Through this collaboration, patients are able to have decreased 30-day re-admission rates among high-risk Medicare patients (Roepers et al, 2017). This innovative model affects interprofessional collaboration because all members of the team work together for the benefit of the patients. This organization has a medical command center (MCC) and a mobile integrated team (MIT) that are both available 24/7 for coordinating resources and managing the health of the patient. The utilization of home-based care providers in collaboration with clinicians and nurses has proved to be cost effective in terms of reduced rate of readmission hence create more space at the facility for patients who are in critical conditions and require highly specialized care. The use of MDT collaboration has reported higher success rates in the oncology departments specializing in head and neck cancers. The MDT comprises of ENT (Ear, Nose and Throat), maxillofacial surgeons, medical oncologists and radiologists (Taberna et al., 2020). I do agree with you that the organizational capacity to incorporate telemedicine into its daily operations have promotes the collaboration and increased the scope of healthcare. The use of telemedicine has proved to be the best option in the wake of Covid-19 pandemic hence reduces chances of spread of the infectious diseases and also eliminates the chances of one contracting hospitalization related infections (Kichloo et al., 2020).

Cleveland Clinic is a regional healthcare delivery system that offers a variety of care through numerous providers and facilities linked by multi-modal transport and information technology. Their system is focused on a patient-centered model of care. Cleveland Clinic has organized its system to provide patients with the appropriate level of care at the right time. By integrating healthcare systems and providing access at all points of care, healthcare costs are reduced. Some other ways cost is reduced include reducing wait times for patient visits, reducing length of stay, implementing drug utilization management strategies, using the electronic medical records to make appointments, and eliminating duplication of services. Another function of this home-based model is to follow up with home visits for recently discharged high- risk patients. The first visit is conducted by an advanced practice nurse. If the patient is considered a high-risk for readmission to the hospital within 30 days, the patient will receive two to five visits from paramedics, along with virtual visits from a physician, within a 30- to 45-day period (Cleveland Clinic, 2020).

The emergence of Covid-19 posed a major challenge to hospitals capacity to cope up with the large number of patients in need of critical care services. However, with the advanced system of home-based care at Cleveland clinics, it was easier to manage the situation. The organization has well established transport routes and telecommunication networks that allows the primary healthcare providers to attend to patients at the comfort of their homes and
