Advance registered nurses role was initially necessitated by the need to fill the gap in primary care providers for the pediatric population. The shortage of the primary care physicians continued to drive the demand for the advanced nurse practitioners while also emphasizing on need for them to practice at their full potential (DeNisco, S.M & Barker, A.M, 2024).Initially, this practice was limited to clinical roles such as clinical nurse specialist, nurse practitioner (NP), certified nurse–midwife (CNM), and certified registered nurse anesthetist (CRNA) (DeNisco, S.M & Barker, A.M, 2024). Over the years, we have witnessed the expansion of this roles to include nurse administrators, public health nurses as well as policy makers which has continuously been made possible by redefining and streamlining the curriculum and competencies requirements to best suit the needs of the advance practice nursing (DeNisco, S.M & Barker, A.M, 2024).
After the Nursing Now campaign was launched in 2018, to scale up the universal healthcare coverage(UHC) , the Pan American Health Organization (PAHO) and the World Health Organization (WHO), continued to lobby the governments to implement the advance practice nurses training since nurses had demonstrated great intellectual capabilities in their areas of specialization (Lopes-Junior, 2021). This was a large boost to the nursing profession as it sought to highlight the potential of nurses with an aim of lobbying for them to operate at their full potential and not just confined to the traditional advance practice nursing roles (The Lancet, 2020). More over, this campaign culminated into the recognition of the nurses with the year 2020 being christened “the year of the nurse” (The Lancet, 2020). It is gains as this ones that ultimately led to the World Health Organization publishing the first ever “state of the world’s Nursing Report” and subsequently “the state of the world’s midwifery report” (The Lancet, 2020). This places advance practice nurses in the limelight while highlighting their contributions to the UHC.

The role of advanced registered nurse first grew out of a necessity to provide access for populations in communities lacking access to primary and preventative health during the 1960’s. However, it quickly grew during the 1970’s to begin educating nurses to diagnose, treat and refer patients for chronic and common illnesses (DeNisco, 2021).
As advanced practice nursing education expanded, so too did variability in scope and authority at the state level despite national standards and a board certification exam. The American Association of Nurse Practitioners distinguishes three separate working domains for advanced registered nurse practitioners which includes full, reduced and restricted authority (American Association of Nurse Practitioners, 2023). States with full practice authority allow practitioners to function at the greatest potential within their scope of practice including but not limited to assessing, diagnosing, treating and prescribing (Kleinpell, et al., 2023).
At the height of the CoVid-19 pandemic, several states across the US enacted temporary emergency waiver of restrictions in authority for advanced practice registered nurses, allowing them to provide care in a crisis. For example, Arkansas now permits these providers to perform duties as primary care providers, and be reimbursed by Medicaid (Kleinpell, et al., 2023). Additionally, the advantages for compact licensure among advanced practice registered nurses has become a topic of discussion. Multistate licensure may allow for these providers to respond more quickly during emergencies and provide telehealth opportunities (Kleinpell, et al., 2023). As the American health care system evolves, so too does the need to serve the population in dynamic ways allowing advanced practice registered nurses the opportunity to expand their role.
