assessed by the nurse practitioner so that necessary adjustments in the care plan can be made. Individualized care plans will cover aspects of care such as medications, side effects, follow-up visits, and required routine labs. Each chapter in the class book, Advanced Pathophysiology, included the essential patient treatment and teaching requirements for each disease or condition presented in the text.
MSN Essential I: Background for Practice from Sciences and Humanities: Recognizes that the master’s prepared nurse integrates scientific findings from nursing, biopsychosocial fields, genetics, public health, quality improvement, and organizational sciences for the continual improvement of nursing care across diverse settings.
Each assignment given in this class encouraged me to do extensive research for quality scholarly articles relating to subjects such as Addison disease, cystic fibrosis, CHF, diabetic ketoacidosis and more. I have a clear understanding of how important peer-reviewed scholarly sources provided evidence for providing care to all patients. I understand that many unreliable sources are also available on the internet such as .com sites, and sites that do not have a professional peer review such as a physician’s approval. It is crucial to apply solid evidence-based research in patient care by utilizing proven or gold standard methods.
The U.S. Preventive Services Task Force (USPSTF) is a reliable source for primary care providers and provides valuable information for both healthcare professionals, and consumers.
The USPSTF (2018) is a volunteer panel of national experts in prevention, recommendations, guidelines, and evidence for the health and well-being of all Americans. Guidelines for screening, immunizations, treatment, and education can be found on this website. Volunteers come from many different areas of medicine including, family practice, psychiatric, pediatric, obstetrics, and gynecology. All recommendations are published in journals or on the USPSTF website.
I am glad I learned about this website in this class, and I am confident I have a solid and reliable source to refer to during my career as a family nurse practitioner.
Nurse Practitioner Core Competencies #1: Scientific Foundations Competencies
- Critically analyzes data and evidence for improving advanced nursing practice.
- Integrates knowledge from the humanities and sciences within the context of nursing science.
- Translates research and other forms of knowledge to improve practice processes and outcomes.
- Develops new practice approaches based on the integration of research, theory, and practice knowledge.
I have a clear understanding of what the requirements are for a scholarly, evidence-based article which will provide relevant information for primary care practice. The case studies in this class allowed me to improve on my research methods and recognition of scholarly articles. I was able to find many peer-reviewed journal articles that I used in supporting my case scenario responses for graded discussions and case scenarios.
I also found that research analysis or meta-analysis articles helped to reinforce my documentation by providing evidence of treatment outcomes, diagnostic testing, or subject comparisons and provided a deeper understanding of the current topic. I was able to provide research evidence as evidenced by my in-text citations and reference page in each graded discussion assignment.
I feel confident that any research I do in the future regarding patient care, I will fully recognize true evidence-based findings and data related to my inquiry. I understand scientific evidence is the foundation for all patient care and must be expanded upon by furthering research efforts and through my contribution to nursing science and quality patient care.
Regards,
Julie

Week 7 Reflection Discussion
Program Outcome #1: Provide high quality, safe, patient-centered care grounded in holistic health principles. (holistic health & patient-centered care)
Throughout this course, the program outcome #1 was exemplified in all assignment and discussions. In week 2, we discussed cystic fibrosis and focused on the pathophysiology behind the disease. In addition to etiology and pathophysiology, the outcome was met by the research and discussion on teaching for the patient. As advanced practice nurses, it is imperative to incorporate holistic care in our everyday practices, including physical, spiritual, emotional, and cultural wellbeing. Our weekly discussion, all ended with teaching that would be provided to the patient and their caretakers. This provided the opportunity for high quality, safe care through education. In addition to expanding patient’s knowledge, this course assisted me in further understanding the background behind diseases and further dissecting each diagnosis. When providers understand the pathophysiology behind each disease, it assists in providing better care for our patients.
NR 507 met the goal of MSN Essential 1 by weekly assignments requiring research using scholarly articles that further emphasized the information that was being taught in the weekly lessons. Advanced practices nurses rely on research to stay abreast evidence-based practices. As medicine continually changes, it is imperative for nurse practitioners to utilize research to provide the best care for their patients. By nurse practitioners staying up to date through scientific research, they are able to also provide information that can be applied to not only their everyday practice, but also practices within their facility they are employed. Evidence based practice is the best practice. APN’s can focus on improving patient care by focusing on their community impact by using scientific exploration.
Nurse Practitioner Core Competencies # 1 Scientific Foundation Competencies
- Critically analyzes data and evidence for improving advanced nursing practice.
- Integrates knowledge from the humanities and sciences within the context of nursing science.
- Translates research and other forms of knowledge to improve practice processes and outcomes.
- Develops new practice approaches based on the integration of research, theory, and practice knowledge.
In the duration of this course, NR 507 provided the foundation for the Nurse Practitioner Core Competencies #1. The week 6 disease project focused on a specific disease. For my assignment, it was asthma. This outcome was met with this assignment by using scientific evidence and research to explore the dynamics of asthma from its etiology to the treatment options for the respiratory disease. In this assignment, I used statistical data to bring awareness to the severity of how many American’s affected by this diagnosis. The knowledge gained from this assignment improves my future practices, because the research that was done gave me a better understanding of the disease and how to diagnose and treat asthma.

Dr. Arredondo and class,
This course has been very challenging yet very rewarding to me on so many levels, and it has prepared me for the rigors of graduate instruction as well as the processes of pathophysiology. The case studies were extremely helpful in understanding some of the more common disease processes. The case studies provided valuable information and helped me know the signs, symptoms, lab values and diagnostic evaluations.
