I have been a nurse for over 20 years providing direct-patient care. Although my field of study is not considered to be advanced practice nursing, I feel that I will still be just as impact-ful within the healthcare industry and requires an advanced level of knowledge. My focus is on Nursing Informatics. Nursing Informatics is defined as a combination of nursing science, information science, and computer science by the American Nursing Informatics Association (Nation, J. & Wangia-Anderson, V., 2019). The role of a Nurse Information Specialist is mostly behind the scenes but it is very important to the delivery of efficient and effective patient care. Nursing Informatics Specialists serve as a liaison between nursing, patient care services, Clinical Engineering, and Information Services to ensure multi- and inter-disciplinary project coordination to improve the flow and quality of patient care. As with any other nursing roles, it still requires the knowledge of the nursing process and nursing practice, relevant regulatory mandates, promoting interoperability, and principles of quality/safety improvement. The nursing informatics projects are planned and implemented using evidence-based practices for planning, understanding, guiding, and delivering responsible nursing care.

The role of advanced registered nurse (ARN) has progressed over the past years. In 1960s for example the role of Nurse Practitioners (NP) emerged to help in primary care due to shortage of physicians at that time. The Nurse Practitioners provided primary care to the less fortunate treating minor illnesses and emergencies that didn’t require critical attention and referral.

In 20th and 21st century this has dramatically evolved and shifted to where now the Nurse Practitioners has become more independent in providing not only primary care but are more involved in advanced specialized care, they now have ability to prescribe, diagnose, provide preventive care services to the public and also  managed patient of all kind with none or minimal physician supervision (ANA, 2022). It is interesting to know that Nurse Practitioners  were listed by US News world report (2021) as second top occupation.

Today more Advanced Registered Nurses s in different specialties have emerged as well, such as Nurse educators, leaders, anesthetists and so on. This comes with more responsibilities and dependency which allow them to better assess, diagnose, plan, implement and evaluate care of patients in a timely manner. Technology has also paved way to  increase opportunities in the hospitals and institutions nowadays, making it easier to train more ARNs, it has also made healthcare systems manageable to both the patient and healthcare workers.

According to ANA, 2024, it can be assumed there will be  more trained and practicing Advanced Registered Nurses , projecting that that the number  of ARNs will increase to 52% compared to 10% for physicians by the year 2030.

If I were to rate myself as a nurse in general, I would give myself a proficient rating. I have specialized in emergency medicine and pediatrics throughout my nursing career, so I have been lucky enough to become comfortable with a wide variety of medical issues and patient populations. I have been a nursing supervisor at a pediatric clinic for about a year now, and this has expanded my nursing knowledge tremendously as well. My past work experience has helped me to gain more of a holistic understanding and to be able to more accurately predict what the assessment and treatment may be for many different medical conditions. I believe that my background in nursing knowledge has given me a great stepping stone to enter graduate school.

However, if I were to rate myself regarding this class, and graduate school in general, I would give myself the rating of advanced beginner. Up until reading from our assigned textbooks for this class, I did not fully understand the roles and vast background of advanced registered nurses. For instance, when I thought of advanced registered nurses, I only thought of certified nurse practitioners and certified registered nurse anesthetists. Finding out how much I did not know about the field of advanced registered nurses makes me very excited to gain a further understanding throughout this class.

I have had the opportunity to work a long side many great advanced registered nurses, and I have learned many of their roles. A handful of my close friends have gone through similar schooling, so I have been able to gain minimal knowledge from their experiences as well. With that being said, I still have so much knowledge to learn, and I am very eager to dig deeper into the specific roles.

Unfortunately, I was not able to open the second link on this discussion, so I may be missing some information with my reply to this discussion post

A holistic understanding of our role as future healthcare leaders and nurse practitioners will go a long way to have a better understanding of the healthcare needs of our patients. For example, we are now dealing with patients not only suffering from hypertension, diabetes, obesity, mental health issues, CAD, and heart failure but also gender dysphoria. We have to traverse that sensitive topic of gender identity crisis because patients can be suffering from anxiety, depression, and suicidal tendencies on top of the disease diagnosis and treatment. Nurse practitioners will fill the gap of primary care providers that can also care for sensitive issues like transgender healthcare provisions.

