Additional Examinations or Diagnostic Tests
In the case presented, it will be important to undertake a magnetic resonance imaging (MRI) of the right limb to investigate the extent of the injury. Besides, radiographs and ultrasounds for the same limb will be investigated to view associated injuries (Halloran, 2014). Due to pain in the thigh, joint aspiration will be performed for the patient. Laboratory investigations involving complete cell count, C-reactive protein and erythrocyte sedimentation rate will also be requested to assess the depth of the injury or possible pathogenic infections (Eltorai, Eberson & Daniels, 2017). Hemoglobin electrophoresis will be another investigation to establish the morphology of the red blood cells. Finally, genetic testing for the child will be requested. The approach will be to determine the presence of an inherited infection for the patient.
Differential Diagnosis
The patient presents a condition of septic arthritis related to an infection in the synovial fluid as evidenced by pain in the right thigh and limb (Halloran, 2014). Pain in the joints may be severe and this necessitates prompt treatment.
The second diagnosis for the patient would be Legg-Calve-Perthes Disease (LCPD) due to pain in the right thigh and limb. The
disease readily affects boys between the age of 4 and 10 years (Price & Miller, 2015). LCPD is related to an insufficient supply of blood to the head of the femur leading to an infarction at the epiphysis. The conditions presented by the patient is alike to the manifestations of LCPD.
Finally, Clay is likely to have sickle cell anemia due to pain in the right thigh which is also worsening progressively (In Biller & In Ferro, 2014). The condition affects joints and alters effective mobility as is identified for the patient.

Most Likely Diagnosis
From the case presented, the patient is most likely to have Legg-Calve-Perthes Disease. Pain in the patient’s right thigh and limb could be attributed to the early onset of osteonecrosis or avascular necrosis (Price & Miller, 2015). The tenuous blood supply in the head of the femur is related to an infection or an injury to the site which necessitates prompt treatment.
Treatment Option
In order to relieve pain, administer Motrin 5 mL PO BID for one month. However, surgery in the localized area is recommended if the pain persists (Price & Miller, 2015). The surgical procedures are undertaken to ensure that the femoral head is maintained within the acetabulum to eliminate pain in the capital femoral epiphysis (Brown et al., 2015).
Patient Education, Health Promotion and Anticipatory Guidance
The patient needs to be encouraged to adhere to periods of immobilization by limiting normal physical activity. Besides, Clay may need to walk with crutches in the course of treatment to lessen the duration for healing (Brown et al., 2015). A physical therapy program is recommended for the patient during the healing process. The caregivers or parents should provide support to the child in occasions of intensive exertion to help in movement.

I strongly encourage that you do not wait until the very last minute to complete your assignments. Your assignments in weeks 4 and 5 require early planning as you would need to present a teaching plan and interview a community health provider. Your initial discussion post should be a minimum of 200 words and response posts should be a minimum of 150 words. Be advised that I grade based on quality and not necessarily the number of words you post. A minimum of TWO references should be used for your initial post. For your response post, you do not need references as personal experiences would count as response posts. If you however cite anything from the literature for your response post, it is required that you cite your reference. You should include a minimum of THREE references for papers in this course. Please note that references should be no more than 5 years old except recommended as a resource for the class. Furthermore, for each discussion board question, you need ONE initial substantive response and TWO substantive responses to either your classmates or your instructor for a total of THREE responses. There are TWO discussion questions each week, hence, you need a total minimum of SIX discussion posts for each week. I usually post a discussion question each week. You could also respond to these as it would count towards your required SIX discussion posts for the week.
I understand this is a lot of information to cover in 5 weeks, however, the Bible says in Philippians 4:13 that we can do all things through Christ that strengthens us.