This course has helped me advance from my foundational background in nursing to understanding more complex pathophysiological processes in everyday practice. It has helped grow in the process of thinking like an FNP by integrating evidence-based care the patient. The evidence-based practice has been recognized as the safer approach for the patient in many aspects. It provides nurses with an up-to-date knowledge base that can be used in practice without reverting to “traditional” unfounded methods of the past. This class has helped me by using current knowledge, theory, and skills in my practice that I can use for further diagnosis in the role of the FNP (National Organization of Nurse Practitioner Faculties, 2017). As nurses, we are always learning and advancing in our practice to better advocate for our patients while growing within our roles. Benner’s theory of Novice to Expert helps explain that we are always learning, regardless of how long one has been a nurse, and this journey will only further strengthen us as family nurse practitioners (Fitzpatrick & Gripshover, 2016).
I gained a lot of knowledge from my classmates by tackling different case studies in weeks 2, 3, and 5. Everyone’s opinions and perspectives were respected without criticizing. The project in Week 6 was gratifying, and I got to learn new things from watching the videos and personal experiences.

- Critically analyzes data and evidence for improving advanced nursing practice
Despite the fact that advanced practice nurses (APNs) contributed to the health industry by increasing the access to health care to patients, service efficiency, and mitigating the cost of health services advance practice nurses still face challenges in ways to improve their practice. In order to improve their practice, APNs must be granted autonomy to practice (Yeong woo, 2017). Previously, APNs is limited to practicing in the primary care setting, however, in the past decade, the scope to practice has expanded to acute care, skill health facilities NP/APNs in emergency and critical care setting. This means that APNs now have a wider scope to practice there by taking the responsibility of performing some medical tasks typically performed by physicians
Using the high-intensity model of care as recommended in 2006 by The Accreditation Council for Graduate Medical Education regulations in the United States of America is another way of improving advance nurse practice. The goal of introducing the service model is to have patient access to 24-hour physician coverage (Yeong Woo, 2017). According to model the only way to increase the patient-doctor access to their doctor is to reduce ratio patient ratio, by the inclusions of more APN s in health care.
- Integrates knowledge from the humanities and sciences within the context of nursing sciences
Nursing as a discipline incorporates knowledge from both the sciences and humanities. The use of science provides empirical knowledge and application nursing and associated disciplines such as biology, the physical sciences, medical science, and chemistry (Kalofissudis, 2018). Nurses utilize knowledge from other sciences in understanding a physiological process as well as utilizing behavioral, social or natural sciences in finding abstract for scientific theories (Kalofissusis,2018). For instance, knowledge borrowed from humanities such as philosophy and sociology is useful in dealing with human behavior and knowledge from physics or chemistry is used to gain an understanding of the human physiological process ( Kalofissudis, 2018).
- Translates, research and other forms of knowledge to improve practice processes and outcome
APNs can translate research and other forms of knowledge through research knowledge created, circulate and adopt of such knowledge into clinical practice (Curtis, 2017). Knowledge translation consists of two phases. The initial creation phase consists of finding new knowledge and synthesizing the knowledge into the production of new tools, such as clinical guidelines in response to an identified clinical problem (Curtis, 2017). The action phase involves the use of critical thinking and making an informed decision from the acquired knowledge. comprises the following, identify the problem and relevant research, adapt research to the local context, assess barriers to using the knowledge, select, tailor and implement interventions, monitor knowledge use, evaluate outcomes, and sustain knowledge use ( Curtis,2017). For instance, if the APN want to minimize the rate of infection in their health cares to set. The APN will look for an evidence-based research on infection control methods. The next step is the implementation of the intervention of the chosen intervention, analyzing the effectiveness of the intervention and final adoption of the process to their practice.
- Develops new practice approaches based on the integration of research, theory, and practice knowledge
Research methodology theory and practical knowledge is part of the nursing competencies scientific foundation. Nurses are in the frontline of health care than any other health care profession, as a nurse‐led research is a helpful pathway to practical, effective and cost‐effective (Curtis, 2017). Evidence-based research not only benefit the nurse practice but patients benefit ways of reducing hospital errors, cutting down on unnecessary costs and improving patient outcomes. Better patient’s outcome and the success of the APN practices depend on the ability to incorporate of an evidence-based research theory and knowledge in their practice. APNs incorporates the evidence base in their practice by using intervention techniques that translate research‐based evidence into practice such as visual cues signs in the clinical area, audit, educational seminars, prompts, clinical guidelines, protocol, and leadership involvement (Curtis et al, 2017).
Also, clinicians must strive to unlearn their old way of practice and willing to new adopted evidenced-based approach to care. The only way to adopt change is through the use of any or combination of these models such as the theoretical domains framework or the behavior change wheel to act as a guide in planning the specified intervention and the implementation (Curtis, 2017).

Manage Discussion Entry
I found this class to be quite informative and interactive especially with having peers from different disciplines. My experience as an ER nurse has helped me relate to the many different advances in treating patients with a multitude of diseases. It has been some time since I’ve been in a pathophysiology class, but the interaction and sharing of knowledge from diverse and experienced nurses was very educational. I’ve gained knowledge from a variety of research and educational resources as well. The information provided in this class has been very informative. This course has helped increase my knowledge of pathophysiology, as well as to be more objective as to how I should approach and view disease processes. Human pathophysiology discusses the disease processes at all levels. Case studies were utilized to develop the information learned to evaluate and analyze how the cascade of events unfold and how body systems are affected. It involved the understanding of lab values, signs and symptoms presented, and how to approach treatment when appropriate.
