This evidence-based project will be conducted among African American adults with heart failure in different community settings within the United States. The program will be implemented by the community health workers (CHWs) through group-based education interventions to ensure better control of and management of heart failure among African American adults. The potential study subjects will be reached through different techniques such as door-to-door visitation by the CHWs, using snowball by informing community members about the characters required for the intervention, requesting from different health facilities to refer patients with required characteristics, asking the patients who have been identified to refer other patients, and using the feedback from surveys to identify the potential subjects.

Fundamentally, the burden of heart diseases is severe and requires patients to take more tasks for their self-management. Therefore, there is need to provide proper time for patient education, which is a crucial intervention for managing heart failure. This project supports the provision of an effective supportive-educational intervention to promote self-care behaviors among African American adults with heart failure. As such, setting ample time for patient education can help in effective management of the condition and prevention of the aggravation of the disease.  Although group education will be offered for the patients, a consideration will also be taken to ensure individualized educational interventions that not only offer support but also consider needed cultural aspects. Consequently, at least eight weeks will be required to complete this project.

Control and management of heart failure among African American adults needs a collaborative multidisciplinary effort in key areas such as physicians, nurses, CHWs, nutritionists, pharmacist, counselors, and physical health trainers. Strategies to Deal with the Management of Any Barriers, Facilitators, and Challenges

Undeniably, this project is likely to face certain barriers, facilitators, and challenges. Therefore, while dealing with the barriers and challenges in controlling and managing heart failure among African Americans, it is necessary to adopt the strategy of ensuring culturally responsive strategies (Gay, 2018). The success of this strategy needs a combination of social, environmental, and regulatory interventions. This strategy works by ensuring the target population and the entire public is educated on risk factors of heart diseases to change the attitude and understanding of the target population. Besides, different strategies such as use of social workers, national campaigns, using peers, public lectures, and using families are also effective in dealing with barriers and challenges while also acting as facilitators of the program.

The Feasibility of the Implementation Plan

Overall, the entire project was anchored on evidence-based practice. According to Driscoll et al., (2016), the project implementation using evidence-based practices tend to succeed because the pertinent evidence is considered while making decisions about the project. Besides, this project opted to use CHWs to reach out to the target population. In turn, the CHWs are expected to foster a close and personal relationship with patients to succeed in their quest. This move to create a personal relationship with patients proves feasible because patients are likely to freely disclose all the information that may be required for the success of the project (Townsend & Morgan, 2017). Taken together, the current implementation program is likely to succeed because of the use of evidence-based practices and the decision to deploy CHWs to work closely with patients which is poised to foster vital elements for controlling and managing heart failure such as reporting, supervision, and leadership. On the other hand, the cost of implementation would be provided by the healthcare facility. However, since heart failure is a growing public health concern, it is easy to source additional funds from the government and other community-based support groups.

Plans to Maintain, Extend, Revise, and Discontinue a Proposed Solution after Implementation

After the implementation, the project can be maintained if it can be proved that the objectives of the project are effectively delivered and used by the target population to gain the optimum the benefits. However, extension of the project can be permitted where the existing situation makes it challenging to realize the anticipated benefits yet the project is still deemed to carry greater benefits. Contrarily, the revision of the project is possible when it is ascertain that additional adjustments can bring more benefits to the project. Finally, discontinuation of the project is allowed where the Post-Implementation review (PIR) is conducted and indicates that the project failed in its objectives and any attempt to continue with the project is futile.

Your assignments in weeks 4 and 5 require early planning as you would need to present a teaching plan and interview a community health provider. Your initial discussion post should be a minimum of 200 words and response posts should be a minimum of 150 words. For your response post, you do not need references as personal experiences would count as response posts. If you however cite anything from the literature for your response post, it is required that you cite your reference. You should include a minimum of THREE references for papers in this course. Please note that references should be no more than 5 years old except recommended as a resource for the class. Furthermore, for each discussion board question, you need ONE initial substantive response and TWO substantive responses to either your classmates or your instructor for a total of THREE responses.
