Summary of the Currency to Address the EBP Project Issue

The work environment in hospitals today is making a shift from the traditional volume-driven to the current value and outcome-driven approach. The premise aims to address speed and efficiency in care for patients (Dang & Dearholt, 2017). Besides, stakeholders in care facilities focus on how to optimize value of care so that patients experience positive outcome with regard to their condition. Evaluation and design of staff schedules is a key determinant of safe practice of care to improve positive outcomes among patients. Reducing work hours and extended shifts for nurses is vital in addressing quality outcomes in care facility.

Patients admitted in hospitals require a day and night monitoring every day by nurses. The ability to fulfill this need depends upon the availability of nursing staff in a facility. Often, an 8-hour shift is the recommended time for nurses to work for effective care for patients. Debate continues to rise on the nursing working hours especially those that exceed the recommended 8 hours. Research by Ball et al. (2017) indicates that human beings are more effective when they work at daylight as opposed to night. Furthermore, working for long hours in extended shifts alters the regular circadian rhythm, and this mostly affects those on night shifts. Long working hours cause burnout and fatigue among nursing professionals. The symptoms are evident through muscle weakness, exhaustion as well as decreased cognitive function. Fatigue among nurses also leads to anxiety, lethargy, listlessness, and low concentration.

Nurses take the longest period with patients during care, and this affirms their most significant role in determining the prognosis of the diagnosed conditions. For nurses to respond to care effectively, their work environment should be devoid of pressure. In case nurses work for long hours or extended shifts, they lose their capability to respond to flash decisions on patient care. Fatigue and anxiety are detrimental to both patients and nurses. The latter’s job will be at stake, and this can result in dismissal if duties are found unsatisfactory (Yoost & Crawford, 2015). Patients, on the other hand, can experience poor disease prognosis that may result in increased length of hospital stay or even death.

Possible Causes of The Difference From The Expected Outcome

The proposed intervention to reduce work hours is a contentious topic of discussion that care facilities attempt to address. While facilities vouch for such a response, the entire process is challenged by a shortage of nursing staff. From a study in a Swedish

hospital by Gyllensten, Andersson, and Muller (2017), a reduction of the working period to 6 hours decreased unemployment and increased the nature of competitiveness of the firms in the industry. However, while the intervention reduced many shifts, it had a negative impact on the overall quality of nurses. Research by Stimpfel and Aiken (2013) indicates that a reduction of the work hours correlated with the removal of breaks that would otherwise be used by nurses to gather more energy for quality work. Such an intervention increases intensity of the work situation for the professionals. Findings by Ball et al. (2017) to investigate psychophysiological effects from intense work situation indicates that the body’s adrenaline is activated based on the number of breaks available for works. Lack of energy recovery time due to limited breaks causes stress-related conditions.  While the intervention aimed at reducing shift hours may address the EBP project issue, it can also be challenged by several factors related to poor recovery from work-induced fatigue. These factors include stress, health status, coping factors as well as working conditions (Gyllensten, Andersson & Muller, 2017). Despite the interest in reducing work hours for nurses, research investigations also reveal the detriments of the initiative especially with regard to psychosocial working setting.

Effect the EBP Project Issue is Having in Practice

Reducing the number of work hours is anticipated to yield a positive impact on nurse practitioners. Studies by Gyllensten, Andersson and Muller (2017) indicates that reducing the working time from 8 to 6 hours enhanced job satisfaction among nurses. The premise allows nurses to have adequate energy to work while they retain their salaries as well as pension and other employment benefits. Further, the research affirms that the intervention improves the work-life balance among nurses and the overall willingness of the professionals to stay in their current job. Shorter workdays due to reduced number of shift hours improve recovery among nurses especially those working in surgical wards where care procedures can be tiring (Stimpfel & Aiken, 2013). The EBP practice results in a decrease in the use of overtime hours especially those related to predictive workload volumes. It means that proper planning is conducted to spread nurse schedules equitably as per the recommended shift hours.

Current Research Evidence On New Practices For Addressing EBP Project Issue

With concerns on quality driven care, facilities are embracing the 6-hour shifts for nurses, but the uptake is still slow due to a shortage of licensed practitioners across the world. The new practice determines the schedules in work stations without a reduction in salary to translate into positive performance among nurses. According to a study by Gerasimou‐Angelidi et al., (2014), a 6-hour shift improved job satisfaction for nurses. The nurse participants in the study were healthier, alert and had less sick days compared to those on 8-hour or 12-hour shifts. The intervention will require care facilities to hire more nurses, reduce the turnover rates of the seasoned practitioners and improve the welfare of staff through better remuneration. However, the reduced shift hours strain the financial resources of a care facility, and this limits the level of competitiveness of such institutions.

Summary of the Evidence-based Practice Model

The following practice framework will guide the implementation of the reduced shift hours for nurses;

A reduction in the use of 12-hour shifts and implementation of 8-hour or 6-hour shifts: Care facility to engage the nursing staff on discussions about alternative work schedules that optimize quality outcomes.

Adhere to strict end of shift hours: The facility administrators to assess possible cause of extended shift hours by ensuring that nurses report to duties 30 minutes earlier.

Workload allocation: Addressing work hours implies allocating duties based on the ability and health status of nurse practitioners (Yoost & Crawford, 2015). Workload volumes should reflect the average number of nurses available so that care services are planned based on the nurse-to-patient ratio formula.

            Monitor the available key performance indicators on the trends of staffing: The change process in shift hours entails the analysis of workforce metrics as leveraged on trends in overtime units, rates of absenteeism, actual work hours and rest intervals between work shifts (Ball et al., 2017). These guides in the planning and monitoring of performance related to reduced shift hours at the facility.

Plagiarism is highly prohibited. Please ensure you are citing your sources correctly using APA 7th edition. All assignments including discussion posts should be formatted in APA with the appropriate spacing, font, margin, and indents. Any papers not well formatted would be returned back to you, hence, I advise you review APA formatting style. I have attached a sample paper in APA format and will also post sample discussion responses in subsequent announcements.

Your initial discussion post should be a minimum of 200 words and response posts should be a minimum of 150 words. Be advised that I grade based on quality and not necessarily the number of words you post. A minimum of TWO references should be used for your initial post. For your response post, you do not need references as personal experiences would count as response posts. If you however cite anything from the literature for your response post, it is required that you cite your reference. You should include a minimum of THREE references for papers in this course. Please note that references should be no more than 5 years old except recommended as a resource for the class. Furthermore, for each discussion board question, you need ONE initial substantive response and TWO substantive responses to either your classmates or your instructor for a total of THREE responses. There are TWO discussion questions each week, hence, you need a total minimum of SIX discussion posts for each week. I usually post a discussion question each week. You could also respond to these as it would count towards your required SIX discussion posts for the week.

I understand this is a lot of information to cover in 5 weeks, however, the Bible says in Philippians 4:13 that we can do all things through Christ that strengthens us. Even in times like this, we are encouraged by God’s word that we have that ability in us to succeed with His strength. I pray that each and every one of you receives strength for this course and life generally as we navigate through this pandemic that is shaking our world today. Relax and enjoy the course!
