It may seem to you that healthcare has been a national topic of debate among political leaders for as long as you can remember. 

Healthcare has been a policy item and a topic of debate not only in recent times but as far back as the administration of the second U.S. president, John Adams. In 1798, Adams signed legislation requiring that 20 cents per month of a sailor’s paycheck be set aside for covering their medical bills. This represented the first major piece of U.S. healthcare legislation, and the topic of healthcare has been woven into presidential agendas and political debate ever since. 

As a healthcare professional, you may be called upon to provide expertise, guidance and/or opinions on healthcare matters as they are debated for inclusion into new policy. You may also be involved in planning new organizational policy and responses to changes in legislation. For all of these reasons you should be prepared to speak to national healthcare issues making the news. 

In this Assignment, you will analyze recent presidential healthcare agendas. You also will prepare a fact sheet to communicate the importance of a healthcare issue and the impact on this issue of recent or proposed policy. 

To Prepare: 

Review the agenda priorities of the current/sitting U.S. president and at least one previous presidential administration. 

Select an issue related to healthcare that was addressed by two U.S. presidential administrations (current and previous). 

Consider how you would communicate the importance of a healthcare issue to a legislator/policymaker or a member of their staff for inclusion on an agenda.

I worked as a nurse in trauma ICU for ten years and spent the last two years in the emergency room. I have also worked as a critical care transport nurse for the last four years. Psych has always been something that has intrigued me. I just find it so fascinating and frustrating all at the same time. I am really excited to be a part of the mental health community because it is really something close to my heart. I am so glad that people are more open to talk about their mental health struggles so that other people can support them and also take comfort in knowing they are not alone in their situation.

I have recently been asked to train to be a house supervisor at the hospital I work in. And I was also thinking about transferring to the psych department to get more experience of the process. I like to gain as much training as I can so that I can be a well-rounded nurse.

I have struggled with going back to school because I have three small children and I work a lot so I never really thought I’d have the time for it. Now I have decided that I need to make it a priority for me to do the best for myself to provide for my family.

According to The Centers for Disease Control and Prevention, the crisis began to emerge in the 1990’s. This began with over prescribing, which led to abuse and addiction. This has resulted in the current opioid crisis. Under the Trump administrations’ agenda, the agenda focus is currently sterner on opioid abusers and are well equipped with better and more advanced measures on fishing out culprits engaged in these acts. There are more strategic checks and balances that have been put in place under the current administration to limit the influx of illegal drug trafficking into the country.

There are also very tight security protocols and measures on the boarders of the country which is aimed at reducing the number of people affected with opioid abuse and dependency. (WhiteHouse.gov) The Obama administrations’ agenda one of the primary focus was to develop special treatment protocols for individuals suffering from opioid abuse and dependency issues and on creating incentives to ensure that there are more thorough measures in place for regulating opioid prescription by medical practitioners and dispensing means from pharmacies. Monitoring of opioid use and events located at neighborhood law enforcement stations for the return of excess drugs that have been supplied illegally within the system. Community drug dispensaries are accepting of these kinds of take back drives to support drug rehabilitation in chronic abusers addicted to drugs. They targeted the center of the illegal opioid supply with more advanced investigative approaches by law enforcement agencies which was a huge preventative method. During The Bush era in administration, his regime, in 2003 there was an estimated total allocation of $19.2 billion raised to fund law enforcement efforts in the battle against opioid trafficking and all other criminal activities associated with drug abuse. The Trump Administration took on the approach of spreading awareness and education the teens and adolescents in our society by partnering The Truth Initiative and Ad Council, which was a campaign initiated to teach them on how to prevent and reduce opioid abuse and addiction. It was very comprehensive in helping young adults understand the dangers associated with addiction as well as how effective they could be in curbing the spread of this problem in various communities. This ad was designed to get the attention of the younger population in the society as it successfully has over 1.4 billion views in total when it aired. This administration declared the current opioid addiction issue a nationwide public health emergency, which has brought so much more attention to this crisis. This prompted and inspired people to become more active in the fight against opioid abuse and dependency. More intense measures were taken at the boarders to intensify the patrol and security measures to avoid smuggling drugs illegally into the country. There have also been measures taken to ensure that opioid prescriptions ate regulated thoroughly, by introducing a Safer Prescribing Plan that will reduce any opioid refill prescription by 1/3 in the next 3 years. Finally, the administration is that will cut opioid prescription fills by one-third within three years.

