The medical field consists of both nursing practice problems and medical practice problems. Nursing practice problems are any issues encountered during nursing practice related to the patient’s response to a health problem they are experiencing. This information can be identified during a thorough assessment. Medical practice problems are concerned with a medical condition or disease, the cause and treatment for the condition. An example of a nursing practice problem is a patient presents to the hospital with a headache and dizziness. This patient will be at a risk for falls. In health care we use evidence-based practices to provide the best outcomes for our patients. We ask questions during an assessment to get to know our patients. Foreground questions can be used in clinical research prior to starting the research, which is known as PICO (Finding the evidence, n.d.). PICO helps to format research in order to make the research process more manageable (Finding the evidence, n.d.).

Deidra, you gave great examples and explained both nursing and medical practice problems very well. I hope that this class will help us gain more insight into research and as we know it will give great access to validated research to dive into. I feel that both medical and nursing practice relay greatly on research to better our patient’s prognosis.

While our focus should be on nursing practice problems and how to use PICOT to research how to solve the problem, we should also be up to date on medical practice problem research. Especially to how it pertains to our specialty. For example, I work on an orthopedic unit and I need to have a good understanding of the surgeries and procedures that are done for hip fractures so that I can apply appropriate intervention of care. When a patient has hip surgery there are precautions that need to be taken to prevent injury. One of the nursing diagnoses we use is hip precautions, and by understanding the medical intervention we can apply appropriate interventions and prevent injury. Part of the nursing practice is education and to educate patients we ourselves need to be confident in our understanding of the medical practice.

The PICOT question format is a consistent mnemonic for developing answerable and researchable questions. When a good PICO question is written or identified, it makes the rest of the process of finding and evaluating evidence more straightforward. Clinicians who are interested in research pursuits that is related to patient care, must consider the use of a literature search and the PICOT format when engaging clinical researchers. This approach will provide clinicians and researchers an initial basis for mutual understanding, communication and direction to help answer clinical study questions of most relevance.

Hello Deidra,

Great post!! You make a lot of great points about medical practice problems vs nursing practice problems. I am actually in a nursing residency program that has us doing an evidenced based practice project and reading the information regarding PICOT questions has been very helpful. One of the things that I think has helped me understand what it means to make a PICOT questions with EBP projects in mind is that the nurse researcher must basically have an idea of where they think the research will take them and why the project is important to our patients (Helbig, 2018). I think that helped me with clinical application of PICOTs because with all the evidence out there it is important to understand what we are trying to accomplish for our patients.

The nursing process includes continuous input from patients, their families, or communities through all phases from assessment to evaluation (Pope et al., 1995). Nurses use evidence based practice to provide patients with safe, quality care and to improve outcomes. When developing evidence based practice, one is looking at research already conducted. PICOT stands for patients, intervention, comparison, outcome, and time (Helbig, 2018). Questions asked during an assessment helps us get to know our patients.

Nursing practice problems is usually identified doing nursing assessments, it what’s going on with the patients and the response of the patient, like what if the patient states, that something is wrong or the patient is complaining that they are having pain and chills and feels warm, and after the nurse assessment it found that the patient has a headache and temp/fever, the nurse does nurse interventions to help patient with symptoms. (Milner 2017) Other nursing problems can be related to nursing shortages, stress on the job, on-the-job bullying, or things going on in the nurse’s personal life. ( Majd T. Mrayyan)

Medical practice problems focus on what is the physical pathology going on with the patient. For example, what is causing the patient to have a headache to come up with a diagnosis medical problems are concerned with the condition for treatment can be prescribed. Your picot should be based on nursing practice problems because it’s based on your evidence-based practice it is used to solve the problem with the patient it shows that you are using safe quality care and you can be trusted and are trying to improve patient outcome is your goal. i can talk of evidenced-Based Practice (EBP) is utilized by the nurse/researcher to provide patients with safe, quality care and to improve outcomes. EBP is research already conducted. The nurse must first develop a PICOT statement or question. The question can be associated with either a quality improvement project or lead to EBP. Information must be gathered during a nursing assessment; this will provide the nurse with an idea as to why the study should be performed. The nurse/researcher must answer the PICOT question with the data that was collected during the nursing assessment (Helbig, 2018).

Providing effective nursing care through evidence-based practice provides a good foundation for producing positive patient outcomes. According to Gaines (2021) proper nursing diagnoses provide evidence-based practice to determine interventions and provide greater patient safety, quality care, and produce better patient outcomes.  The nursing diagnosis plays a vital role in defining patient problems and guiding the plan of care. This week has provided a unique perspective on the correlation between research, evidence practice, quality care, and positive patient outcomes. Those factors are important to the nursing process and creating a better health care system for patients.

That is, the PICOT question is used as a search strategy that leads to impartial and effective search. The impartial and effective search results in evidence. Consequently, the evidence answers the clinical question and support evidence-based practice (Gallagher Ford & Melnyk, 2019). The utilization of appropriately developed PICOT question when performing literature search leads identification of a small number of applicable studies to answer the clinical question at hand (Bell & O’Donovan, 2021). The PICOT question is the initial step of enhancing evidence-based practice with the objective of addressing patients’ problems.