This course has exceeded my expectations. At first, I was apprehensive of the thought of taking advanced pathophysiology; as a seasoned nurse, I knew the mechanisms of physiologic responses of the body in relationship to certain disease processes but the thought of having to dig deeper gave me much anxiety. Looking back, I was overly worried and anxious for no reason. I can say that I have enjoyed this class and learned a lot of great information that will be useful in now and in future practice. This class allowed me to analyze, apply, and evaluate normal and abnormal responses to pathophysiologic mechanisms and conditions by building on knowledge from humanities and sciences.
This course has helped me to build on the foundational knowledge needed as an FNP by helping to integrate evidence-based knowledge with patient care. Identifying and incorporating epidemiologic and pathophysiologic concepts of a patient’s clinical presentation helps to include or exclude certain conditions to give the best evidence-based care to each individual patient (American Association of Colleges of Nursing, 2015).
Finally, this class has prepared me to meet Nurse Practitioner Core Competency #1 by using current knowledge, theory, and skills learned to assess, diagnosis, and treat multiple disease based on clinical presentation (National Organization of Nurse Practitioner Faculties, 2017). The weekly discussions and quizzes have been very beneficial in developing my analytical skills and applying information from evidence-based research and clinical practice guidelines to improve patient care and clinical outcomes. Each of the course objectives and program outcomes have contributed to my progress toward meeting MSN Essential I, Nurse Practitioner Core Competency #1, and to my future role as a competent and caring nurse practitioner.

Dr. Arredondo and fellow classmate,
Over the past seven weeks of the class, I learned several new information that will help in my future career pathway as an APN (Advanced Practice Nurse).
MSN Program Outcome #1
Provide high quality, safe, patient-centered care grounded in holistic health principles. (Holistic health & patient-centered care).
The Master of Nursing program (MSN) provides an organized, sequential plan of study that I believe it helps in the preparation of a specialized nursing role. According to Grove, Burns, & Gray (2014), it is essential to set goals to be able to measure the outcomes of the master of the nursing program, as I reflected on the course, the MSN program prepared me to provide high-quality patient-centered care grounded in holistic health principles. Understanding the detail of the disease progress going to help me to involve the patient care more professionally. Teaching and explaining patient disease progress and condition helps the patient and family members can understand and encouraging families to participate in the care of their loved ones can result in improved patient outcomes. Example: As a Medical/Surgical nurse, I can provide more detailed information about the different type of fractures and the outcome of the healing process. I can differentiate between the types of fractures and what surgical procedure will be necessary to fix it. I can teach and explain to the patients warning signs and symptoms of the various disease process.
MSN Essential I: Background for Practice from Sciences and Humanities:
For seven weeks, I have expanded a better understanding of analyzing pathophysiologic mechanisms associated with different disease conditions. The knowledge that I received from MSN essential 1 helps me to explore the patient situation and understand the compensation mechanism theoretically. As an APN I recognize that family history and genetics has an important role when treating a patient. Genetic testing plays a dynamic role in determining the risk of developing certain diseases as well as screening and sometimes medical treatment. This course thought me more comprehensive information about the hereditary illness like cystic fibrosis through my disease process presentations. I increased my confidence to communicate to the public about the disease process. All the different area of knowledge I learned through the discussions, presentations, and theory, I will be able to integrate learning into practice and provide safe, and high-quality care.
Nurse Practitioner Core Competencies # 1 Scientific Foundation Competencies.
To critically analyzes data and evidence for improving advanced nursing practice. The use of conceptual and theoretical frameworks to organize the educational curriculum of nursing programs is essential to protect and preserve the focus and clarity of nursing’s distinct contribution to health care (Wilson et al., 2015). My Integrates knowledge from the humanities and sciences within the context of nursing science. Education in pathophysiology prepares nurses to recognize disease states and progression. It consequently leads to improved interventions in patient-centered care. Translates research and other forms of knowledge to improve practice processes and outcomes. Training in pathophysiology also prepares APN to be active members of clinical teams. This course made me develop and become successful FNP in the primary care setting to provide ethical, quality, affordable and safe practice to every patient I encounter. We all can achieve our goals by providing the holistic care we learned in the early stages of our nursing careers. The development of new practice approaches based on the integration of research, theory, and practical knowledge. According to Teixeria & Bryne (2015), nursing reflects the comprehensive care that can see through the four of nursing which are client, environment, health, and nursing. As an APN I have a responsibility to ensure patients are receiving the highest quality of care. Pathophysiology help to improve my knowledge in the cell biology and chronic medical conditions, and autoimmune disease processes. The various disease that I learn will help me to recognize the disease process and disease progression going to help me to treat chronic patients optimally, avoiding hospitalization and improving the patient’s quality of life. This course stretches me with the confidence to work more effectively with patients and physicians. I will utilize and integrates my knowledge from the pathophysiology, the biological basis for disease in adults and children to provide the care necessary for each patient I encounter. I will apply my expertise in critical analysis to advance my nursing practice. I will attempt to convey all the knowledge in analyzing the pathophysiological mechanisms associated with the disease process to the career I obtain as an FNP. As an APN, I will ensure that all patients receive the quality care, they need and deserve.

NR 507 WK 7 Reflections
Professor and Class,
Program Outcome #1: Provide high quality, safe, patient-centered care grounded in holistic health principles. (Holistic health & patient-centered care).
