Case management is one of the ways in which quality and continuous care can be provided to diverse populations. It entails the organization as well as coordination of formal and informal network of activities that optimize the health and wellbeing of the patients. Case managers focus on ensuring that there is active stakeholder engagement to promote sustainability and empowerment of the patients and their significant others. It therefore demonstrates that the success of case management is highly dependent on the manner in which the existing resources are utilized. In this case, the focus of care should be on ensuring that limited resources are utilized in a manner that results in optimum health outcomes for the client. Consequently, this research paper explores an interview I conducted with a case manager of Institution X. It explores the manner in which they determine community resources, identify the available agencies, and methods used to determine the relationship between different stakeholders.
The interview for this assignment was conducted with case manager for institution X. The institution specializes in the provision of case management services to clients in and out of the State of Georgia. One of the questions that I asked him was concerning the ways in which they determine community resources that might be required for a case. According to him, the organization utilizes a wide range of strategies to determine the community resources needed for case management. One of them is the use of interagency case management. This is an approach where the organization contacts other agencies in the region where a client resides in to determine the available community resources. They connect with other case managers in determining the ways in which the available community resources can be utilized optimally to promote recovery and wellbeing. The agency also utilizes information from United Way, Public Health Department, and county government to determine the resource availability and need, potential gaps in service availability, and provider linkages. They also provide them with data on health, social services, health and housing within the community, hence, the determination of the interventions to meet the needs of the client in the community (Weinstein et al., 2017). The case manager also reported the use of directories to identify the potential resources in the community. The last approach used by the organization is geo-mapping. In this approach, automated package is used to identify the resource availability in the community.
The interview also sought to obtain information concerning the manner in which the case manager identifies the available agencies. One of the ways in which the case manager determines the available agencies in the community is geo-mapping. According to him, the use of technology enables them to determine the location and contacts of the agencies that might be needed to provide essential care to the clients in the region. There is also the use of data from Public Health Department of Georgia where the list and contacts of the available agencies is obtained. The case manager also reported the use of snowball surveys to identify the available agencies. In this method, they use interagency surveys that will lead them to other agencies. The existing agencies are used to provide linkages with other agencies in the region (Zander, 2017). Published directories are also an important source of information related to the available agencies. However, the case manager noted that this method is associated with weaknesses that include being outdated and consumes too much time when determining the accuracy of the published data. He also asserted that published directories of the available agencies do not provide the needed information in case of a shift in the focus of an agency. Therefore, a combination of these sources of data enables the case manager and his organization to identify the available agencies in the region.

The interview also sought to establish the manner in which the case manager initiates, monitors, and evaluates relationship between the patient and the resource that has been provided or suggested. In the first stage, beginning case management, the case manager
performs outreach, eligibility determination, and enrolls the client into the program. The manager reaches to the client to assess his or her needs, resource availability, support systems, and their effectiveness in meeting the identified needs. This phase allows the case manager to determine whether the client will benefit from case management services or not. The manager also reviews other information such as current health status of the patient, health history, claims information, and health habits to determine the ability of case management to meet the identified needs. The information obtained from this stage will determine whether the client will be enrolled to the case management program or not (Zander, 2017). Planning occurs if the client is determined suitable for the program. it is followed by the development of specific care goals, objectives, and actions that should be implemented to meet the identified needs of the client. According to the case manager, goals should be short and long-term. They should also be time-specific and action-oriented to allow for the determination of the effectiveness of the adopted interventions (Savage, 2020).
The specific interventions are then implemented and monitored collaboratively by the case manager, patient, family, and other stakeholders involved in the provision of care. The resources are modified to address the critical needs of the client. The case manager also acts as the liaison between the patient, providers, support system, and the insurance firm to ensure that optimum care is provided to the patient. Information on the response of the patient to the interventions is obtained on a regular basis to determine the effectiveness of the case management plan. According to the case manager, evaluation is an important step that allows them to determine whether the plan was sustainable or not. The evaluation process takes into consideration a wide range of aspects. One of them is the evaluation of the treatment outcomes. It entails determining whether the plan resulted in the realization of the pre-developed outcomes of treatment at the end of the engagement. Client satisfaction with the plan is also evaluated. The case manager seeks to determine the subjective perception of the client concerning the effectiveness of the program. The last aspect of monitoring that the manager identified is stakeholder participation (Savage, 2020). According to him, there should be active stakeholder participation to achieve outcomes that include stakeholder empowerment and sustainability of the adopted interventions.
There exists a wide range of resources for case management in my hometown, Georgia. They include case management agencies such as the Georgia Mentor and counseling center at the Georgia Southern University. These case management centers provide a wide range of services to children and adults with different conditions such as mental and developmental disabilities, mental health problems, and complex medical health conditions. They also provide advocacy, crisis, and emergency support services (Georgia Mentor, n.d.; Georgia State University, n.d.). The clients requiring these services must have an insurance coverage to support the care they need. However, the cost of services varies depending on the health care needs of a patient.
In conclusion, case management is important in the modern world since it promotes the provision of client-centered high-quality care. Case managers have the critical role of determining the resources needed to promote the health and wellbeing of their clients. They utilize information from different sources such as informant surveys and department of health to plan how the needs of their clients can be met. Therefore, active stakeholder engagement should be explored to ensure that optimum health and recovery of the affected is promoted.

For your response post, you do not need references as personal experiences would count as response posts. If you however cite anything from the literature for your response post, it is required that you cite your reference. You should include a minimum of THREE references for papers in this course. Please note that references should be no more than 5 years old except recommended as a resource for the class. Furthermore, for each discussion board question, you need ONE initial substantive response and TWO substantive responses to either your classmates or your instructor for a total of THREE responses.
