Title: DNP-815A Case Report Application of Theory to Organizational Mission, Vision, and the Christian Worldview

The proposed project focuses on the reduction of patient falls, focusing specifically on post-operative patients in a post-operative rehabilitation center. Traditionally, health professionals rely solely on clinical fall prevention strategies; thinking of ways of optimizing patient outcomes in this respect, it has been determined that more than traditional strategies is needed. The intention is therefore to determine whether or not combining specific patient-centered interventions with clinical fall prevention strategies succeeds in achieving significant reductions in patient falls over a two-month period.

Purpose Of Project

The purpose of this quantitative, quasi-experimental quality improvement (QI) project is to determine if the implementation of patient-centered fall interventions in rehabilitation centers for post-operative patients would impact the number of fall incidents among post-operative patients in Rehabilitation Centers in District of Columbia over four weeks.

Everyone has a worldview. Whether or not we realize it, we all have certain presuppositions and biases that affect the way we view all of life and reality. A worldview is like a set of lenses which taint our vision or alter the way we perceive the world around us. The Christian worldview on the other hand makes us believe that God exists (Heb. 11:6) and that He is the standard by which we measure everything else. And to crown it all it is GCU’s believe on the message of salvation as an “indictment against all forms of alienation oppression and discrimination, denouncing evil and injustice wherever it exists”

Hence for GCU, it is their mission that through academic excellence, the university equips students with knowledge of the Christian worldview, instilling a sense of purpose and vocational calling that enables students to be innovative thinkers, effective communicators, global contributors and transformative leaders who change their communities by placing the interests of others before their own.

 Organization’s Mission and Vision to Theory and Model

Sibley hospital vision is to be a role model for innovation in healthcare and wellness for all. Mission is on delivering excellence and compassionate care, every person, every time. The mission of Sibley Memorial Hospital is to provide quality health services and facilities for the community, to promote wellness, to relieve suffering, and to restore health as swiftly, safely, and humanely as it can be done, consistent with the best service we can give at the highest value for all concerned (Sibley ,2021). The proposed theory is the patient centered theory and the proposed model is Lewin’s change model.  Lewin’s change model supposes a change of the fall prevention paradigm. Through this change model, the idea is to move away from solely implementing fall prevention strategies, and moving towards the combination of said strategies with patient-centered interventions.

Connection Of Mission and Vision to Nursing Theory.

Patient-centered nursing theory proposes moving away from the actual disease or condition, and instead moving towards the patient as an individual or human being. This is valuable to the extent that it leads health professionals towards seeing patients, not as statistics or cases to be solved, but rather as human beings in need of comprehensive, personalized care. This humanizes the profession of nursing, and it also facilitates the establishment of rapport, trust and collaboration between nurses and patients. The mission and vision of my proposed site is aligned with the nursing theory chosen, this site relies and practices on delivering a compassionate , excellence patient centered care to each individual that walks through their door “ every patient ,every time(Sibley ,2021).

Connection Of Vision and Mission to Evidence-Based Change Model

The proposed change model is Lewin’s change model, which offers a simplified approach to change implementation. This model proposes three fundamental strategies, i.e. unfreezing, movement and refreezing. This is considered valuable for successful implementation    This model, as previously mentioned, contemplates three distinct stages. The first is refreezing, which supposes disruption. It supposes creating awareness about the need for change, thereby promoting support for said change. The second stage is movement, which means deciding on how to proceed with actual implementation of the change intervention. The third and final stage, i.e. refreezing, implies assimilating and normalizing the change intervention as standard practice (Teguh et al., 2019).

The vision and mission of my DPI proposed site can be intertwined by their proposals that centers on change in an individual. They create a support and promote support through implementation of proposed change that is innovative, individualized and patientive centered.

Pulling It Together

The patient-centered nursing theory is based on human needs theory; it hinges on the nursing professional’s ability to identify the patient’s problem(s) and subsequently develop a specific typology of treatment for that patient. The process to be followed in the application of this nursing theory integrates a sequence of steps. It all starts with the nurse knowing the patient, and then sorting out the relevant, significant information (Ortiz, 2018). The process also calls for the patient and their family need to be included in treatment and the nurse needs to engage in introspection to determine their own feelings relative to the patient prior to discussing a comprehensive care plan (Eklund et al., 2019). Therefore, applying the Christian world view to this model is going to be successful as both emphasis on the human or individual been at the forefront of care. a patient centered care entails putting a patient above self, compassion care every person every time. Therefore, the proposed change model fits in with the proposed clinical question, the mission and vision of the proposed site, GCU mission and the Christian world view. The combination of these theories and views and mission statements will indeed change the direction of patient care and promote improved positive patient outcomes.