MS Nursing field continues to evolve over time, and the AANC defines APRNs as nurses providing direct care to patients. Demand for healthcare providers is overloaded and the system is stretched to the limit. This demand has created new nursing roles for APRNs to work in collaboration with physician oversight. The advanced nursing practice creates opportunities and res[possibilities to provide care to patients in a broader way.

In my 7 years of Nursing experience, I have worked in Medical-surgical nursing, Telemetry, and now the Intensive Care Unit. According to Patricia E. Benner’s Novice to Expert theory, I would rate myself as proficient. I remember when I started as a new graduate nurse, I learned how to draw insulin in the syringe, inserted a foley and nasogastric tube, and understood the basic tasks as a Registered Nurse. As foreign graduate, we did not have many resources to have hands-on experience and our understanding of how to do things as a nurse is very limited. I was very task-oriented and was not looking at the bigger picture. I had a mentor and an educator who helped me grow and learn simple tasks. When I started telemetry, I was very comfortable in basic nursing tasks and started looking at the bigger picture, learning why and how the patient acquired the disease. I realized that’s when everything I learned in school and hands-on experience come along. Then I started working in the Cardiac Intensive Care unit, and that’s when everything came together. I started sharpening my critical thinking skills and learning more in-depth specializing in cardiac diseases. As a travel nurse, I get to experience all types of Intensive Care Units from Medical-Surgical ICU to Trauma ICU. I have so many things to learn and will continue to learn as much as I can. I was able to take my telemetry and Critical Care certifications as well as my cardiac certifications to test myself and grasp a better understanding of how to take care of my patients.Hey,

I’m impressed with your response and the experience you’ve gained through your RN career. it’s important to learn that nursing is a growing profession. my clinical experience will be handy in the MS program. the most significant challenge will be re-learning how to write research papers and use the APA format since it’s been a long time doing it. the nursing profession is ever-growing and advancing in relation to needs and demands and this category includes Nurse Midwife, Nurse Practitioners, CRNA, and Clinical Nurse Specialists(DeNisco et al., 2019). other fields like Nurse Administrators, Policymakers, and Public health nurses were established to integrate leadership, research, education, and clinical management to improve healthcare outcomes.

With the evolvement of the nursing profession’s versatility and adaptive capacity, new career pathways for nurses have developed , attracting a larger and more broadly talented applicant and leading roles to expanded scopes of practice and responsibilities for nurses. Nurses have been an enabling force for change in health care along many dimensions (Aiken et al., 2009) .The changing landscape of the health care system and the changing profile of the population require that the system undergo a fundamental shift to provide patient-centered care; deliver more primary as opposed to specialty care; deliver more care in the community rather than the acute care setting; provide seamless care; enable all health professionals to practice to the full extent of their education, training, and competencies; and foster inter-professional collaboration. Achieving such a change will enable the health care system to provide higher-quality care, reduce errors, and increase safety. Providing care in this way and in these areas taps traditional strengths of the nursing profession.

Title: DESCRIBE HOW THE ROLE OF ADVANCED REGISTERED NURSE TRANSFORMED OVER TIME NUR 513

The advanced practice registered nurse (APRN) has seemed to evolve based on shortages and demands of the population within our healthcare system. Deficits have expanded many nursing roles, leading to the need to define the nursing scope of practice and educational requirements. The traditional four roles of APRN, nurse practitioner (NP), certified nurse-midwife (CNM), certified registered nurse anesthetist (CRNA), and clinical nurse specialist (CNS); have all grown exponentially since the 20th century (DeNisco & Baker, 2016). These four roles all seemed to have stemmed from disparities in our healthcare system, whether it be a shortage of physicians, under-severed rural areas, vulnerable populations, or times of war. A great example of this is the advancements within the CRNA field. CRNAs were initially brought about during World War I due to a physician shortage during wartimes (DeNisco & Baker, 2016). The continuation of wars through United States history gave the CRNA field a strong foothold as a chief provider of anesthetics to the frontline. This expansion of the nursing scope of practice in anesthetics eventually led to credentialing and the American Association of Nurse Anesthetists (AANA) to monitor and regulate this growing field (DeNisco & Baker, 2016). Similarly, the growth and expansion of the other original APRN roles through the end of the 20th and 21st century required the nursing field to define graduate-level educational requirements and each specialty’s nursing role. The transformation of traditional APRN roles has now begun to open doorways for other nontraditional advanced practice nursing areas like public health nursing, clinical nurse leaders, nurse administrators, nursing research, and nurse educators.