In reflection, over the last several weeks the advanced pathophysiology class has allowed the student to build upon the already acquired knowledge related to nursing care that encompasses the person looking at multiple spheres of the individual health disparities, social and environmental determinants for optimal health. In regards to the requirements for understanding the pathology of diseases, their effects and the clinical outcomes recommended. As a result of the course, the student feels confident in her abilities, as a future practitioner to be able to provide a comprehensive education. Which aligns with the intended program outcome referencing high-quality patient-centered care, through encouraging the decision-making process and autonomy regarding health care choices through knowledge integration. That will allow future health care consumers more communication with their providers concerning diagnosis, treatment and self-management actions (Kogan & Wilbur, 2015).
The educational curriculum encountered during the last several weeks prepared the student with a greater understanding of the pathophysiological, pharmacokinetics and pharmacodynamics of multiple disease processes encountered in primary care, more specifically the nurse practitioner. The knowledge ascertained allowed the writer to analyze health care disparities on an advanced level taking a closer look at the disease process such as the role of cellular manipulation, to include the genetic anomalies and how they affect individuals across the lifespan. Which will allow the student to integrate pathophysiology, as well as incorporating various nursing theories such as self-care deficit to help individuals and their families meet the necessities of daily living (American Association of Colleges of Nursing, 2011).
- Develops new practice approaches based on the integration of research, theory, and practical knowledge.
In reflecting over the last several weeks analyzing the integration of learned concepts of the science related to disease processes through advanced pathophysiology. The student will be able to apply various treatment modalities for disease processes taking into account evidence-based practice guidelines for improving health outcomes. Through the application and understanding of the importance of maintaining equilibrium and homeostasis within the human host. Which requires the student to have the ascertained foundational understanding of pathophysiology as well as the various components such as genetics, behavioral, developmental, physiologic and community paradigms. That guide clinical practice, which previous classes in addition to NR 507 has contributed to the student’s confidence and preparation preparing for clinical practice (The National Organization of Nurse Practitioner Faculties [NONPF], 2017).
Thanks,

Stacy,
You correctly identified and addressed the pre-determined program outcomes, MSN essentials, and NP core competencies, but unfortunately, I do not see that you provided two or more examples of how you achieved or had exposure to these outcomes or competencies.
Aug 23, 2018Aug 23 at 11:46pm
You really gave more insight to nurse practitioner core and scientific competencies. I believe that despite the fact that advances nurse practitioners have contributed to the healthcare there is still an area of improvement. Advance particle nurses should be given the autonomy of practice. This means they should be able to diagnose treat their patients with any prescription that they deemed proper to manage the ailment rather than waiting for the physician to approve the certain line of treatment. The long delay between communicating with the physician and the nurse practitioner normally results to the poor outcome which affects the patients’ experience as being unsatisfactory, expensive and in extreme case death.
Advanced practice nurses should be able to transform, knowledge gain from science and other disciplines in the management of patients under their care. This means as an advanced nurse practitioner it is my sole responsibility to perform a thorough physical examination so that the objective finding and the subjective finding will correlate to the particular diseases in question. I must be able to understand the Pathophysiology of the disease in question. Understanding the Pathophysiology will also help to determine the best line of treatment and in staging the disease process, and the ways to contain the diseases to prevent community outbreak in cases the disease is infectious Also, knowledge gained from humanities will be useful in managing the patients other human dimensions such as physical, social, psychological, and spiritual needs using a holistic approach.
The use of evidence-based research is useful in the proper diagnosis of disease and treatment of the disease. Using the right latest diagnostic technique and treatment not only benefit the patient because their needs are addressed in a timely manner, however, the burden of trial and error in the management of the disease is eliminated from the practitioner,
Developing new practice approach will also benefit the patient as well as the clinical practice. Patients will be educated about their disease condition and they will able to make an informed decision about their choice of treatment. The practice will also adopt specific theories and new evidence-based practice to adopt in their clinical practice, thereby preventing facing the challenge of the ethical dilemma. The clinical will come to work every day knowing exactly what they are expected to do and the right way of archiving the outcome

Program Outcome #1: Provide high quality, safe, patient-centered care grounded in holistic health principles. (holistic health & patient-centered care)
The MSN program outcome number one is about providing holistic, quality, and patient-centered care. In this course we achieved this outcome through various assignments and studies we completed. Each week we were assigned readings of various chapters that went along with the learning concepts for the week. We would learn information about the different body systems along with alterations in them and how they were treated. After completion of the readings we would also take an assigned quiz to test our knowledge. Through the readings and quizzes, we learned how to provide safe patient care for a variety of diagnoses. Another assignment that helped me to achieve this outcome was the Week 6 recorded presentation. I was assigned Alzheimer Disease. Through this assignment I studied the disease and learned how to provide patients with the diagnosis quality care through treatment and intervention. Holistic care was involved because I looked at treatment and prevention measures such as exercise, diet, and psychological aspects of care relating to the patient.
The MSN Essential I is about integrating findings from a variety of different fields in science and humanities such as public health, genetics, quality improvement, and nursing. This essential was achieved through discussion postings. In the discussion postings we looked at different topics and diseases in patient care scenarios. We researched each topic and disease and associated possible genetic factors. We also looked at how to improve care of patients with different interventions and teaching topics for each diagnosis. Another assignment we completed to achieve this essential was the week six presentation. During this, I took Alzheimer disease and looked at genetic components and how it related to the disease, and what could be done to improve the quality of treatment for those patients with genetic susceptibility. IT allowed me to take information from different sciences and apply it to a common disease encountered as a nurse practitioner.
