What are two essential leadership skills needed to implement this plan?
Excellent communication skills will be essential throughout the plan implementation. Communication skills will be required in communicating my plan and goals of the plan to staff. I will need to communicate the reasons for the plan and desired outcome to generate support and convince the staff to work towards achieving the same goals (Fischer, 2017).
Interpersonal skills will be vital in leading the plan as successful as possible. This will entail being a good listener and delegator in leading the plan strategy (Fischer, 2017).

What are the specific actions/tasks you will need to implement on your unit to achieve your goal(s)?
Actions will include:
Conducting a risk-assessment of every hospitalized patient to determine patient factors that increase the risk of developing pressure ulcers such as bed-ridden, advanced age, fall-risk patients, and being overweight or underweight (Qaseem, Mir, Starkey & Denberg, 2015).
4-hourly turning of bed-ridden patients with the guide of a patient-turning chart.
Strict documentation of patient turning charts by the staff.
Proper skin-care of inpatients including cleaning, drying, and moisturizing of the skin with skin ointments with attention to pressure areas (Padula, 2017).
Use of pressure-reducing surfaces such as cushions on pressure points and alternating pressure mattresses to reduce mechanical load.
Use of advanced static mattresses or overlays in patients at risk of developing pressure ulcers. Advance static mattresses provide a constant level of inflation and support and distribute body weight evenly hence preventing ulcers on pressure point areas (Qaseem et al., 2015).
Supporting and cushioning the heel, to protect against shear and distribute pressure evenly
Giving nutritional supplementation to help promote skin integrity. Addition of nutrients, such as protein, vitamins, and minerals in the diet to improve wound healing (Qaseem et al., 2015).
Use of wheelchair cushions which are designed to distribute pressure evenly and provide added cushion support for patients using wheelchairs.
Silicone border foam dressing for patients with wounds. This is a water-resistant type of dressing with nonexpanding foam that maintains a moist wound site for healing (Padula, 2017).

How will you communicate your final plan to your staff?

I will communicate my final plan using face-to-face meetings where I will guide the staff on how the plan will be implemented. This will ensure that any clarifications in the plan are made.
I will also communicate via a Memo which will indicate the overall goal, strategies that will be used to implement the plan, actions for the staff to effect, and the targets at every step of the implementation process.
How will you keep your team motivated toward the goal?
I will keep my team motivated by continually reminding them of our initial goal of improving patient outcome and encouraging them to work towards the goal (Fischer, 2017).

I will collect data on the frequency of use of patient turning charts.

I will compare the data with previous statistics to determine if there has been an increase or decrease in the number of patients who develop hospital-acquired pressure ulcers.
How will you know you have accomplished your goal?

Environmental safety: This will help to evaluate patients’ safety in the hospital environment (Stalpers et al., 2016). It is the mandate of every health facility to provide health services in a conducive and safe environment. Environmental safety will help determine what should be improved to promote patient safety.
Delivering-same-sex-accommodation (DSSA) compliance: The DSSA compliance will help to establish whether the hospital prioritizes the safety, privacy, and dignity of patients (Stalpers et al., 2016). This should include evaluating DSSA compliance by assessing sleeping areas and bathroom and toilet facilities.

Late Policy

The university’s policy on late assignments is 10% penalty PER DAY LATE. This also applies to late DQ replies.

I strongly encourage that you do not wait until the very last minute to complete your assignments. Your assignments in weeks 4 and 5 require early planning as you would need to present a teaching plan and interview a community health provider. Your initial discussion post should be a minimum of 200 words and response posts should be a minimum of 150 words. Be advised that I grade based on quality and not necessarily the number of words you post. A minimum of TWO references should be used for your initial post. For your response post, you do not need references as personal experiences would count as response posts. If you however cite anything from the literature for your response post, it is required that you cite your reference. You should include a minimum of THREE references for papers in this course. Please note that references should be no more than 5 years old except recommended as a resource for the class. Furthermore, for each discussion board question, you need ONE initial substantive response and TWO substantive responses to either your classmates or your instructor for a total of THREE responses. There are TWO discussion questions each week, hence, you need a total minimum of SIX discussion posts for each week. I usually post a discussion question each week. You could also respond to these as it would count towards your required SIX discussion posts for the week.

I understand this is a lot of information to cover in 5 weeks, however, the Bible says in Philippians 4:13 that we can do all things through Christ that strengthens us.