DeNisco and Baker do a great job of laying out the ongoing evolution and need of the APRN role in the American healthcare system and the need for more advanced practice educators to sustain the APRN field’s continuous growth (2016). With a growing and aging population and a continuing shortage of physicians, the APRN roles offer a cost-effective solution that can also improve patient satisfaction (DeNisco & Baker, 2016). I will say that this week’s reading has made the decision to continue my education in nursing even more appealing and necessary.

I agree with your statement that the advanced nurse practice numbers have soared high in the 21st century compared by the 20th century. The current Covid-19 pandemic has however, proved that health and wellness is a collective responsibility (Kovac, 2020). In this context both the patient and the nurse are at risk of contracting the disease regardless of age, gender or religious underpinnings hence adequate precautionary measures should be taken. The advanced nursing role has taken up active role in preventive measures of the disease by offering the routine guidelines of hygiene the emerging trends in the spread and severity of the disease. The concept of preventive care is one of the key pillars in the patient protection and well-being. The advanced nurse role requires the nurses to carry out educational programs, seminars and workshops that are meant to help the public on the need of maintaining our bodies for optimal health and reduced chances of hospitalization. The one situation where the advanced nurses’ practitioners have shown resilience is the tackling of the covid-19 pandemic. Countries that embraced strict, firm and taken early hard precautionary measures have known to be safer with very low reported cases in the country (Trnka et al., 2021)

Title: DESCRIBE HOW THE ROLE OF ADVANCED REGISTERED NURSE TRANSFORMED OVER TIME NUR 513

As I journey through my master’s program, I foresee many transformations occurring within myself. I believe I will become a stronger professional as I am challenged by the curriculum in the MSN program. As a registered nurse I am equipped with the tools to take care of my patients to the best of my ability. I am confident in my critical thinking and judgement, my assessment skills, and compassion for others. As a registered nurse (RN) the focus is directed to patient care. I expect that progressing to an advanced practice nurse (APN) will prepare me with knowledge to help make changes within the healthcare system. The focus as a master’s prepared nurse will no longer be specific to direct patient care, but policy changes, innovation, quality improvement and evidence-based practice as well. As the text reads, “Advanced practice nurses are being called on and challenged to lead changes in healthcare organizations to improve quality, decrease adverse events, reduce costs, and enhance patient satisfaction” (Boyd and Barker, 2024). My expectation is that as an APN with a concentration in leadership, I will be called upon to create action plans, and modify hospital processes to meet the current needs of the healthcare industry. For example, currently mental health is a priority for healthcare. We are continuously improving policies and procedures when caring for patients of this population.

In my current role, I sit in conversations about policy and procedure changes. These decisions have typically already been made by a committee of nurses holding advanced degrees. The research has already been conducted, and at that point I am present to provide feedback on how best to implement the changes at the bedside. Becoming an advanced practice nurse would allow me the opportunity to sit on those committees and create the policy changes that I know would help better the experiences for the patients and staff throughout the hospital. The changes that I am hopeful to see within myself are the broadening of my perspective and challenging my way of thinking. I would like to be able to see the “bigger picture” as it relates to leading change and improving the quality of patient care.

My practice will probably broaden as a result of the move to an advanced practice position. In addition to enhancing my personal abilities, this larger responsibility helps me deliver healthcare in a more efficient and patient-centered manner. I also hope to improve my leadership abilities. Leading healthcare teams and interacting with other medical experts are frequently critical roles for advanced practice nurses. Additionally, I see myself developing both emotionally and professionally. My clinical skills will deepen, which will enable me to handle more complex patients and make independent clinical judgments.

The previous paragraph is further emphasized by the statement that advanced practice nurses are entrusted with spearheading changes within healthcare organizations, seeking to elevate quality, reduce adverse events, cut costs, and boost patient satisfaction. The preferred leadership approach for attaining these objectives is transformational leadership. In 2004, the Institute of Medicine (IOM) allocated an entire chapter in the influential report “Keeping Patients Safe: Transforming the Work Environment of Nurses” to the theory and evidence supporting transformational leadership. The report cited research suggesting that transformational leadership is associated with better patient outcomes, decreased medical errors, and heightened staff satisfaction (DeNisco, 2023).