The Nurse Practitioner Core Competency #1 involved the analysis of data and evidence in order to improve nursing practice. This was achieved through a variety of assignments such as assigned readings, examinations, discussion postings, and the presentation. The assigned readings helped us learn about diseases we will encounter in our future practice and how we can continually improve to manage these conditions. The discussion postings helped us to find evidence regarding best practice, and also read and converse with other students from different backgrounds of nursing and their experiences and research. Examinations helped us to evaluate our knowledge of concepts and discover what we needed improvement in. The graded presentation also helped to advance our practice in commonly encountered diseases. Through this assignment we also got to share the information we found among peers to learn about a variety of best care practices.
Overall, I enjoyed this class and have learned a wealth of information. I aspire to continually build upon the knowledge I have gained and use it in my future practice.

Hello Doctor Arredondo and Class,
Week 7 Reflection
Program Outcome Achievement
The first program outcome achievement is for students to provide high quality, safe, patient-centered care grounded in holistic health principles. (holistic health & patient-centered care). In the advanced pathophysiology course, students delved into different diseases related to systems and learn about how the body responds to a disease process. It is interesting how disease processes overlap each other. As we started with an open forum discussion in week 1, I had no idea what to expect and reading back on my original comments, I reflected on allergic reactions and remembering how I was going to remember all the information. As the class progressed to week 2, I felt more comfortable as scenarios were presented. The scenarios helped me to focus on the disease states as well as how the disease presentation looked like from a patient’s perspective. For example, the five-month-old Caucasian female with foul-smelling, greasy diarrhea, huge appetite, and wheezing will always stay in my mind as an example of cystic fibrosis. And each lesson from week 2 to week 7 presented different disease processes and safe provisions for quality care. Reflecting on week one, there is so much more to cystic fibrosis as the patient has increased sweating, diarrhea, vomiting, decreased fluid intake to which causes dehydration and the electrolytes to shift causing extracellular dehydration commonly known as metabolic alkalosis (Van Biervliet, de Clercq, Declercq, Van Braeckel, Van Daele, De Baets & De Looze, 2016). This must be scary for patients as they can wind up with a partial or complete intentional obstruction (DIOS). Teaching airway clearance would be a major factor in education to which the techniques would be demonstrated and the parents would return the demonstration. Management of infections and preventative measures would be of equal importance
Create a caring environment for achieving quality health outcomes (Care-focused): Caring goes beyond the textbook. It is amazing how I am applying what I am learning in class with work. However, I am very analytical, and my supervisor told me that I can be that analytical when I become a Nurse Practitioner and it was not necessary now. After thinking and reviewing, I am going to look for another position as my supervisor’s comment alarmed me. If we as nurses and doctors do not analyze, then how are we to be respected in the medical field and more importantly, how will our patient’s respect us. Not enough of attention is given to detail as people are so rushed. I like that commercial that states: “Enjoy the roses”. Furthermore, detail is so critical in assessing patients as diseases overlap and one must know the differences to separate the information. For example, in week 3, we studied about sickle cell anemia: the petrophysical process, the hallmark signs, pathophysiology, as well as providing a teaching related to the patient’s diagnosis. In addition, the special question for the week challenged me and made me realize that there is a pattern with the diseased especially sickle cell anemia in relationship to pulmonary embolism. This is amazing as our assessment must be detailed with assessment criteria in asking questions, providing critical testing for the patient as well as interpreting information.
As I had stated in a previous post: “Folsom, Tang, Roetker, Kshirsagar, Derebail, Lutsey & … Cushman (2015) conducted a study which was trying to connect those with the sickle cell trait in lieu of deep vein thrombosis and pulmonary embolism and added that those with the sickle cell trait have and increased risk for pulmonary embolism at 40%. In sickle cell their thrombin is produced in excess and Folsom, Tang, Roetker, Kshirsagar, Derebail, Lutsey & … Cushman (2015) added something that caught my attention which is “sickle trait red cells may be unusually prothrombotic due to abnormal membrane phosphatidylserine exposure and may be incorporated in a venous ‘red clot,’ altering clot structure”. Also, the D-dimer marker for thrombin generation but also linked to hyperfibrinolysis promotes embolisms and in addition the hypoxia that is induces from sickling which is located in the pulmonary vasculature can lead to in situ pulmonary thrombi which in turn also is part of the pathophysiology of acute chest syndrome and hypertension of the pulmonary system”.
Engage in lifelong personal and professional growth through reflective practice and appreciation of cultural diversity (cultural humility)
This objective is ongoing and through the Advanced Pathophysiology course, every week we delved into new information from evidence-based resources that were peer reviewed. This is critical in that current information is integral as new technologies and advancements help to create better outcomes for our patients and promote the quality of life. Cultural diversity means respect with humility engaged to every patient and realizing that the care must be individualized. As I had taught my students, everyone brings into their health care different religious beliefs and practices, different types of food, music, and family environment. For example, I am Polish, and we have wonderful food that I really enjoy but I have to be careful because an overabundance of Kielbasa could bring increased cholesterol and heart problem including atherosclerosis and arteriosclerosis. I know not to indulge but I have seen individuals partake in a hefty artery slamming consumption of meals.
