Nurses play a critical role in the promotion of the health of the communities that they serve. They engage in the identification of health problems that face communities and how they can be addressed. They utilize information from community diagnosis to come up with interventions that aim at empowering the members of the community to adopt healthy behaviors and lifestyles. Therefore, this research paper explores an agency visit that was conducted to identify the health needs, resources, and ways in which the health of the community can be promoted.

Description of Agency

The data for the assignment was obtained from Outpatient Recovery Center that is located at Fenton Street, downtown Silver Spring in Montgomery County, Maryland. The ecomap for the research is based on Aspen Hill Community. The organization had different staff involved in the provision of care to diverse communities. They include nurse practitioners, physicians, psychiatrists, and registered nurses. The Branch Director of the organization was interviewed for this research. He was considered the most suitable person to provide the information needed for the assignment due to his long experience in the outpatient center. Similarly, he was among the people who spend most of their time with the patients in the unit. The outpatient center was established with the main purpose of providing mental healthcare to its diverse patients. The focus of the organization in this area of care is attributed to the increasing cases of drug addiction in the region. According to the director interviewed, the organization’s sources of funding include insurance payment and out-of-pocket payment for the services offered by the center.  Therefore, the agency has grown to be among the leading providers of mental health services in the region due to the quality of care it offers to the patients.

Impact of the Agency on the Community

According to the branch director who was interviewed in this assignment, the impact of the agency on the community is immense. One of the impacts is the promotion of the mental health and wellbeing of the populations it serves in the community. As shown earlier, the director who was interviewed reported that the outpatient center provides mental healthcare services to its populations. The care has been effective as evidenced by the reduction in the prevalence of rates of drug addiction in the region. There has also been a reduction in readmission rates among patients with drug-related mental health problems in the community.

The outpatient center has also influenced the health of the community by engaging in activities that promote their health and wellbeing. Often, health promotion activities aimed at promoting lifestyle and behavioral change among the target populations. These outcomes are achieved with the use of interventions that increase their level of awareness of the desired modifications that will promote their health (Clendon & Munns, 2018). According to the registered nurse interviewed in this assignment, the outpatient center provides several health promotion interventions that include screening and health education as a way of enhancing their health and wellbeing.

There is a high rate of utilization of teamwork in the outpatient center. According to the branch director interviewed in this assignment, the staff in the center work as a team. The decision-making process incorporates inputs from the different healthcare providers involved in the provision of care. There is also the utilization of approaches to care that promote patient-centeredness. The patients and their significant others are involved in the process of care to ensure their empowerment and sustainability of the interventions used to promote their health and wellbeing. The director also reported that the healthcare teams in the center work in shifts to ensure continuity in the care they provide to their patients. Teamwork has significant benefits for health organizations. It is associated with enhanced quality, safety, and efficiency of care. Furthermore, it increases patient satisfaction with care, hence, the realization of the desired goals of care in the center (Ellis, 2018). Therefore, teamwork has enhanced the quality of care offered in the center to patients suffering from mental health issues.

There is also the utilization of the concept of community partnership in the outpatient center. According to the director, effective partnership with the community is important for the realization of the desired health outcomes. Community partnership has been promoted in the center through the active collaboration it has with primary care centers in the community. There is also the involvement of legal experts in care in the decisions made in the center. This ensures that ethics is promoted when providing care to patients seeking their needed healthcare in the outpatient center.

Community Needs and Epidemiology

The health needs that were identified in the Aspen Hill Community included care that meets the needs of people with obesity, heart disease, pneumonia, and diabetes. On the other hand, effective management of adults with mental health and addiction issues were identified as the health needs in the Outpatient Recovery Center. According to data obtained from KOLMAC (2019), the center serves a large population of adult patients suffering from drug and alcohol addiction. Statistics further show that diabetes is ranked as the leading cause of mortality in the Aspen Hill Community. It is estimated that about 11% of people in Montgomery County, Maryland have diabetes. It was also estimated that about 33% of the adults in Aspen Hill were overweight in 2010. The overall statistics of overweight in Maryland in the same year was 34%. Therefore, obesity and behavioral health are identified as the most prioritized areas of health that should be addressed to promote the health and wellbeing of the people of Maryland (Healthy People, 2020).

Literature Review

As identified above, obesity was identified as one of the most prioritized health needs in the region where the agency visit was undertaken. According to the Maryland State Nutrition, Physical Activity, and Obesity Profile, 65.4% of citizens aged above 25 years are overweight or obese in Maryland. Of this proportion, 27.1% of them are obese with a body mass index of 30 or more. The profile further reveals that 18.5% of the population does not engage in any physical activity with 39.1% watching television for more than three hours a day (Maryland State Nutrition, Physical Activity, and Obesity Profile, 2012). Statistics among the children reveal that approximately 16.8% of the children between 2 and 4 years are overweight with 16.2% being obese in the state (Maryland State Nutrition, Physical Activity, and Obesity Profile, 2016). Effective management and prevention of obesity are highly dependent on behavioral and dietary modifications. This includes eating healthy diets and engaging in active physical activity. According to adult statistics on dietary behaviors in Maryland, 35.9% of the adults consumed fruit less than once in a day while 22.0% reported consuming vegetables less than once daily in 2016. In terms of physical activity, 48.6% of the adults reported achieving the desired minimum of 150 minutes of moderate physical activity every week (Maryland State Nutrition, Physical Activity, and Obesity Profile, 2016). Therefore, the high prevalence of obesity in Maryland increases the need for the adoption of measures that aim at addressing it in the state.

