Advanced practice nurses play a critical role in the provision of care that matches the healthcare needs of those in need. They undertake the assessment, diagnosing, planning, implementation, and evaluation of care that they give to their patients. Advanced practice nurses also engage in reflective practice intending to improve the quality of care that they offer. Evidence-based practice is an essential component of care that advanced practice nursing gives to their populations. It entails ensuring that patients are provided care with guidelines from best-available clinical evidence. Therefore, this research paper explores my philosophy as an advanced practice nurse and a theory that is congruent with my future roles as a Psychiatric Mental Health Nurse Practitioner.

According to L. Gottlieb and B. Gottlieb (2017), philosophy in nursing refers to a statement or statements that show one’s attitude as well as perspectives forming their beliefs, values, and assumptions about the practice of nursing. Advanced practice nurses engage in the provision of care that addresses the health needs of their diverse populations. They work with the sickly to assess how their diseases and illnesses can be treated. As a result, I credit that the role of advanced practice nurses is not to address the health issues only. Instead, advanced practice nurses provide patients with pathways that can be used to regain their health in every aspect of the individual lives holistically. Some patients might wish to engage in activities that contribute to their health. However, there might be limitations due to their health conditions. Therefore, I believe that an advanced practice nurse has the responsibility of assisting these patients to achieve their independence in performing activities that contribute to their health and well-being. The management of a disease can lead to either recovery or death. In case of a lack of improvement, I trust that advanced practice nurses have the responsibility of promoting peaceful death. Patients should have a dignified death where any pain and suffering are minimized as possible (American Nurses’ Association, 2016). I believe that health, as well as the well-being of the patients, will be encouraged.

 

The health and well-being of individuals are dependent on their level of awareness about their health needs. They should be informed about how their healthcare needs can be treated, with the adoption of positive lifestyles and behaviors. I believe that advanced practice nurses have the responsibility of ensuring that individuals, families, and communities are aware of their health needs, risks, and ways of preventing them. They engage in activities like health education and the provision of preventive services such as vaccination. It’s to promote the health of the vulnerable in the community. They also participate in researches that focus on addressing the health issues affecting their populations (DeNisco, 2019). Theories are critical in the practice of nursing. They inform the interventions that are used to meet the diverse healthcare needs of the patients. Dorothea Orem’s theory supports my future roles as an advanced practice nurse. Dorothea Orem created the self-care theory. The theory focuses on the ability of individuals to engage in self-care activities independently to maintain their health, life, and well-being. It emphasizes that patients should always be encouraged to bring out their best in times of illness for their health and well-being. According to it, nurses have the responsibility of providing care that maintains or improves the functioning of the patients for their recovery from a disease or illness (Masters, 2019). The support that nurses give to the patients aims at enabling them to perform their self-care.

According to Orem, nurses provide their care with a focus on many assumptions. The first assumption is that individuals or patients should be self-reliant and be responsible for their care. Patients are perceived to be distinct beings with unique healthcare needs. As a result, the nature of nursing care is a product of an interaction between nurses and patients. The theory also assumes that patients’ socio-cultural context is important in the uptake of nursing care (Masters, 2019). Therefore, nurses should understand the multi-dimensional nature of the healthcare needs of the patients and how they can be treated.

Dorothea Orem’s theory has goals that are congruent with the goals of nursing practice. The theory aims at increasing the independence of the patients.  It also embarks on activities that contribute to their health and well-being. It also focuses on ensuring that nursing practice promotes patient empowerment through a consideration of their diverse needs and ways of meeting them. According to Orem, nurses must interact with their patients to understand their health needs. This understanding informs the plans of care that should be used to promote the recovery of the patient from their illnesses. The theory also advocates the provision of safe, ethical, and efficient care. These provisions align with the goals of nursing practice; hence, the relevance of the theory (Masters, 2019). They also make its context to be in alignment with those of nursing practice (Masters, 2019). The self-care agency is a complex acquired ability of mature and maturing persons to know and meet their continuing requirements for deliberate, purposive action to regulate their own human functioning and development (Alligood 2018).

There exists similarity in the variables of Orem’s theory with those of nursing practice and its relevance to nursing research. The approach has concepts that include nursing, environment, person, and health. The theory perceives nursing as an art, technology, and helping service. It consists of the use of the patient’s perspective to promote health, well-being, and recovery. It perceives health as the primary and sound functionality of human systems. The environment consists of factors that are important for social development. The nurses engage in activities that promote independence in patient’s abilities in meeting their health needs. The patient is the receiver of nursing actions and is an independent being that explores ways of meeting his or her health needs (Masters, 2014). These concepts guide nursing practice. Several studies have been conducted to examine the applicability of Orem’s theory in nursing practice. Examples include the researches by Naz (2019) Younas (2017) and Santos, Ramos, and Fonseca (2017), where the use of Orem’s theory in nursing practice has been explored. However, Orem’s theory has significant implications for research.

Application of Theory to Future APN Role

I will apply Orem’s theory to my future APN roles as a Psychiatric Mental Health Nurse Practitioner in several ways. One of them is research. I will utilize it in research to explore how optimum goals of care can be achieved when dealing with a variety of mental health and substance abuse cases. Through this, I hope to contribute to evidence-based practice in mental health nursing. Orem used philosophical and scientific methods in developing her insights and validating her conclusion (Alligood, 2018). I will also apply it in the nursing assessment of mental health cases. I will use it to explore the healthcare needs of the patients, their ability to achieve them, and how I can facilitate their realization. I will also apply it in planning the healthcare needs of the patients I will work with in the future. I will use it to determine the strategies that can be used to promote the health and well-being of the patients I serve. I’ll utilize it in implementing and evaluating the care that I give to the patients. Further, I will use it to determine the effectiveness of the care that I provide as well as the patient’s ability to achieve their mental health-related goals independently. A helping method from the nursing perspective is a sequential series of actions that, if performed, will overcome or compensate for the health-associated limitations of persons to engage in actions to regulate their own function and development or that of their dependents (Alligood, 2018).

In summary, nursing philosophy has significant implications for my future roles as advanced practice nursing. It informs my ability to provide safe, efficient, and quality care. It also reinforces the provision of care that considers the cultural needs of the populations in need of healthcare. Nursing theories influence the practice of advanced practice nurses. They provide the bases in which the healthcare needs are determined and evaluated aftercare has been given. Therefore, advanced practice nurses should explore nursing theories that relate to their philosophies and how they can be applied to their professional roles.