Integrate professional values through scholarship and service in health care. (Professional identity): My professional values consist of high standards and I make it a point to go above and beyond with my patients and their families. I am not in this profession for a paycheck. I am honored to serve my patients, but I also point out to my patients that honesty is important and if I feel that they have unrealistic goals, that I will point them diplomatically in the right direction. I think that service also deals with staying current with evidenced based research and that it is important for practitioners to keep up with the current information. This way, the patient can make an informed decision to his or her health care. I want to take another advanced pathophysiology course as I feel 8 weeks is just not enough time to get the intricate details. If there are any other resources that you could point me int the direction, I would appreciate that and will be working to higher my standards and knowledge.
Advocates for positive health outcomes through compassionate, evidence-based, collaborative advanced nursing practice. (Extraordinary nursing)
Again, compassion means listening to the patient and family holistically and really discovering what they have to say. I keep my mouth shut unless it is asking questions. When my patient overdosed on OxyIR last week, I was doing more watching her non-verbal communication which helped me to assess her and stay at the house a bit longer. Normally my visits are no more than one hour but I was seeing a progression of symptomology appear right before my eyes which I am very happy that I spent a half hour more to keep an eye on her. Suddenly, she stopped breathing, no pulse, and no blood pressure. I told the husband to call 911 as I worked on her. Narcan was administer and the patient was revived. The husband was floored. I was admitting this patient to homecare. I am very grateful for this course because I remembered our chapters on addiction and overdose. I knew what was happening. It is still unbelievable, and this also happened a second time with the same patient. For homecare, one would not expect to see this but because of the opioid epidemic, it is happening more frequently. This is something that we as Practitioners will be facing and that is why I chose to oversee this areas Pocono Region Kacies Cause. I would encourage everyone to go to www.KaciesCause.com (Links to an external site.) and become a chapter member in your area. We can do something about this. Also, I was present with Search and Rescue in the Albrightsville area for the 25-year-old man named Jeremy Cosmi who was on the news. He did not make it. We must become active in stopping addiction and must teach people about the signs and symptoms and do much more.
The reflection specifically identifies and addresses the pre-determined Program Outcome* and provides two or more examples of how the student specifically achieved or had exposure to this outcome during this course/clinical.
MSN Essential Competence: As mentioned above, I gave two examples and these past weeks have been very trying for me as I have been in touch with Jeremy’s mother as the investigation is proceeding. She has multiple sclerosis and is in relapse from all the stress. I ask that all of you pray for her. I have had new stations ask questions about the search but have told them to contact State Police as the investigation continues. Opioids and addiction are very much a part of this case. This course has taught me the importance of education in a society that has been crippled. In the clinical setting, we must be advocates for our patients but must also promote evidence-based research outside of the office of work to the community as this is an extension and then globally.
The objectives for the course were:
Analyze pathophysiologic mechanisms associated with selected disease states. (PO 1) 1, 2, 3, 4, 5, 6, 7, 8
My examples were listed above with sickle cell anemia, cystic fibrosis, and pulmonary embolism; Examine the way in which homeostatic, adaptive, and compensatory physiological mechanisms can be supported and/or altered through specific therapeutic interventions. (PO 1) 1, 2, 3, 4, 5, 6, 7, 8; Distinguish risk factors associated with selected disease states. (PO 1) 1, 2, 3, 4, 5, 6, 7, 8; Integrate advanced pathophysiological concepts in the diagnosis and treatment of health problems in selected populations. (PO 1) 1, 2, 3, 4, 5, 6, 7; and Relate research findings to the management of patients with complex pathophysiologic dysfunction. (PO 1) 1, 2, 3, 4, 5, 6, 7, 8

NR 507 Course Reflection
Hello Dr. Arredondo and class, the past seven weeks had been an intriguing and empowering time for this writer. NR 507 was developed based on the Master of Science in Nursing (MSN) program outcomes number one (#1) in alignment with both the MSN Essentials and the Nurse Practitioner Core Competencies #1(NPCC).
MSN Program Outcomes #1: From the NR 507 course, based on the MSN program outcome #1, this writer extensively explored the protective body processes; including how the body maintains homeostasis within and outside the cells; the bodies defense mechanisms and factors that could alter them causing disorders in various systems of the body across the lifespan. This author is confident of a solid understanding of different etiological and genetic factors in the development of various disorders and their pathophysiological processes. Through the Chamberlain College of Nursing (CCN) provided course lessons; timely facilitation from the faculty; extensive studying of the recommended textbook; course assignments including academic interaction with peers about assigned systemic patient disorders; weekly quizzes and term examinations which provided positive review and reinforcement of what the writer had learned from the course. She was equipped and prepared to provide high quality, safe, holistic patient-centered care in her current area of practice and her future advanced clinical nursing practice (CCN, 2018).
Understanding the human body and what is going on within empowered this caregiver to focus her assessments with knowledge, diagnose patients’ condition correctly in a timely manner using a cost-effective, and harmless approach. NR 507 had also prepared the author to provide personalized patient and family education according to the patients’ needs, culture, economic and psychosocial orientation with respect thereby promoting their understanding of the patient’s condition allowing them to respond positively for better health outcomes. The knowledge of various diseases’ pathophysiologic processes had driven this writer to critically think and understood that sometimes “what is good for the goose might not be good for the gander” (n.d.) and one size does not always fit all when it comes to patient care. Meeting the patients at their levels and helping them to understand and make evidence-based care choices while participating actively in their own care was a decision that had been made by this writer based on the knowledge she gained from the course. For example; Two patients with asthma may require entirely different management formats based on their individual types and causes of the disorder either intrinsic or extrinsic (McCance, Huether, Brashers, and Rote, 2013).