Finally, the administration has been a strong proponent for vaccines that prevent opioid addiction. (WhiteHouse.gov) The Obama administration offered health care coverage plans that were mandatory for individuals battling with opioid abuse and addiction. This was able to facilitate their treatment regimen and made provision for naloxone, a drug of choice administered to patients that have been suspected of high levels of Opioid-Induced complications. This administration also strengthens efforts at combating drug trafficking which drastically reduces opioid overdoses and deaths. The White House National Drug Control Strategy devised a strategic plan based on evidence-based practices that aims at controlling opioid addiction and substance abuse. The primary goal of this approach was focused on public health, by creating programs that encourage the safe and proper use of opioids and comprehensive treatment plan for this suffering from opioid dependency. Finally, the administration encouraged clinicians and their stakeholders to collaborate effectively in the implementation of more structured protocols while prescribing opioid to the right patients. The Obama administration set up the Substance Abuse and Mental Health Services Administration (SAMHSA) that was focused in ensuring that adequate funding was allocated to individuals suffering with mental illness as they believed it was a caused and effect related diagnosis between individuals suffering with mental disorders and opioid addiction. This organization offered programs that were instrumental in addressing underlining issues in opioid addiction, encouraged preventative strategies that would help reduce opioid related deaths and complications. Finally, there were support groups that were made accessible to chronic opioid users and addicts which would provide comprehensive treatment and rehabilitative services as well extra funding for recovery program expansion. The Bush administration did not completely recognize the effect of opioid crisis and didn’t shed much light on the damage it caused on the overall health and well-being of local communities. Setting the Agenda Program Transcript

FEMALE SPEAKER: How do you get a policymaker’s attention? It’s very simple. You develop a relationship with that person. Don’t wait until you need something. Develop the relationship early on.

FEMALE SPEAKER: Sometimes it’s difficult to get issues on the agenda that are our issues, that might be an issue that’s of interest to a nurse, if it doesn’t align with the interests and the priorities of that elected official. So it’s really important to know what their priorities are, what their health issues are that they’re most interested in. You can usually Google their name. You can get their official bios. You can read about the committees that they serve on and the issues that matter to them. And look to see whether or not their agenda matches with yours, or if there’s a way you can leverage your issue into their agenda.

FEMALE SPEAKER: Know exactly what the congressional members have on their agenda. And you can get them online, through their local office, or through their congressional office in Washington, DC. Find out what they’re interested in. Find out how you can help them. You need to call them up, or you need to go visit the office and find out exactly what is being done.

FEMALE SPEAKER: Call up a state senator and say, I am so-and-so. I would like to meet with you. I’m a registered nurse. And I think I may have something that you might be interested to talk about.

FEMALE SPEAKER: Do the homework you need to do to get up to speed, not just on the issue and a potential solution you may be advocating, but also their background so that you can calibrate your conversation with them in ways that will resonate for them, that they’ll understand and appreciate.

FEMALE SPEAKER: It’s often helpful to have what is known as a policy entrepreneur. That’s a big word that simply says, do you have anybody who’s on your side or who’s really interested or who can continue to work with your policymaker when you’re out doing your own practice. It could be a retired legislator. It could be a current legislator, someone who’s willing to support a bill and then could perhaps introduce your bill, because only legislators can introduce bills. The president cannot, the governor cannot. That’s the purview of the General Assembly or the Congress.

FEMALE SPEAKER: People want to hear from their constituents. Ideally, you are communicating with the people who represent you. So that’s almost always the first place to start, your elected official, not the one two states over. That might be appealing, because you know their name from national news. And you certainly

can communicate to them. But the voices that often matter the most, I’ve found, are the voices from local communities back home in the district or the state that that Senator or Congressman or state legislator represents.

FEMALE SPEAKER: You have to make sure that you are in it for the long run. These kinds of political processes and policy issues may take years. So you say, oh, my gosh, are you on this forever. Sometimes it seems like it. It may take one session of two years. It may take two. It may take five sessions. But if it’s important enough to you, you need to go.

FEMALE SPEAKER: This particular meeting with the president was my coming over to the White House, because he’d asked me to meet with him. And he started by saying, Mary, I want to thank you for. And I did what people probably never should do and that my mother would have been very unhappy about my having done, I interrupted the President of the United States. And I did that because I thought I had something more important as a nurse to tell him than he had to tell me.

So I had hijacked that meeting with him. And given that feedback, very quickly went through those programs that I thought were important for him to hear about, told him the impact, speaking as quickly as I am right now to convey that message. There is information that nurses are uniquely positioned, because of our expertise and our practice, to inform the work of policymakers. And you don’t ever want to lose that opportunity to do just that, even when you’re talking to the President of the United States.
