As a registered nurse, I get the privilege to sit front and center, as well as participate in the advancement of the nursing profession and healthcare in general. The National Academies of Sciences, Engineering, and Medicine, The Future of Nursing 2020-2030: Charting a Path to Achieve Health Equity details a comprehensive review for what is necessary in an ever-changing environment of safety, culture, education, leadership, and practice within nursing. Although I see the benefits of all the recommendations within this report, the concept that nurses improve health equity stands out as one of the most effective for creating change within the industry. Nurses play a particular role in screening for social issues that can interfere with an individual’s access to food, a stable home, transportation, basic needs like heat and water and domestic violence (National Academies of Sciences, Engineering, and Medicine, 2021).

This is of particular interest to me as a former emergency department nurse that often conducted these screenings, provided food and temporary shelter to those that did not have the means to acquire it and gain the trust of some at risk of self-harm or domestic violence. Although these short-term provisions did not directly solve any health inequities, it highlighted areas of needs within the community and allowed me to collaborate with nurses in social services and care management to begin bridging these gaps.

A community college in Rapid city has produced thousands of licensed practical nurses(LPN) who are immediately transitioning to Associate Diploma in Nursing (ADN) through their LPN -ADN program and many are later joining online Registered Nurse (RN) to baccalaureate (BSN) programs while already employed. Regarding accreditation, the report further recommended that the accrediting and licensing of nurses should be based on demonstrated mastery of core skills and competencies besides the board examination. I believe this requirement will enable the production of competency sound nurses with hands on and critical thinking skills. I have met new graduate nurses who are very good in theoretical knowledge but lack simple competencies required for the job. Trying to learn a host of new skills and competencies, results in possible frustration leading to loss of passion in the profession which should not be the case.

Some sections of the report suggested that baccalaureate prepared nurse should be the entry level while phasing out associate diploma in nursing training within a stipulated time frame. While BSN prepared nurses are associated with positive patient outcome, lower mortality rates and reduced hospital stay (Blegen et al., 2013), this move will deny the low income earning students from pursuing nursing due to the cost or the distance to the institutions. Therefore, I disagree with the recommendation to phase out the ADN programs instead, reinforce the need for career progression in the nursing curriculum.

By 2030, the nursing profession will look vastly different and will be caring for a changing America (National Academies of Sciences, Engineering, and Medicine. 2021). I believe this is true as a nurse with 20 years of experience, I have seen many changes within the healthcare industry and education whether it is nursing school education or continued education provided by employer.

One of the recommendations for nursing education that I believe will be most effective is to prepare nurses to address social determinants of health (SDOH). This is an especially important recommendation because everyone has some type of challenge such as their income status, social status and/or cultural factors.

When I initiated my nursing career at a hospital in southern Texas, 95% were Mexican American who spoke Spanish and English or only Spanish. I am Mexican American and was well versed with some of the social determinants of health within this population. However, that changed throughout the years of my nursing career and when I moved to Minnesota. Currently, I work at a well-known hospital as a certified wound and ostomy nurse (CWON) where people come from all over the United States and around the world. We see a diverse types of ethnicity, religious, cultures, etc…. Although, we receive continued education twice a year on these topics within our ostomy and wound care departments, I feel it would be essential to prepare us more in depth to address these challenges. This is also important for the student nurses to receive more education and prepare them for when they are facing these difficult tasks in their future nursing careers.

Due to aging, population growth, and a greater insured population following the Affordable Care Act (ACA), physician availability to patients has been recognized as one of the top barriers to meet the healthcare needs of patients in the United States of America (Zhang et al, 2020) With that said, I agree with the evolution of advanced practice registered nurse’s role as mentioned on the report. This is essential to the future of health care access to the population. With physician shortages, advanced practical nurses (APRN) should have the autonomy to practice to the full extent of their education and training, even as they work collaboratively with other health professionals (National Academies of Sciences, Engineering, and Medicine. 2021).

Therefor they are now in worse shape than they would have been if they had received preventative care to begin with. The evolution of the advanced practice registered nurse will greatly benefit those who require preventative care but are just out of reach of the physician’s care. Nurses have a very important role in achieving health equity. For nurses to achieve that goal, they need more education and a healthcare system that will support a work environment and autonomy. Nurses need to conduct research for nursing professionals to help build a culture of health, reduce disparities in people, and help them achieve health equality (National Academies of Science, Engineering, and Medicine, 2021).

Nursing can have a big impact on social determinants of Health (SDOH). Nurses can bring about change in the clinical setting by learning about their community finding social determinants and making changes to help bring about better health equality. Nurses can help bring about change in their organization by learning about the organization’s policies for SDOH and advocating for changes to help social determinants (Scowart, 2023). Nurses can advocate and support organization strategies to implement SDOH. Nurses can also bring together other healthcare organizations to make decisions with patients to see if they are willing to talk about their SDOH. If nurses could get patients to talk about their SDOH they could bring about great changes to help patients overcome or bring about some level of change for their social determinants of health.

I do agree with the reports and how they describe the evolution of the advanced practice registered nurse’s role. The reason that I agree, is because APRNs, can bring about greater specialty areas such as midwives, and nurse practitioners it can help bring healthcare teams to rural areas that do not have physicians. APRNs have worked with patients and can research and find the communities’ SDOH and take their healthcare concerns to the stakeholders to bring about the greatest change.