The Master of Science in Nursing Essentials #1: The NR 507 course outcomes enabled this writer to meet the MSN Essentials #1 by gaining additional knowledge to what she had previously attained from her Bachelor of Nursing Education. She now has an in-depth knowledge of the sciences of the human body and how history, culture, environments, genetics, and behaviors could affect the functioning of the body.
In addition to course lessons and published textbooks, she acquired a solid knowledge background from an ongoing exposure to peer-reviewed literature in all areas of her coursework throughout the session. Moreover, the electronic presentation project on a disease process and a review of her peers’ presentation was a big part of knowledge enhancement for this writer. While preparing the project, she was able to search and apply various research results in the exposition of different disease processes, etiology, diagnosis and current evidence-based management across the lifespan.
Acquisition of this advanced scientific knowledge and background equipped the author with the tools to deliver quality, time appropriate, efficient, and advanced nursing management to her patients across the lifespan. She was prepared ready to utilize her knowledge base in recognizing current evidence-based genetic findings and provide clinically relevant and culturally competent care to patients’ population at various levels either individually, families, or communities upholding patients’ values as appropriate (AACN, 2011).
As a baccalaureate nurse with years of experience, this writer realized during this class the advanced scientific basis and rationale behind some interventions she had been performing and had assisted physicians to perform or transcribed. She recorded a lot of “Oh! I see” moments which were like turning on of light bulbs and the knowledge was really confidence solidifying for her.
Having gained advanced knowledge and background in the area of pathophysiology process of various human diseases the writer is ready to utilize a high level of critical thinking coupled with her scientific background in optimizing patient satisfaction, accurate diagnosis, and management especially when faced with undefined clinical manifestations; she could efficiently utilize her knowledge about etiology, genetics, environment in relation to diseases and their pathogenesis in performing differential diagnosis and mapping out individualized management for the patients in collaboration with other healthcare teams as necessary (AACN,2011).
For example, the writer currently takes care of patients with chronic diseases such as; Diabetes, Asthma; Heart disease; Chronic kidney diseases, and many more. Having an in-depth understanding of the pathogenesis of sepsis and its effect on all organs really motivated the writer to be more passionate about infection control and patients teaching. Also, having a clear knowledge of how smoking causes lung cancer encouraged her to be more proactive about patients and community education concerning smoking cessation
Nurse Practitioner Core Competencies #1: This author acquired an important skill which is one of the core competencies and a part of the essential behaviors of all Nurse practitioners ((The National Organization of Nurse Practitioner Faculties [NONPF, 2012]).
NR 507 allowed her to acquire the necessary scientific knowledge base and the ability to be an independent, advanced clinical nurse practitioner as recommended by the nurse practitioner core competencies #1. She acquired the ability to recognize and translate knowledge into practice and be able to function efficiently in the current rapidly changing healthcare environment (NONPF, 2012). The mandatory course requirement for students to utilize current peer-reviewed articles and research studies in all class assignments and group discussions allowed her to master how to search, analyze, and extract evidence-based research findings as applied to patient’s condition management. At her current area of practice as a nurse case manager, she was able to utilize the knowledge from this class in facilitating a change at her current workplace when she presented current proven pieces of evidence for better health outcomes in diabetes patients when diabetes education is patient-centered.
In conclusion, meeting the NR 507 Advanced Pathophysiology course outcomes adequately equipped and empowered this writer to meet the MSN program outcome #1, MSN Essential #1, and the Nurse practitioner core competencies #1 and afforded her the tools she needed for being an efficient advanced clinical nurse practitioner.

Dr. Arredondo & Class;
This past 8 weeks has been so informative; the level of learning has been much more than I imagined. Reflecting back over this course we have gained well rounded knowledge in terms of holistic care, how to integrate research into our care practice, and how to develop new ways to improve practice. Questions and responses for this week’s discussion assignment are as follows:
Program Outcome #1: Provide high quality, safe, patient-centered care grounded in holistic health principles. (holistic health & patient-centered care)
As an advanced practice nurse, it is important to have a well-rounded master’s level educational background as we are in a position to provide a higher level of care which includes diagnosis, treatment, and prevention. This class has been crucial as it has been very thorough in discussing the anatomy of the body all the way down to the cellular level, the different organ systems and their functioning, how disease or dysfunction within the body manifests itself, the pathophysiology behind it, and treatments available, etc. In terms of holistic health and centering care around our patients, it is important that we understand that all of a patient’s needs should be addressed including physical, emotional, and even spiritual. While novice nurses may be more task oriented, an advanced practice nurse will look at the patient’s entire story in order to find ways to help improve their health and day-to-day living.
I learned this through the disease process project I was assigned namely Schizophrenia. The etiology of this disease is hard to pinpoint but a large determining factor is environmental stress in many forms experienced by children and teens (Chen, Selvendra, Stewart, & Castle, 2018). Knowing this along with all of the information I learned regarding Schizophrenia for example, I am better prepared to be cognizant of symptoms and provide accurate diagnoses and appropriate treatment. With a deeper focus on the patients, advanced practice nurses will seek out ways to improve a patient’s current care plan such as medication modification and further inter-professional collaboration (Hurlock-Chorostecki et al., 2015).