The other issue of health need that was identified during the agency visit was behavioral interventions to address the issue of drug addiction and dependence in Maryland. Drug abuse is a serious public health issue in Maryland. It is estimated that 8.11% of people in Maryland reported abusing illicit drugs between 2009 and 2010. This made Maryland be among the top ten states with the highest level of dependence on illicit drugs in America. The high rate of illicit drug use has also made Maryland report drug-induced mortalities that exceed the set national average. Of all the abused drugs, Heroine is reported to be the most misused drug in the state (ONDCP, 2012). Other drugs that are highly abused in Baltimore and surrounding regions include marijuana, cocaine, hallucinogens, and methamphetamine. The number of drug-related deaths due to toxicity has been rising significantly in Maryland. As shown by Hogan and Rutherford (2017), the number of total deaths was 815 in 2007 and rose to 2089 in 2016. Therefore, this rise implies the need for the adoption of responsive strategies to address the issue in Maryland.

Resources and Gaps in Service Need

During my agency interview with the KOLMAC Outpatient Recovery Center, several resources, which can be used to managed and support patients with addiction exist (KOLMAC, 2020). The center has a team made up of a physician, Nurse Practitioner, Psychiatrist, registered nurse, and a physical therapist. The psychiatrist usually visits the center once a week. The outpatient center, KOLMAC, has been operating since 1973. Its success in the region can be seen in its six outpatient recovery locations in the DC, Maryland, and Virginia areas. It, therefore, implies that the communities have wide access to their needed care in the region. The other resource is the community primary care centers that enhance the referral services and management of mental health issues in the region. The last resource is the existence of community support programs for the mentally ill. The programs ensure that those suffering from mental health problems receive the social support they need for their health and wellbeing.

Access to health is considered one of the rights to patients. Patients should access high quality, safe, and efficient care irrespective of their backgrounds. However, this is not the case in the outpatient center (Griffith & Tengnah, 2020). According to the branch director interviewed in this assignment, patients who do not have insurance coverage payout of the pocket for the healthcare services they need. This increases the risk of health inequality in the region. Patients who do not have insurance coverage or out-of-pocket money to pay for their health are likely to receive sub-optimal care. According to Glassman, Giedion, and Smith (2017), health inequality is a major problem facing the American healthcare system. Often, people from ethnic minorities experience challenges in accessing their needed care due to the high cost of care. Consequently, the risk of poor health outcomes such as increased mortality and morbidity rates among them are enhanced. Therefore, it makes it important for the healthcare stakeholders in Montgomery County, Maryland to engage in activities that will address the issue of health inequality in the region. This will ensure that patients with mental health problems access the care that they need for their health and wellbeing.

Recommendations

It is recommended that interventions that aim at promoting behavioral and lifestyle change among the population be embraced in Aspen Hill Community. This includes the consideration of programs such as summer park activities that will promote active physical activity among the citizens as a way of preventing the risks of chronic illnesses. This recommendation aligns with the provisions of Maryland State Nutrition, Physical Activity, and Obesity Profile. According to the profile, active physical activity should be encouraged with the adoption of supportive policies. Objectives that support active physical activity among the populations were developed. They include the expansion of transportation choices, increasing peoples’ connectedness with nature, and constructing workable communities (Maryland State Nutrition, Physical Activity, and Obesity Profile, 2012). There is also a need to focus on the promotion of activities that included healthy eating, early screening, and effective management of those suffering from chronic conditions. It is also recommended that the outpatient center consider the adoption of models of healthcare that will lower the cost of care incurred by those suffering from mental health issues. This will include soliciting government funding to reduce the cost of services to the uninsured in the region.

The above recommendations have several implications for nursing practice. Firstly, it will translate into a need for the adoption of interventions that aim at promoting the health of vulnerable populations. This includes engaging in active health education, screening, and treating identified cases that are at risk of the above health problems. It also implies that a reduction in health problems related to obesity and drug addiction. This change will result in an improvement in the quality of care due to a decline in the disease burden in the population. The recommendations also increase the need for nurses engaging in clinical and community-based research to determine how optimum health and outcomes can be promoted in vulnerable populations. Therefore, the adoption of the proposed recommendations will have a significant impact on the nursing profession.

Conclusion

The assignment has revealed the importance of teamwork in the promotion of the health and wellbeing of diverse populations. It has also identified the health needs of the populations that should be addressed to promote their health and wellbeing. Health inequalities exist in access to and utilization of healthcare services. Therefore, it is recommended that models of healthcare that reduce the cost of care and the promotion of healthy lifestyles among those at risk should be encouraged in the region.
