As a registered nurse, I get the privilege to sit front and center, as well as participate in the advancement of the nursing profession and healthcare in general. The National Academies of Sciences, Engineering, and Medicine, The Future of Nursing 2020-2030: Charting a Path to Achieve Health Equity details a comprehensive review for what is necessary in an ever-changing environment of safety, culture, education, leadership, and practice within nursing. Although I see the benefits of all the recommendations within this report, the concept that nurses improve health equity stands out as one of the most effective for creating change within the industry. Health equity is “the state in which everyone has the opportunity to attain full health potential and no one is disadvantaged from achieving this potential because of social position or any other socially defined circumstance” (NASEM, 2017, as cited in National Academies of Sciences, Engineering, and Medicine, 2021). Nurses play a particular role in screening for social issues that can interfere with an individual’s access to food, a stable home, transportation, basic needs like heat and water and domestic violence (National Academies of Sciences, Engineering, and Medicine, 2021).
This is of particular interest to me as a former emergency department nurse that often conducted these screenings, provided food and temporary shelter to those that did not have the means to acquire it and gain the trust of some at risk of self-harm or domestic violence. Although these short-term provisions did not directly solve any health inequities, it highlighted areas of needs within the community and allowed me to collaborate with nurses in social services and care management to begin bridging these gaps.

The consensus report of the National Academies of Sciences, Engineering, and Medicine, The Future of Nursing 2020-2030: Charting a Path to Achieve Health Equity, made recommendations to improve the equity of healthcare for the population. These recommendations are a step in the right direction toward preparing nurses to promote health equity, reduce health disparities, and improve the health and well-being of everyone (The National Academies Press, 2021).
The consensus report identified while most nursing education programs included content on social determinants of health (SDOH) within the public or community health courses, the material was not integrated throughout the curriculum. The recommendation for the accrediting body to include health equity and SDOH would require nursing education to prioritize the inclusion of these standards in their curriculum to maintain accreditation.
The report revealed that nurses and nurse practitioners who work in schools, public health, community health, and emergency and urgent care settings do not believe they received adequate training in recognizing SDOH. I believe that incorporating SDOH training for nurses in all practice settings would be beneficial for patients as providers could refer patients to resources, as needed.
As a nurse who has worked away from the bedside for many years, I have witnessed healthcare providers who do not consider poverty, unequal access to healthcare, racism, or education when developing a plan of care for medically complex patients. It is important to recognize the most well-written plan of care will not be followed if the patient does not have the knowledge or resources to maintain compliance with the plan.
I agree that nurses will remain on the front line of change within healthcare. Advanced practice nurses must be educated on SDOH to continue to evolve and provide equitable care for patients in their charge.

Thank you for highlighting the importance of incorporating education regarding the Social Determinants of Health (SDOH) in the nurses practice. I read about one of the recommendations where nurses should be educated by physicians and other professionals as students and throughout their career (National Academies of Sciences, Engineering and medicine, 2021). To be honest, as one of the bedside nurses, I thought this was not very important in creating change in the industry. However, reading your experience, I agree that other professionals who have expertise in different areas of work such as research, public health, or mental health may help nursing students as well as practicing nurses to keep a brace with the current situation and the needs of different populations that end up in hospitals. This is in line with the primary goal of nursing education which emphasizes that nurses should be prepared to meet the diverse needs of the patients (National Academies of Sciences, Engineering and medicine, 2021). This kind of training will help equip nurses with the necessary knowledge on how to care for them, information on available resources in the communities and will provide seamless transition of patients from hospitals to communities.

The National Academies of Science, Engineering, and Medicine, The Future of Nursing 2020-2030: Charting a Path to Achieve Health Equity report was a collaboration between the Robert Johnson Foundation and the Institute of Medicine(IOM) to explore the contributions of nursing in addressing the social determinants of health (National Academies of Sciences, Engineering and medicine, 2021). The report tabled recommendations regarding nursing education that are aimed at improving healthcare.
The recommendations regarding the nursing educations that I believe will be most effective in creating change in the industry focused on training, accreditation and advancement in the nursing profession. On training, the report recommended that nurses should receive education to help them function as leaders, advance science and meet diverse patients needs (National Academies of Sciences, Engineering and medicine, 2021). For nursing profession fully advocate for itself, there needs to be nurses on the policy table coupled by sound research to push for reforms such as allowing advance practice nurses to practice with multi-state licenses. On advancement, the report emphasized the need for the entry level nurses to understand the importance of furthering their education while providing opportunities to do so early in their career (National Academies of Sciences, Engineering and medicine, 2021). When nurses graduate with an understanding of the many pathways of growth, it helps them continue into the many divisions of advance practice nursing.
A community college in Rapid city has produced thousands of licensed practical nurses(LPN) who are immediately transitioning to Associate Diploma in Nursing (ADN) through their LPN -ADN program and many are later joining online Registered Nurse (RN) to baccalaureate (BSN) programs while already employed. Regarding accreditation, the report further recommended that the accrediting and licensing of nurses should be based on demonstrated mastery of core skills and competencies besides the board examination. I believe this requirement will enable the production of competency sound nurses with hands on and critical thinking skills. I have met new graduate nurses who are very good in theoretical knowledge but lack simple competencies required for the job. Trying to learn a host of new skills and competencies, results in possible frustration leading to loss of passion in the profession which should not be the case.
Some sections of the report suggested that baccalaureate prepared nurse should be the entry level while phasing out associate diploma in nursing training within a stipulated time frame. While BSN prepared nurses are associated with positive patient outcome, lower mortality rates and reduced hospital stay (Blegen et al., 2013), this move will deny the low income earning students from pursuing nursing due to the cost or the distance to the institutions. Moreover, the ADN to Masters of Science in Nursing (MSN) program has been noted to have contributed majorly to advanced practice nurses and the nursing faculty (National Academies of Sciences, Engineering and medicine, 2021). Therefore, I disagree with the recommendation to phase out the ADN programs instead, reinforce the need for career progression in the nursing curriculum.
