Purpose

The concepts of epidemiology provide the framework for the study of infectious and chronic health issues/diseases, which provides a rich source of data for the analysis of trends in disease and health. This assignment will offer the opportunity to explore the population health effects associated with sleep health. Epidemiology defines the distribution and burden of a disease among a population. Epidemiological studies help identify the populations at risk for given health, so appropriate interventions can be developed to address it. The targeted population for this study is American Indian and Alaska Native elders. The population lives in rural and experiences significant health disparities. Also, poor economic conditions are a significant concern among the populations with a poverty index of 18.8%. The majority of the elders are aged above 65 years (2017 Profile of American Indians and Alaska Natives Age 65 and Over, n.d.). The current population is estimated at 258,616, with the numbers likely to increase in the future. Most of the population is not educated past the high school level. 48% of the population are married in terms of marital status, while 27% are divorced (2017 Profile of American Indians and Alaska Natives Age 65 and Over, n.d.). The current study explores the oral health practices among the indigenous populations. Poor oral health could lead to diseases such as gum diseases, cavities, sensitive teeth, and oral cancer, among others.

Overview of the Problem in the State

The oral health issue is among the significant health burdens in Alaska. The prevalence of oral health issues among children in the state is almost five times the national statistics. The survey conducted by the Indian Health Service indicated that about 50% of the adult populations in the region had untreated tooth decay. The prevalence of dental diseases among the elderly population is also high. Most of the oral health issues are self-limiting and so do not lead to mortality. The high rates of oral health problems among the populations in the state are attributed to their health beliefs. According to Tynan et al. (2020), indigenous people suffer significant oral health issues compared to the other population groups. Similarly, the study indicates that poor oral health among the population is complicated by the profound social concern provenances, including poor economic status and low social capital. The current study explores the burden of oral health issues among the American Indian population and the possible interventions to address the issue.

Descriptive epidemiological and demographic data on mortality/morbidity and risk

The prevalence of untreated oral health issues among the populations globally is estimated at 35%, with more than 50% of the adult American populations having periodontitis Adolph et al. (2017). Similarly, Adolph et al. (2017) indicated that oral care is linked to mortalities among individuals with tooth loss. Most of the moralities are associated with oral cancer. The study estimated a mortality rate of 0.5%, which is low. However, there is a need to respond to the oral health issues urgently because of the high morbidity rates among the U.S.

HP2020

The goal of the HP2020 is to prevent oral and craniofacial diseases and ensure that the affected individuals have access to preventive and dental care. Therefore, in addressing the health issue, there is a need to create awareness of the importance of oral health, increase the acceptance and adoption of preventive measures, and reduce the disparities in access to healthcare services. Healthy People 2020 aims at eliminating oral health issues among both adults and children. Similarly, there is a need to introduce a monitoring and surveillance system to identify and respond to various oral health issues, including the cleft palate.

Population-Based Health Promotion

According to Niranjan et al. (2017), oral health promotion uplifts community health. On the other hand, a majority of the people in the indigenous communities are unaware of the best oral health practices. Therefore, the education will focus on helping the community understand the importance of oral health and the risks associated with its failure. The key areas to be addressed in oral health promotion will include the need for healthy eating, oral hygiene methods, access to preventive oral health services, and promotion of the topical fluoride application. The education will be administered through direct interaction with the targeted populations. Also, printed posters with adequate information on oral health will be made available to the population. The approach will help ensure that the targeted population is constantly aware of the need to change their behavioral practices and reduce the risks of oral health issues.

Conclusion

Finally, oral health is one of the neglected sectors in health delivery because of the low mortality rates. However, the findings in this study indicate that the burden and risk for morbidity are significantly high. Health promotion measures should be developed, especially those targeting indigenous populations, to reduce the risk for oral health issues. The burden of oral health is high among the indigenous populations in Alaska because of the beliefs and perceptions on oral health. The health promotion should target creating awareness on the preventive measures and the availability of the health facilities offering dental care.
