A community health assessment is conducted while driving around (or walking, if it is safe) your community and provides a visual overview that allows you to assess conditions and trends that could affect the health of the population; this data then provides you with the background and context needed to work with individuals and families in their community. For this community health assessment, you can also look at community dashboards and/or websites to gather statistical or geographical information that is relevant to the community.