Much of our class has revolved around gaining a full understanding of how our bodies work, the genetic makeup, and care involved revolving around different health complications. Being an advanced practice nurse involves a strong educational background, strong clinical judgment and essential critical thinking skills in order to develop an evidence-based care plan for patients. Advanced nurses also strive to perform quality clinical practice, facilitate change, discern and incorporate evidence based practices, and facilitate communication across the entire healthcare team (Mannix, Wilkes, & Jackson, 2013).
More so than any class I’ve had before, this class has required a great deal of evidence-based research. This has really strengthened by skills going forward as a Family Nurse Practitioner as I will be able to use current research materials to ensure that the best plan of care is delivered to my patients. A FNP, through a master’s level education, will build on the skills and knowledge obtained through a baccalaureate nursing program by gaining a deeper level of understanding of nursing and nursing practices and be able to fully analyze, design, implement, and evaluate nursing care. One example on the type of learning I experienced in this class was on the topic of DKA. It was surprising to me to find that for a condition that is so dangerous it is largely preventable; in fact up to 75% of DKA occurrences are due to insulin therapy non-compliance and poor management of the illness (Crossen, Wilson, Saynina, & Sanders, 2016). What was obvious after this segment in particular was the strong need for patient education. For DKA, occurrences often occur after illness or traumas or if the patient has a poor economic status. Knowing this and everything we learned involving this condition will improve my practice.
The scholarly discussions that we have done throughout this class have been very crucial to help build our critical thinking skills; this type of education is very important and reliable because it is based on information from credible sources and professional experiences to support the topic. As people gain more qualitative information, they then develop stronger critical thinking skills. A person who achieves better thinking skills is then inspired to “research the existing body of topical knowledge and respond by repeating the theories of experts” (Jones, 2013). Instead of offering up one’s own personal beliefs, students are delivering material that has been researched by scholarly authors in that field and therefore much more credible. As a FNP, providing care and implementing changes should be based on evidence-based research from many different health related fields such as humanities, biopsychosocial fields, genetics, public health, quality improvement, health economics, etc. there by upholding the standards that are expected across the healthcare continuum in our country. Maintaining quality standards will mean not only giving the patient the best possible care but striving for the best possible outcomes as well.
Nurse Practitioner Core Competencies # 1 Scientific Foundation Competencies
-
Critically analyzes data and evidence for improving advanced nursing practice.
One of the main goals of becoming a master’s level nurse is to become more knowledgeable and proficient across the entire healthcare continuum (Abraham, Gohan, & Pfrimmer, 2015). FNP’s will gain proficient masters level skills to be able to discern cues and to make decisions with more advanced critical thinking skills than what a novice nurse may have (Mannix, Wilkes, & Jackson, 2013). This has become an important tool for me especially when it comes to analyzing data and particularly diagnostic results. I decided to be tested for BRCA having gained a better understanding of the genetics involved as well as the family risks (I had an aunt die from breast cancer). I learned that my risk was actually lower than I had thought it would be after having an additional test performed. In my schizophrenia project, I learned the importance of providing genetic counseling and how further education about ways to avoid the disease’s development can be paramount.
-
Integrates knowledge from the humanities and sciences within the context of nursing science.
As the healthcare industry becomes more and more demanding with the escalation in acutely and chronically ill patients and the predicted shortage of physicians as healthcare providers, the role of Nurse Practitioner (NP) is going to become increasingly paramount. This role becomes clearly evident in how NP’s are able to demonstrate the skills and knowledge they have acquired through their advanced education and training and provide quality care that is essentially effective and cost saving (Poronsky, 2013). As nursing experts, FNP’s can perform quality clinical practice, facilitate change, discern and incorporate evidence based practices, and facilitate communication across the entire healthcare team (Mannix, Wilkes, & Jackson, 2013).
-
Translates research and other forms of knowledge to improve practice processes and outcomes.
One of the main goals of becoming a master’s prepared nurse is to become more knowledgeable and proficient across the entire healthcare continuum (Abraham, Gohan, & Pfrimmer, 2015). It is critical that we are able to look at research data and statistical evidence, decipher legitimate recommendations for change, and implement these into our nursing practice. Translating research and expert knowledge is critical when it comes to patient care especially in situations that are life-threatening. This is evident in our lessons on how to treat DKA and how it involves a careful protocol of fluid replacement, balance of insulin and other electrolytes such as potassium, and the diagnostics involved to ensure treatment is working.
-
Develops new practice approaches based on the integration of research, theory, and practice knowledge.
Being an advanced clinical practice nurse requires many different attributes such as caring, compassion, leadership, and intuitiveness. Nurses, with the main goal of helping people, already have an innate sense of caring and compassion in their heart. Leadership and intuitiveness comes with training and experiences lived through the journey of each patient. Nursing theories are crucial as they can deepen our knowledge and fine tune our skills, even the “human” ones such as caring and compassion, through expanding our mind to think about more abstract concepts and internalize the information in different way with a different mindset.
Through a better understanding of the etiology behind some of the diseases, the extent of the symptoms they feel, and realizing the prognosis they are facing helps us to be very “real” with our patients. For example, I gained a deeper knowledge in this class regarding sickle cell disease. While I had a general knowledge, I really didn’t have a good understanding of the pain that these patients feel or the extent of it. I just recently had a sickle cell patient for a procedure and understanding that sickle cell involves a lack of oxygen to tissues and organs from slowing or impeding of blood flow from rigid, sticky, misshaped blood cells causes pain and often patients are cold was very valuable and helped to make me more empathetic towards her and ensure she was provided with plenty of warm blankets and pain medication as appropriate (McCance & Huether, 2014).
